A female patient in her 70s. She was referred to our hospital with a huge left breast tumor. She was diagnosed with locally advanced Stage Ⅳ triple negative breast cancer. She refused chemotherapy that caused hair loss. Locoregional surgery and S-1 made it possible to continue treatment while maintaining a good quality of life.

We presented a 73-year-old patient with a history of hospitalizations for heart failure as well as treatment for arterial hypertension and permanent atrial fibrillation and who was found to have cor triatriatum sinistrum in combination with bicuspid aortic valve. Patient refused surgical correction, but his condition improved on conservative therapy for heart failure and atrial fibrillation.

The objective of this study was to evaluate intravesical gemcitabine in high-risk nonmuscle-invasive bladder cancer (NMIBC) refractory to bacillus Calmette-Guérin (BCG). Effective treatment of opioid use disorder (OUD) must target both the medical and psychosocial aspects of a patient's condition. This, in turn, requires a collaboration between medical providers and social supports. We would like to illustrate a key difficulty in this collaboration for some patients in our country: many post-discharge recovery houses continue to refuse to allow patients to remain on medication treatment for OUD (M-OUD). This barrier to M-OUD access in recovery houses is a significant obstacle to effective OUD treatment.

OBJECTIVE: We report the case of an external lymphatic fistula that appeared through an abdominal drainage after laparoscopic resection of the rectum and sigmoid colon for cancer, with lymphadenectomy. There was no chylous ascites because of controlled external drainage.
METHODS: The fistula was managed conservatively by interruption of oral feeding and administration of total parenteral nutrition and subcutaneous octreotide and management then shifted to intravenous somatostatin, with decreased lymph output through the fistula to 100 to 200 mL/d.

Eosinophilic fasciitis (EF) is a rare connective tissue disorder characterized by symmetrical sclerodermatous skin changes primarily affecting the extremities and histologically, by thickening of the fascia with chronic inflammatory infiltrate containing eosinophils. EF is associated with peripheral eosinophilia, hypergammaglobulinemia, and an elevated ESR. Reported is a case of EF in a 57-year-old Japanese-American woman who refused treatment with prednisone, review of other treatment options, and discussion of key differences between this disease and scleroderma.

The aim of the study was to evaluate the course of vestibular schwannoma (VS) when surgery was not attempted. The tumor may be slowly growing and surgery a risk for a patient. Twenty-eight patients out of 390 VS patients during years 1981-1995 were primarily recommended a non-surgical treatment. Another 3 patients refused the operation. BACKGROUND: We report three patients (respectively 36, 52 and 52 years old) with a long history of hidradenitis suppurativa associated with pubic lymph oedema.
CASE REPORT: Dramatic pubic oedema was noted, with "rugby-ball and bag-like" tumefaction associated with nodules and fistula. There was no evidence of lymph node involvement or abdominal lymphatic blockade. One patient underwent surgical excision combined with split-thickness graft and complete healing ensued. Pathological examination of the excised tissue showed telangiectasia and fibrosis.

This exploratory analysis compared adherence to antiretroviral medication on days that illicit drugs were used and on matched abstinence days in twelve subjects who used MEMS caps during participation in a clinical trial. OBJECTIVE: Given the extreme rarity of primary diffuse large B-cell lymphoma of the prostate (P-DLBCL-P), medical institutions face challenges in accumulating a sufficient number of cases. Thus, investigating the clinical manifestations of this disease and exploring potential therapeutic strategies holds significant importance.
