The endovascular repair of blunt traumatic thoracic aortic injury in Asia: A systematic review and meta-analysis.

INTRODUCTION: Blunt traumatic thoracic aortic injury, the second leading cause of death from trauma, poses unique challenges in its management. However, there has not yet been a corresponding consolidated series of outcomes reported in Asia, which this systematic review aims to address. METHOD: This review was performed as per the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. Searches were performed on MEDLINE, EMBASE, Google Scholar, and Cochrane Database for studies performed in Asia reporting the endovascular management of blunt traumatic thoracic aortic in

Reassessing the cardiac box: A comprehensive evaluation of the relationship between thoracic gunshot wounds and cardiac injury.

BACKGROUND: High-energy missiles can cause cardiac injury regardless of entrance site. This study assesses the adequacy of the anatomic borders of the current "cardiac box" to predict cardiac injury. METHODS: Retrospective autopsy review was performed to identify patients with penetrating torso gunshot wounds (GSWs) 2011 to 2013. Using a circumferential grid system around the thorax, logistic regression analysis was performed to detect differences in rates of cardiac injury from entrance/exit wounds in the "cardiac box" versus the same for entrance/exit wounds outside the box. Analysis was re

Intraoperative neuroprotective interventions prevent spinal cord ischemia and injury in thoracic endovascular aortic repair.

OBJECTIVE: Transient and permanent paraparesis and paraplegia (spinal cord injury [SCI]) are reported in up to 13% of patients undergoing thoracic endovascular aortic repair (TEVAR) for descending thoracic aortic aneurysm, thoracoabdominal aortic aneurysm, and thoracic aortic dissection. We hypothesize that aggressive intraoperative and postoperative neuroprotective interventions prevent or significantly reduce all SCI in TEVAR. METHODS: Using a prospectively maintained, Institutional Review Board-approved database, we retrospectively reviewed all TEVARs performed in a university tertiary ref

Percutaneous pedicle screw fixation for an unstable thoracic spine fracture after a traumatic degloving injury.

Unstable spinal fractures require urgent surgical intervention to relieve compression of the spinal cord, correct spinal deformity, stabilise the spine and prevent further neurological injury. We report the case of a young man with a thoracic chance fracture in the setting of a devastating degloving injury, whose fracture was stabilised using minimally invasive, percutaneous pedicle screw fixation. We discuss the advantages of using a minimally invasive technique for spinal fixation and its role in the treatment of complicated, multisystem trauma patients.

Combined thoracic endovascular aortic repair and neurosurgical intervention for injury due to posterior spine surgery.

Aortic injuries represent a rare but life-threatening complication of spinal surgery. Perforation of the aorta due to pedicle screw penetration or misplacement can lead to immediate bleeding with hemodynamic instability or to pseudoaneurysm development with delayed risk of rupture, which can occur weeks to months later. Recently, thoracic endovascular aortic repair (TEVAR) in aortic trauma has contributed to a reduction of both mortality and morbidity. The literature on this subject is reviewed.

Reappearance of sympathetic skin response below a thoracic level-9 complete spinal cord injury.

Reappearance of sympathetic skin response (SSR) below lesion is reported in a patient with a complete thoracic-9 spinal cord injury 6 months following injury. SSR was elicited by electrical stimulation of supraorbital nerve (SON) and pudendal nerve (PN). SON stimulation induced SSRs only in the hand. SSRs were initially absent below the level of SCI but reappeared only with PN stimulation. This case suggests that 6 months following a complete lesion, the isolated spinal cord can generate a SSR. Possible underlying mechanisms and implications for autonomic plasticity below spinal lesion are dis

Ability of a chest X-ray and an abdominal computed tomography scan to identify traumatic thoracic injury.

OBJECTIVE: Our objective was to show that a chest X-ray (CXR) and an abdominal computed tomography (CT) scan are sufficient to identify most clinically significant thoracic injuries in trauma patients, rendering the thoracic CT scan useful in only a subset of patients. METHODS: A retrospective study identified thoracic injuries in 374 trauma patients evaluated with a CXR, a thoracic CT scan, and an abdominal CT scan. Injuries seen on the initial CXR versus those seen on a CT scan only (occult) were identified and assessed for clinical relevance. RESULTS: An abdominal CT scan identified 65% (…

Traumatic transection of the aorta and thoracic spinal cord injury without radiographic abnormality in an adult patient.

PURPOSE: To describe a rare case of spinal cord injury without radiographic abnormalities (SCIWORA) associated with a traumatic aortic transection. CASE REPORT: A 48-year-old man was transferred for endovascular treatment of a traumatic aortic transection. Primary traumatic paraplegia had been documented at the scene of the accident. No osseous or ligamentous injuries were found in the conventional radiographic images. The transection was treated successfully with an endovascular stent-graft, but the patient remained paraplegic. On the magnetic resonance images 8 days later, discrete trauma o

Injury to the spinal cord by a thoracic epidural catheter used to control postpneumonectomy pain : A case report.

A thoracotomy is one of the most severe painful operations. This severe pain can usually be controlled by thoracic epidural analgesia. Epidural catheterization for analgesia has several complications, e.g., epidural hematoma, abscess, spinal stenosis, spinal infarction, direct cord trauma, and neurotoxicity by chemical contamination. These complications can cause acute paraplegia, but permanent paraplegia is extremely rare. We report a case of paresthesia and temporary paralysis in a 54-year-old patient who suffered spinal cord injury after thoracic epidural catheterization for the control of

A meningeal cyst of the thoracic spine presenting as prolonged paresis after ankle injury: case report.

A case of a thoracic meningeal cyst causing right lower extremity weakness in a fifteen-year-old girl is presented. The diagnosis was delayed due to a history of ankle injury which was initially used to explain her symptoms. The authors explain the difficulty in diagnosing intraspinal meningeal cysts and stress the importance of surgical excision before spinal cord injury progresses.

[Experimental model of ischemic injury of the spinal cord after temporary occlusion of the thoracic artery in the dog].

An experimental model for the ischemic damage of the spinal marrow is described by occlusion of the descending thoracic aorta at a level with the 5th or 6th thoracic vertebrae during 30 min. 17 dogs were operated in general anesthesia. The thoracic aorta was clamped by a Pean's clamp in the 1st group (11 animals). One dog was not paralysed, another dog died after thrombosis of the aorta in the locality of clamping. The occlusion was carried out by means of a tourniquet in a 2nd group (6 animals) and pH, blood gas values, and blood pressure in the axillary artery or the common carotid artery we

Impact of Injury Type Related to the Severity of Thoracic and Chest Wall Trauma in Deployed Military Soldiers.

IntroductionThere remains a paucity of literature surrounding chest wall injury within military trauma. The purpose of this study was to examine the US Military Trauma database hypothesizing differences in chest wall injury patterns and rates given the different mechanisms of injury within this population.MethodsRecords of injuries that occurred between 2004 and 2020 from the US Military Trauma database were evaluated. Patients' injury characteristics were coded based on mechanism of injury (MOI) and injury severity score (ISS). Individuals with secondary injury types (ITs) were coded as combi

Predictors and clinical outcomes of acute kidney injury after thoracic surgery: a retrospective cohort study.

BACKGROUND: Acute kidney injury (AKI), defined as a rapid decline of renal function, is a frequent complication observed after major surgical procedures. In patients undergoing thoracic surgery, the reported incidence of AKI ranges from 5% to 15%. The development of AKI is associated with an elevated risk of postoperative complications, including the need for reintubation, longer hospital admissions, and higher mortality rates. During one-lung ventilation (OLV), intraoperative hypoxemia and hypercarbia, along with systemic inflammation induced by alveolar over-distension, can contribute to dec
