Preclinical models for translating regenerative medicine therapies for rotator cuff repair.

Despite improvements in the understanding of rotator cuff pathology and advances in surgical treatment options, repairs of chronic rotator cuff tears often re-tear or fail to heal after surgery. Hence, there is a critical need for new regenerative repair strategies that provide effective mechanical reinforcement of rotator cuff repair as well as stimulate and enhance the patient's intrinsic healing potential. This article will discuss and identify appropriate models for translating regenerative medicine therapies for rotator cuff repair. Animal models are an essential part of the research and

Patient-Derived Organoids in Functional Precision Oncology: Tumor Modeling, Drug-Response Prediction, and Clinical Translation.

Patient-derived organoids (PDOs) have emerged as an effective experimental platform for bridging the long-standing gap between traditional in vitro cancer models and the biological complexity of actual tumors. These three-dimensional cultures (3D), derived directly from patient tissues, preserve essential architectural, genomic, and phenotypic characteristics of their tumors of origin, enabling more accurate modeling of tumor heterogeneity and disease evolution. Over the last decade, rapid methodological advances in organoid culture systems, extracellular matrices (ECMs), and growth factor opt

RNA therapeutics in rheumatoid arthritis, systemic lupus erythematosus, and systemic sclerosis: a compartment-aware translational framework.

Rheumatoid arthritis (RA), systemic lupus erythematosus/lupus nephritis (SLE/LN), and systemic sclerosis (SSc) remain major causes of chronic inflammation, disability, organ damage, and treatment burden despite therapeutic advances. RNA therapeutics offer programmable transcript suppression, network-level microRNA modulation, transient restoration of protective proteins, and targeting-ligand functions. Existing reviews largely catalogue modalities or individual delivery systems; this review instead asks whether a candidate forms an interpretable translational program across target biology, dis

Immunomodulatory potential of Mongolian medicine in sepsis: current evidence, challenges, and translational perspectives.

Sepsis is a heterogeneous, time-dependent syndrome in which inflammatory injury, immune suppression, endothelial dysfunction, metabolic disturbance, and barrier failure may coexist. This narrative review evaluates whether current pharmacological evidence provides a defensible basis for investigating Traditional Mongolian Medicine as an adjunctive source of immunomodulatory candidates in sepsis. A concise account of sepsis immunopathology is used as an interpretive framework, after which evidence from named Mongolian formulas, extracts, medicinal materials, and isolated constituents is mapped a

Skull Bone Marrow as a Cranial Neuroimmune Reservoir in Alzheimer's Disease: Amyloid, Osteoimmune Remodeling, and Translational Imaging Evidence.

Alzheimer's disease (AD) has traditionally been conceptualized as a disorder driven primarily by cerebral amyloid-β accumulation, tau pathology, synaptic failure, vascular dysfunction, and chronic neuroinflammation. However, emerging evidence suggests that AD pathobiology may also involve extracerebral cranial compartments positioned at the interface between the brain, meninges, cerebrospinal fluid, and skull bone marrow. Recent anatomical and experimental studies have identified direct vascular and osseous channels connecting skull bone marrow with the dura mater, allowing bidirectional excha

Organoids in translation: a bench-to-bedside framework for pancreatic cancer precision medicine.

INTRODUCTION: Pancreatic ductal adenocarcinoma (PDAC) is one of the most lethal malignancies with a 5-year survival rate of < 13%. Standard treatments such as FOLFIRINOX or gemcitabine/nab-paclitaxel yield modest response rates, underscoring the urgent need for precision oncology approaches. Patient-derived organoids (PDOs) preserve the genomic, phenotypic, and histopathological features of the source tumor and offer a promising platform for drug screening, biomarker development, and personalized therapy. However, a systematic evaluation of their translational capacities is lacking. METHODS:

Reframing precision nutrition in irritable bowel syndrome: a mechanism-informed conceptual framework for responder prediction and clinical translation.

BACKGROUND: The low-Fermentable Oligosaccharides, Disaccharides, Monosaccharides and Polyols (FODMAP) diet is widely used for irritable bowel syndrome (IBS), but response varies markedly across patients. This heterogeneity has shifted the field from testing average efficacy toward forecasting individual benefit and translating microbiome science into practical precision-nutrition tools. METHODS: We present a conceptual analysis grounded in evidence mapping from human IBS studies that paired dietary interventions (primarily low-FODMAP pathways) with baseline microbiome and/or multi-omics measu

Synthetic lethality in cancer therapy: Mechanisms, models and clinical translation for overcoming therapeutic resistance.

BACKGROUND AND RATIONALE: Synthetic lethality (SL)-based strategies hold significant promise for overcoming therapeutic resistance, a critical bottleneck in cancer treatment where cancer cells evade anticancer therapies, leading to diminished efficacy or treatment failure. The core of SL lies in exploiting tumour-specific vulnerabilities: drug-resistant cells often acquire unique genetic defects or compensatory adaptive responses, and SL strategies selectively target genes or pathways dependent on these vulnerabilities to induce specific cell death, thereby reversing resistance. From Unmet Medical Need to Drug Candidate: A Translational Therapeutic Development Roadmap Illustrated by Dual-Payload Antibody-Drug Conjugates.

Transformative therapeutic innovation should not begin with a molecule-or even a molecular target. It should begin with a clearly defined unmet clinical need. Here, we present a seven-step Translational Therapeutic Development Roadmap that systematically connects an unmet medical need to a developable drug candidate through the disciplined sequence of (i) defining the need, (ii) understanding disease and resistance biology, (iii) building a mechanistic hypothesis, (iv) defining a target product profile (TPP), (v) molecular design and experimental validation, (vi) developability and manufactura

Natural products in lymphoma therapy: Integrating mechanisms, tumor microenvironment modulation, and translational challenges.

Lymphoma is a heterogeneous group of malignant neoplasms originating from the lymphohematopoietic system, with relapse, drug resistance, and treatment-related toxicity remaining major clinical challenges. Owing to their structural diversity, capacity to modulate multiple signaling nodes, and potential to reshape the tumor immune microenvironment, natural products have emerged as a promising source for anti-lymphoma drug development and for sensitizing combination regimens. This review summarizes the pharmacological activities, mechanisms of action, preclinical evidence, and translational limit

Micro- and Nanoplastics in the Human Brain: Mechanistic Plausibility, Translational Challenges, and Links to Neurological Disease Trends.

The exponential growth in plastic production since the mid-twentieth century has led to the pervasive presence of micro- and nanoplastics (MNPs) across ecosystems and human exposure pathways, coinciding with a rising global burden of neurological disorders. Increasing evidence demonstrates that MNPs are not confined to peripheral tissues but can accumulate even in the human brain, raising concerns about their potential contribution to neurological disease. This structured review synthesizes global trends in plastic production, environmental MNP burden, and human exposure, together with emergin

Central Sympathetic Nerve Activation-Mediated Hypertension: Target Mechanisms and Multimodal Interventions-From Basic Research to Clinical Translation.

Hypertension is the leading global risk factor for cardiovascular diseases, and its pathogenesis is closely linked to excessive sympathetic activation, which markedly elevates the risk of stroke, heart failure and other adverse cardiovascular events. Traditional therapies mainly target peripheral mechanisms, whereas the clinical efficacy of renal denervation highlights the critical role of central regulation in sympathetic hyperactivity. This review focuses on the core sympathetic nuclei including the rostral ventrolateral medulla (RVLM) and paraventricular nucleus (PVN), with epigenetic regul

Podometrics as a translational platform for kidney health and disease: Oshima award address 2025.

Podocyte injury and loss are pivotal early events in the development and progression of chronic kidney disease (CKD). As podocytes have a limited capacity for regeneration and proliferation, quantitative changes in podocyte number, size, and density are critical determinants of kidney health and disease. Experimental studies have shown that podocyte depletion alone is sufficient to induce albuminuria and glomerulosclerosis and that increased podocyte volume occurs in response to glomerular hypertrophy and various stresses on podocytes. These findings led to the emergence of podometrics (i.e.,

Advancing translational research in binge-eating: Integrating insights from clinical practice into animal models.

Binge-eating behaviors are key components of several types of eating disorders, yet their etiology remains unclear. Animal models have provided valuable insights by enabling experimentally controlled investigations of biological, behavioral, and environmental factors contributing to eating disorders. This narrative review examines the clinical relevance of animal models in advancing our understanding of binge-eating-related disorders. We propose five translational priorities focused on clinically-meaningful features of binge eating and their relevance for improving animal-model development: (1

Interleukins in Major Depressive Disorder: Lessons From Autoimmune Diseases and Pathways to Clinical Translation.

BACKGROUND: Major Depressive Disorder (MDD) is a leading cause of disability, and limitations of the monoamine hypothesis have driven the exploration of complementary models, including the inflammatory hypothesis. This hypothesis suggests that in a subset of patients, immune dysregulation, especially involving interleukins (ILs), contributes to depression. This review compares IL dysregulation in MDD and autoimmune diseases to identify common and unique inflammatory mechanisms for treatment. RESULTS AND CONCLUSION: Some MDD patients exhibit chronic, low-grade inflammation both systemically (i

Regulatory T cells in autoimmune diseases: biological mechanisms, clinical translation, and translational challenges.

Autoimmune diseases (ADs) are a group of inflammatory disorders triggered by aberrant immune responses against autoantigens and the breakdown of immune tolerance. Current therapeutic strategies, such as glucocorticoids and immunosuppressants, exert largely non-specific immunomodulation and are frequently associated with substantial adverse effects upon long-term administration. As such, these approaches are often insufficient to re-establish immune homeostasis. In recent years, regulatory T cell (Treg)-based therapeutics have undergone rapid advancement, with continuously updated therapeutic m
