Is there a role for parenteral nutrition or hydration at the end of life?

PURPOSE OF REVIEW: This review aims to update healthcare providers on the role of parenteral nutrition/hydration in terminal patients and highlight recent research. RECENT FINDINGS: Cachexia is felt to be refractory to treatment at the last stages of life. The majority of terminally ill patients will derive no benefit from parenteral nutrition with some exceptions including patients with a good functional status and a nonfunctional gastrointestinal tract or a slow growing tumor.Dehydration can potentially be reversible in patients at the end of life. However, recent research examining parente

Losing a child to spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a genetic disease that affects mostly children but also some adults. There are Types I, II, and III that affect children, and Type IV that affects adults. There is no cure or treatment. SMA sufferers lack a protein (SMN) that controls muscles, therefore muscles weaken, and in the most severe of cases death occurs. Research is ongoing to find a cure for SMA. This personal account by the mother of Matthew who died aged three months of spinal muscular atrophy aims to inform nurses about this life-threatening condition.

[Palliative sedation].

Terminal illnesses can cause distressing symptoms such as severe pain, mental confusions, feelings of suffocation, and agitation. Despite skilled palliative care in some cases these symptoms may not respond to standard interventions. After all other means to provide comfort and relief to a dying patient have been tried and are unsuccessful, clinical caregivers and patients can consider palliative sedation. Sedation in the context of palliative medicine is the monitored use of medications to induce varying degrees of unconsciousness to bring about a state of decreased or absent awareness in ord

Reinterpreting the controlled substances act: predictions for the effect on pain relief.

On November 6, 2001 U.S. Attorney General Ashcroft reinterpreted the Controlled Substances Act in a manner consistent with the Pain Relief Promotion Act, a bill introduced in the last Congress. This reinterpretation criminalizes actions related to the provision of controlled substances (e.g. opioids, barbiturates) in the event that a patient dies and external observers believe the intent of providing the medication was to hasten the patient's death. This paper analyzes the arguments regarding the need for reinterpretation and the potential consequences of such an action on pain relief. The con

Integrating Telehealth into Community-Based Palliative Care: A Systematic Review.

INTRODUCTION: Community-based palliative care implementation faces challenges in terms of providing services to people living with terminal illness in remote and underserved areas. Integrating telehealth into palliative care has the potential to improve access to and effectiveness of palliative care by enabling remote interactions between people living with terminal illness, their caregivers, and healthcare providers. However, the models and outcomes of telehealth in the context of community palliative care remain poorly understood. OBJECTIVE: The aim of this systematic review was to investig

End-of-Life Care of Young Adult Solid Organ Transplant Recipients Compared to Nontransplant Recipients.

IntroductionLittle is known about end-of-life care of young adult solid organ transplant-recipients and how it compares to care received by nontransplant-recipient young adults with chronic illness.Research Questions(1) What aggressive and palliative care methods are used among young adults by solid organ transplant status (ie, recipient vs nonrecipient), and (2) what factors, including transplant recipient status, predict receiving a palliative care consult in the last 90 days of life.DesignData on young adults (18-39 years) with a history of solid organ transplant (recipient cohort) or cance

Double-effect sedation: do physicians not intend a decrease in consciousness when it is caused by drugs that can also reduce specific symptoms?

The ethics of palliative sedation has been debated intensely. Recently, it has been emphasised that many drugs with sedating effects are also effective in reducing specific symptoms like pain and seizures. For example, midazolam is commonly used to sedate terminally ill patients but it can also reduce seizures. Hence, when midazolam is administered to a patient suffering from seizures, it may also lower the patient's consciousness. Similarly, morphine is useful in the management of end-stage pain and dyspnoea, but can also reduce the patient's consciousness, especially when administered in lar

Preventive medication deprescribing in advanced cancer patients approaching end of life.

Previous reports indicated many patients with advanced cancer and limited life expectancy have ongoing preventive medication prescription (PMP) of uncertain benefit and increased risk. Our review of palliative care oncology admissions found high rates of PMP (69%) at time of first palliative care admission, despite high rates of inpatient deprescription (88%) and death at a median of 16 days (interquartile range 10-45) following admission. Rates of PMPs did not vary by time from last systemic treatment (P = 0.29) or by prior palliative care involvement (P = 0.82). Physicians and the wider mult

[Medically induced death in the European Union and Switzerland : Differences and similarities, ethical issues].

À draft law on 'support for the sick and the end of life' was presented in 2024 in France and debated by the deputies at first reading. The first part concerned 'supportive care and the patients' rights', while the second concerned "active assistance in dying'. With the dissolution of the National Assembly in June 2024, discussion of the proposed law has been suspended. In this summary, we review the development of the legislation governing palliative care in France, from the Laroque circular in 1986 to the Claeys-Léonetti law currently in force. Secondly, we develop the legislation and texts

Palliative care involvement and intensity of end-of-life care among adolescents and young adults with cancer: a population-based study.

BACKGROUND: Adolescents and young adults (AYAs) with cancer often experience high-intensity end-of-life care and low utilization of palliative care. To explore this further, we evaluated the frequency of palliative care involvement and its association with end-of-life care intensity among AYAs with cancer in Ontario, Canada. METHODS: We conducted a retrospective cohort study using health administrative databases in Ontario, Canada, from Jan. 1, 2018, to Dec. 31, 2022. The cohort included AYA cancer decedents, aged 15 to 39 years. We categorized palliative care involvement into lifetime involv

Care-full negotiation of hospital discharge and end-of-life care in an Indonesian palliative care unit.

How do palliative care professionals negotiate end-of-life care with family members when prognosis and dying are not openly discussed? Based on ethnographic fieldwork in an Indonesian palliative care unit, I argue that palliative care professionals employ implicit, ambiguous and culturally sensitive communication to carefully negotiate hospital discharge and discuss end-of-life care. I focus on listening to what is said and what remains unspoken in the embodied communicative practices about end-of-life care in family meetings to discuss hospital discharge. I show how palliative care profession

Upper arm movements in the last days of life: A new possible sign of impending death.

One of the most crucial stages of palliative care is the last days and hours of life, which require special attention and knowledgeable identification of clinical signs described as signs of impending death (SID). Our case series of 11 patients receiving home palliative care describes bilateral hypoactive, stereotyped upper arm movements (scratching of the head, forehead, and nose) that were previously unknown or described, often accompanied by SID.

[New recommendations on palliative sedation].

Palliative sedation aims to relieve refractory suffering in patients with life-limiting disease. The 2009 framework on palliative sedation of the European Association for Palliative Care (EAPC) has recently been updated. Recommendations have also been formulated by the research group SedPall in Germany. This article describes the social and ethical complexity of decision-making and summarises the recommendations. Patient autonomy is emphasised. Refractoriness of the suffering should be determined jointly by physician and patient. Sedation should be proportional, that is to say, its form and du

End of Life in Boys and Young Men With Duchenne Muscular Dystrophy - The Perspective of Dying Men and Their Families: A Systematic Review and Thematic Synthesis of Qualitative Evidence.

Understanding the perceptions and experiences related to the end of life (EoL) of boys and men with Duchenne muscular dystrophy from their own and/or family perspective is limited based on the available qualitative empirical studies. This systematic review was done with a thematic synthesis of qualitative evidence according to the PRISMA Statement Guidelines and the SPIDER search tool. The review included empirical, qualitative, and relevant full-text studies published in 2000-2023 in the EBSCO Discovery Service, ISI Web of Science, Scopus, PubMed, and ProQuest databases. Advance care planning: End-of-life hopes and fears among community dwelling adults.

End-of-life (EOL) medical care in the United States often does not align with patients' goals and preferences. This study explored EOL hopes and fears among 86 community-dwelling adults and examined medical and psychological predictors of death anxiety. Common EOL hopes included absence of suffering, closure, and personal fulfillment. Common EOL fears included suffering, lack of competence, and specific types of death. Fear of the dying process was greater than fear of death itself. Health predicted death anxiety; age alone, did not. Advance care planning and clinical decision making should in
