Vitrectomy for vitreous bleeding and tractional retinal detachment in a case of Evans syndrome.

PURPOSE: To evaluate the effect of vitrectomy to treat Evans syndrome with vitreous bleeding followed by tractional retinal detachment. CASE: Vitrectomy was performed on a 21-year-old man with Evans syndrome who developed tractional retinal detachment in the left eye after experiencing vitreous bleeding and tissue proliferation related to the bleeding. OBSERVATIONS: After the proliferative tissue was removed and tractional retinal detachment corrected, the patient's visual acuity improved from 6/20 to 10/20. A small amount of residual proliferative tissue remained after the vitrectomy. CONC…

The efficacy of side arm traction in the reduction of supracondylar fracture humerus in children.

156 consecutive children with supracondylar fracture humerus were reviewed. Of these, 56 children with severely displaced fractures were treated with side arm traction. A retrospective review revealed that a mean post-reduction Baumann angle of 74.2 degrees for boys and 75.9 degrees for girls; and mean post-reduction humero-ulna angle of 11.6 degrees for boys and 12.7 degrees for girls was achieved. A linear correlation was noted between the duration of traction and the age of the patient, older patients requiring longer traction. The region below the line in the graph, plotting the duration o…

Iris diaphragm implantation in post-traumatic aniridia and tractional retinal detachment.

The implantation of an artificial iris diaphragm is described in a case of aniridia and traumatic aphakia with a relapsed, tractional retinal detachment. The addition of heparin and dexamethasone to the infusion liquid is useful to reduce the fibrinous exudation which seems to be the main cause of diaphragm obliteration.

[The effect of traction treatment on blood flow in the immature hip--an animal study].

9 immature pigs (age: 8-12 weeks) underwent longitudinal traction with 10%, 30%, and 50% body weight followed by overhead traction with 10% and 30% body weight. Microcirculation of the femoral head was monitored by a laser-doppler flow probe and macrocirculation within the lateral circumflex artery by an ultrasonic perivascular transit time flow probe. Changes in blood flow during each traction period were compared to the preceding resting period and expressed in percentages. With any type of longitudinal and overhead traction a trend towards an improvement in micro-and macrocirculation of the…

Fracture-dislocation of the thoracic and lumbar spine: advantages of halo-bifemoral traction.

Thoracic and lumbar fracture-dislocations with 100% spinal column displacement are uncommon injuries that usually have associated neurologic injury. Severe spinal malalignment and instability can result in blood loss, tenting of skin and, severe pain. Four patients with thoracic or lumbar fracture-dislocations were initially treated and reduced with halo-bifemoral traction. Fracture reduction with halo-bifemoral traction decreased pain, controlled instability, relieved skin pressure, and prevented the need for acute surgical intervention. In addition, use of halo-bifemoral traction allowed for…

Vitreoretinal traction and perimacular retinal folds in the eyes of deliberately traumatized children.

The pathophysiology of perimacular folds in eyes of deliberately traumatized children is disputed. The authors reviewed the clinical and for … hed to the internal limiting membrane at the apices of the folds and the vitreous base, implicating traction in the pathogenesis of these folds. Although some intraocular findings in deliberately traumatized children may be explained by direct head injury, the possibility of both direct head trauma and shaking must be considered. Perimacular folds may deve …

Traction force profiles associated with the use of a fracture table: a preliminary report.

A load cell was developed that can reliably monitor longitudinal traction forces applied to limbs during several different orthopaedic procedures. This permitted determination of tensile loads and the effects of patient limb positions on tension magnitudes. The relationships between the magnitude and duration of applied tension to functional nerve changes were also studied. This preliminary report reviews traction force data on seven patients for eight operations. Mean peak loads for femoral, tibial, and hip procedures were 686, 356, and 306 N, respectively. Adduction of the limb around a fixe…

Traction bow for acute reduction of fracture and/or dislocation of the thoracic or thoracolumbar spine.

Treatment of acute fractures and/or fracture dislocations of the thoracic or thoracolumbar spine has traditionally involved bedrest or the use of traction devices with external hanging weights, until surgical correction can be accomplished. A fiberglass tubular traction bow with continuous adjustable elastic tension has been designed for the application of skeletal traction. When used to treat thoracic or thoracolumbar fractures and/or dislocations, it can maintain distraction forces in an uninterrupted fashion. Ten patients with acute fractures and/or dislocations of the thoracic or thoracolu…

Posterior tractional retinal breaks complicating branch retinal vein occlusion.

A total of 358 patients with branch retinal vein occlusion (BRVO) were reviewed to examine the relationship between branch retinal vein occlusion with neovascularization elsewhere (NVE), vitreous hemorrhage, and posterior tractional retinal breaks. Twenty-eight of 358 (7.8%) BRVO patients had vitreous hemorrhage. In this group of 28 patients, 24 of the 28 (85.7%) had NVE. Of the 24 BRVO patients with vitreous hemorrhage and NVE, six (21.4%) were found to have posterior tractional retinal breaks adjacent to avulsed neovascular tissue. These data indicate a strong association between BRVO patien…

Fragmentation of the proximal pole of the patella. Another manifestation of juvenile traction osteochondritis?

Six boys with fragmentation of the proximal pole of the patella are reported; the condition was bilateral in one. Four of the six presented with symptoms of Osgood-Schlatter's disease or Larsen-Johansson disease of the same or of the contralateral knee, but they had no symptoms or signs relating to the proximal pole of the patella; one also had features suggesting minimal chondromalacia patellae. Two boys had no objective abnormality in either knee. It is suggested that the fragmentation may be a further form of traction osteochondritis of the attachments of the quadriceps mechanism. Attention…

[Pre-operative retina reapplication by active immobilization of the eye with traction sutures].

Twenty-five cases of retinal detachment were treated pre-operatively by the so-called active immobilization method described by Rosengren and Algvère. This procedure can be employed for all types of detachment, and gives significant reapplication in 80 p. cent of cases, simplifying subsequent surgery. Complications are rarely observed and can be easily prevented.

[Is treatment of C.D.H. by progressive reduction by traction still advisable? (author's transl)].

The authors analyse the results that they have obtained in the treatment of C.D.H. either by conservative or surgical methods. The prevention of osteochondritis remains the main worry. However traction leads to long-lasting immobilization and sometimes it is not sufficient. The authors think that osteochondritis may be related to hyper-pressure on the femoral head or to lesions of the vascular pedicle and that in some cases shortening of the femur or tenotomies may allow a decrease of the plaster cast period. The indications on capculotomy, rotation osteotomy, salter osteotomy are discussed. I…

Clinical Outcomes of Ocular Toxoplasmosis-Associated Rhegmatogenous and Tractional Retinal Detachment.

Purpose: To investigate the incidence, characteristics, and outcomes of ocular toxoplasmosis-associated rhegmatogenous or tractional retinal detachment (RD). Methods: This was a single-center, interventional retrospective case series of consecutive patients diagnosed with ocular toxoplasmosis who subsequently developed rhegmatogenous RD (RRD) or tractional RD (TRD) between January 2013 and October 2022. Patients who were followed for at least 3 postoperative months were included. The location of the retinochoroidal scar, anatomy of the RD, surgical techniques used for repair, and anatomic and …
