Sacral stress fracture in a distance runner.

Sacral stress fractures are uncommon injuries that are associated with repetitive load-bearing activities. The authors describe a 23-year-old male cross-country runner who presented with low back and buttocks pain. Radiographic findings were unremarkable, but a hop test identified a sacral stress fracture. The patient was instructed to stop running and to take calcium and vitamin D supplements. Four months later, his symptoms completely resolved and he began running again. The authors also present a review of the literature, which revealed that patients with sacral stress fractures are likely

Child abuse and smoking among young women: the importance of severity, accumulation, and timing.

PURPOSE: We examined the association between severity, accumulation, and timing of abuse in childhood and adolescence and smoking status among young women. METHODS: Retrospective self-reported childhood abuse was ascertained with the modified Conflict Tactics Scale from 91,286 Nurses Health Study II participants in 2001 (68,505 returned; 75.0% response rate). Childhood abuse was categorized by severity (mile/moderate/severe), type (physical/sexual), and timing (childhood/adolescence). Smoking status during adolescence was reported at baseline (1989). Logistic regression was used to predict sm

Bench-to-bedside review: oxygen debt and its metabolic correlates as quantifiers of the severity of hemorrhagic and post-traumatic shock.

Evidence is increasing that oxygen debt and its metabolic correlates are important quantifiers of the severity of hemorrhagic and post-traumatic shock and and may serve as useful guides in the treatment of these conditions. The aim of this review is to demonstrate the similarity between experimental oxygen debt in animals and human hemorrhage/post-traumatic conditions, and to examine metabolic oxygen debt correlates, namely base deficit and lactate, as indices of shock severity and adequacy of volume resuscitation. Relevant studies in the medical literature were identified using Medline and Co

Lung contusion-lacerations after blunt thoracic trauma in children.

Chest trauma in children is an indicator of injury severity and is associated with a high mortality rate. The aim of this study was to investigate the impact of pulmonary contusion-laceration on short and long-term outcome of pediatric patients after blunt thoracic trauma. A retrospective analysis of records of 41 children aged 10 months to 17 years who were treated for pulmonary and associated injuries between 1986 and 2000 was done concerning mode of injury, types of injuries, management and outcome. In addition, a follow-up investigation was performed 4.5+/-1 years after injury. Of the pati

Tissue and serum sialic acid levels in an animal head injury model.

BACKGROUND: Significant elevations of serum sialic acid level have been documented in various diseases including in a variety of central nervous system disorders. But, in head injury, there is no any study on the serum and brain tissue sialic acid levels. So, we planned an experimental study to evaluate serum and brain tissue sialic acid levels in head injury. METHODS: Marmarou's impact-acceleration model was used in rats to produce diffuse brain injury. Rats were divided into equal three groups. In Group I, 450 g weight was fell from 1 m height to heads of subjects, and from 2 m in Group II.

Operative management of the flail chest.

The management of flail chest (FC) has been the subject of controversy for many years. The aim of our study was to compare results of the management of patients with FC after operative stabilization (OS) vs. nonoperative treatment (NT). One hundred and thirty-three consecutive patients with FC were assigned to treatment with OS (40 patients) and NT (93 patients). Different kinds of external extramedullar osteosynthesis were performed within 24 hour period after admission. NT includes endotracheal lung ventilation, epidural and regional anaesthesia. No difference in age, sex, shock ISS, severit

Assessment of vision and visual attention in minimally responsive brain injured patients.

Sensory, motor, and cognitive assessment is a critical priority in the care of minimally responsive brain-injured patients. However, their inconsistent pattern of responding may interfere with the use of traditional forms of assessment such as the mental status or neurological examinations. In this article we describe a quantitative method, based on single-subject experimental design, for evaluating vision and visual attention in this patient population. In a series of randomized trials, combinations of a blank card, a colorful photograph, and no stimulus are presented to either or both visual

Evaluation of the cushion effect in blunt abdominal trauma patients: A computerized analysis.

BACKGROUND: Obesity may serve as a protective factor in blunt abdominal trauma (BAT) patients due to a "cushion effect". In this study, we aim to use computed tomography (CT) scans to measure abdominal adiposity and its correlation with injury severity in BAT patients. METHODS: We conducted a retrospective analysis of male BAT patients who had undergone CT scans. Subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT) were manually measured and height-normalized for analysis at lumbar levels L2 and L3. Statistical methods were used to compare differences in adiposity between patie

England-wide injury-severity analysis of e-scooter riders using a Bayesian spatial field model.

As electric scooter (e-scooter) use has expanded, understanding the factors associated with e-scooter rider injury severity has become increasingly important for road safety policy. This study analyses 2,128 crashes involving e-scooters and motor vehicles across England (2020-2023) to identify factors associated with severe and fatal injuries to e-scooter riders. Using the geographic coordinates of crashes, we developed a Bayesian spatial field model implemented via the Stochastic Partial Differential Equation (SPDE) approach for fast Bayesian estimation. Our approach accounts for spatial unob

Sports injury or trauma? Injuries of the competition off-road motorcyclist.

A prospective analysis of the injuries of off-road competition motorcyclist at four International Six Day Enduro (ISDE) events was performed utilizing the injury severity score (ISS) and the abbreviated injury scale (AIS). Of the 1787 participants, approximately 10% received injuries that required attention from a medical response unit. The majority (85%) sustained a mild injury (mean ISS 3.9). Loss of control while jumping and striking immovable objects were important risk determinants for serious injury. Although seasoned in off-road experiences, mean 15.3 years, 54% of those injured were fi

Exposure, relaxation, and rescripting treatment for trauma-related nighmares.

Trauma victims frequently report sleep disturbances, including nightmares, following traumatic events. Research indicates that nightmares are associated with the level and severity of other indices of distress. The recognition of the prominence of trauma-related nightmares, in prevalence and impact, has in part heightened efforts to develop and assess techniques to alleviate these difficulties. The majority of such previous efforts were predominantly case studies or uncontrolled group studies; however, recently several controlled studies have been conducted. The treatment with the most empiric
