Static versus dynamic superior augmentation in anterior latissimus dorsi and teres major tendon transfer for anterosuperior irreparable rotator cuff tears with narrow acromiohumeral distances.

INTRODUCTION: This study compared outcomes between static superior augmentation using superior capsular reconstruction (SCR) and dynamic augmentation using arthroscopy-assisted middle trapezius tendon transfer (aMTT) combined with anterior latissimus dorsi and teres major (aLDTM) tendon transfer in anterosuperior irreparable rotator cuff tears (ASIRCTs) with narrow acromiohumeral distance (AHD). METHODS: Thirty-four patients with ASIRCTs and preoperative AHD <6 mm who underwent aLDTM tendon transfer between 2018 and 2023 were retrospectively reviewed. Group S (n = 14) underwent aLDTM with SCR…

Combined anterior latissimus dorsi and teres major transfer and superior capsular reconstruction using autogenous biceps tendon for anterosuperior irreparable rotator cuff tears: technical note.

This technical note describes the surgical technique of combining anterior latissimus dorsi and teres major (LDTM) tendon transfer with biceps superior capsular reconstruction (SCR) for the treatment of anterosuperior irreparable rotator cuff tears (AIRCTs). Anterosuperior irreparable rotator cuff tears, characterized by irreparable tears in the subscapularis and supraspinatus tendons, can lead to significant shoulder dysfunction. The described procedure aims to restore glenohumeral stability, prevent humeral head migration, and optimize shoulder mechanics. The LDTM tendon transfer replicates …

Superior Capsular Reconstruction Technique for Massive Rotator Cuff Tear With Arthroscopic Transfer and Augmentation of Long Head of Biceps Brachii Tendon-The Biobridge Technique.

Massive irreparable rotator cuff tears remain a major surgical challenge, often leading to pain, functional loss, and superior humeral migration. Superior capsular reconstruction has emerged as a technique to restore superior stability. Autologous augmentation using the long head of the biceps brachii tendon offers a cost-effective, biologically viable option, avoiding graft harvest and morbidity. We describe an all-arthroscopic technique of superior capsular reconstruction with long head of the biceps brachii tendon transposition and augmentation called Biobridge technique. The method involve

Transfer of distal peroneus longus tendon to tibialis anterior by retrograde fixation to treat spastic equinovarus foot in adults: Surgical Technique and Preliminary Results.

We describe a retrograde transfer of the distal tendon of the peroneus longus (PL) onto the tibialis anterior (TA) tendon to treat spastic equinovarus foot (SEVF) in adults. The fact that the distal tendon insertions of the PL and TA are a mirror image makes them antagonists. The aim is to divert the distal tendon in front the inactive distal PL tendon, by fixing to the TA in the middle third of the lower leg. This transforms it into a dorsiflexor and reinforces its eversion ability. The suture level helps to avoid skin impingement when wearing shoes, and the complications inherent to transoss

Peroneal perforator-based peroneus longus tendon and sural neurofasciocutaneous composite flap transfer for a large soft-tissue defect of the forearm: A case report.

We describe the use of a composite flap composed of a sural neurofasciocutaneous flap and a vascularized peroneus longus tendon for the reconstruction of severe composite forearm tissue defects in a patient. A 43-year-old man had his left arm caught in a conveyor belt resulting in a large soft-tissue defect of 18 × 11 cm over the dorsum forearm. The extensor carpi radialis, superficial radial nerve, and radial artery were severely damaged. A free neurofasciocutaneous composite flap measuring 16 × 11 cm was outlined on the patient's left lower leg to allow simultaneous skin, tendon, nerve, and

Stem cells and basic fibroblast growth factor failed to improve tendon healing: an in vivo study using lentiviral gene transfer in a rat model.

BACKGROUND: The aim of this controlled study was to investigate the influence of mesenchymal stem cells (MSCs) and lentiviral (LV) expression of basic fibroblast growth factor (bFGF) on tendon remodeling in an in vivo rat model of an Achilles tendon defect. METHODS: In eighty-four male Lewis rats, complete 2.4-mm tendon defects were created and were either left untreated (the phosphate-buffered saline solution [PBS] group) or were treated with mesenchymal stem cells expressing enhanced green fluorescent protein (the MSC-LV-eGFP group) or with mesenchymal stem cells expressing basic fibroblast

Split anterior tibial tendon transfer for varus equinus spastic foot deformity Initial clinical findings correlate with functional results: A series of 132 operated feet.

Spastic equinovarus foot deformity commonly occurs after stroke or head trauma. Management by split anterior tibial tendon transfer to the lateral border of the foot was chosen by the authors. The purpose of that work was to assess the reliability of this method to improve patient's functional autonomy and investigate the relationship between the initial clinical examination findings and the quality of the functional results. 84 feet, operated in 82 patients out of 130, of mean age 46 years, were reviewed at a mean follow-up of 65 months. Very few complications were observed. Toe deformities r…

Lateral Displacement Calcaneal Osteotomy Combined With Anterolateral Transfer of the Tibialis Posterior Tendon for Treating Flexible Cavovarus Foot in an Adult Patient: A Case Report.

The cavovarus foot is a spectrum of foot deformity primarily caused by muscular imbalance and characterized by increased pitch, varus of the hindfoot, plantar flexion of the midfoot, and pronation and adduction of the forefoot. Surgery for cavovarus foot commonly involves some combination of procedures, however, no consensus has emerged regarding their choice. We report the case of a 68-year-old male with a flexible cavovarus foot. The patient complained of pain in the subtalar joint and plantar callosities. For this patient, we performed lateral displacement calcaneal osteotomy (LDCO), antero

Ulnar nerve compression following flexor digitorum superficialis tendon transfers around the ulnar border of the forearm to restore digital extension: case report.

This case report describes a complication that occurred following long and ring finger flexor digitorum superficialis tendon transfers routed around the ulnar border of the forearm to restore digital extension. An ulnar mononeuropathy developed in the early postoperative period that was characterized by decreased ring and small finger sensation and interosseous muscle weakness. Operative exploration demonstrated extrinsic compression of the ulnar nerve by the long and ring finger flexor digitorum superficialis tendons. When superficialis tendon transfers are chosen to restore digital extension…

Thermal and non-thermal effects off capacitive-resistive electric transfer application on the Achilles tendon and musculotendinous junction of the gastrocnemius muscle: a cadaveric study.

BACKGROUND: Calf muscle strain and Achilles tendon injuries are common in many sports. For the treatment of muscular and tendinous injuries, one of the newer approaches in sports medicine is capacitive-resistive electric transfer therapy. Our objective was to analyze this in vitro, using invasive temperature measurements in cadaveric specimens. METHODS: A cross-sectional study designed with five fresh frozen cadavers (10 legs) were included in this study. Four interventions (capacitive and resistive modes; low- and high-power) was performed for 5 min each by a diathermy "T-Plus" device. Achil

The correction of ulnar claw fingers: a follow-up study comparing the extensor-to-flexor with the palmaris longus 4-tailed tendon transfer in patients with leprosy.

PURPOSE: The extensor to flexor 4-tailed tendon transfer (EF4T) and the palmaris longus 4-tailed tendon transfer (PL4T) are 2 surgical procedures used to correct intrinsic paralysis of the hand in leprosy. The EF4T traditionally is the more common procedure and requires the transfer of a wrist extensor muscle. The PL4T requires the transfer of the palmaris longus and morbidity is expected to be lower. A follow-up study was performed to determine whether the clinical outcome of the PL4T is superior to the EF4T procedure in leprosy patients with ulnar claw fingers that are considered mobile befo…

Electromyographic biofeedback training in a cerebral palsy patient undergoing pronator teres rerouting and brachioradialis to extensor carpi radialis brevis tendon transfer surgery: A case report.

Several surgical procedures are used to treat dynamic pronation position of the forearm and flexion deformity of the wrist in cerebral palsy. Postoperative results of pronator teres rerouting were explored, while specially designed postoperative physiotherapy and its outcomes were limited. Herein, we present a case in whom the outcomes of electromyographic biofeedback (EMG-BF) training were assessed after pronator teres rerouting and brachioradialis tendon to extensor carpi radialis brevis tendon transfer combined with derotation osteotomy. The peak value increased, while the resting value dec…

Transfer of abductor hallucis tendon combined with scarf osteotomy versus single scarf osteotomy in moderate to severe hallux valgus deformity: a comparative retrospective cohort study.

BACKGROUND: Scarf osteotomy (SO) was broadly applied in moderate to severe hallux valgus (MSHV), and the results were satisfactory. However, due to the complicated pathologic changes in hallux valgus, the ideal surgical treatment is still controversial. Transfer of the abductor hallucis tendon combined with Scarf osteotomy (TAHTCSO) was an innovative alternative technique. This retrospective cohort study aimed to define if TAHTCSO mode resulted in improved outcomes as compared with the single SO in MSHV. METHODS: Of 73 patients (92 ft) with MSHV, 36 (45 ft) were treated through TAHTCSO and 37
