Tuberculin skin test conversion among HIV patients on antiretroviral therapy in Uganda.

SETTING: A human immunodeficiency virus (HIV) clinic in a setting of high tuberculosis (TB) and HIV prevalence. OBJECTIVE: To study the incidence of and factors associated with tuberculin skin test (TST) conversion in HIV patients on antiretroviral therapy (ART). DESIGN: Prospective cohort study of TST-negative, ART-naïve HIV patients (CD4 cell count < 250 cells/l) without active TB. TST was repeated at 2 months and, if negative, at 6 months. TST positivity was defined as an induration of ≥5 mm. Clinical examination, chest X-ray and CD4 cell counts were performed at baseline and follow-up. P

Isolations of Bwamba virus from south central Uganda and north eastern Tanzania.

BACKGROUND: Bwamba virus (Genus Bunyavirus, family Bunyaviridae) is widely distributed in Africa. It causes many unidentified fevers because of its benign nature. OBJECTIVES: Samples of blood from patients were received at Uganda Virus Research Institute for diagnosis and confirmation of infections. Mosquito collections obtained in the O'nyong-nyong virus fever epidemic in Rakai in 1997 were also investigated in an effort to confirm the vectors of O'nyong-nyong virus fever. METHODS: Patientsapos; serum and aliquots of mosquito pools were inoculated into the brain of 1-day old albino mice for…

Implementation of multisectoral interventions to improve nutrition in Uganda: stakeholder perceptions and experiences.

BACKGROUND: Malnutrition in sub-Saharan Africa remains a persistent public health challenge driven by food insecurity, inadequate health services, suboptimal caregiving practices, and weak livelihoods. Addressing these complex determinants requires multisectoral interventions that integrate nutrition, health, agriculture, and community development. The Uganda Community Connector (UCC) programme was designed to address these challenges through integrated, community-based approaches. This study explored stakeholder perceptions of UCC implementation strategies using the RE-AIM framework. METHODS…

The continuing relevance of Mary Ainsworth's classic, Infancy in Uganda (1967/2026).

This article introduces the reissue of Mary Ainsworth's (1967/2026) Infancy in Uganda and provides context for reading the book today. We reflect on the study's cultural and intellectual background and the insightfulness of Ainsworth's detailed case studies of infants and their families. We then focus on what she identified as the book's chief contribution: its account of attachment development. Reading Infancy in Uganda in light of subsequent developments in attachment theory and research, we see early formulations of questions that continue to shape the field. Infancy in Uganda presents infa…

Spatiotemporal and clinical characterisation of acute flaccid paralysis in Uganda, 2016-2023.

INTRODUCTION: Uganda, in line with global efforts to eradicate polio by 2026, conducts surveillance for acute flaccid paralysis (AFP) among children <15 years to enable the timely detection of poliovirus. We analysed national AFP surveillance data to characterise case demographics, spatiotemporal patterns and clinical outcomes, and to evaluate surveillance performance and progress towards eradication. METHODS: We conducted a retrospective descriptive analysis of routine AFP surveillance data from 2016 to 2023. Variables included timeliness indicators, non-polio AFP (NPAFP) rates, stool adequa…

Sex differences in alcohol use and perceptions of cognitive health in rural Uganda.

BACKGROUND: Dementia is a significant public health issue affecting over 50 million people worldwide. Alcohol consumption has been identified as a modifiable risk factor for dementia and its progression. This study aimed to explore the sociocultural factors influencing sex differences in alcohol consumption and community perceptions of its potential cognitive effects, including dementia risk in rural Uganda, where alcohol use is prevalent. METHODS: We employed a qualitative study design in the Ibanda and Lira districts, guided by the Gender Analysis Framework (GAF). We conducted in-depth inter…

Climate change, livelihoods, gender and violence in Rukiga, Uganda: Intersections and pathways.

Climate change disproportionately affects poorer countries like Uganda, intensifying poverty and livelihood stress, which can escalate gender-based violence (GBV). Although the parent study was not designed to focus on GBV, GBV emerged repeatedly during interviews and focus groups; this paper presents a GBV-focused thematic analysis of those narratives. Particularly, we examine how GBV interconnects with poverty, shifting gender roles, alcoholism, environmental stress, and family planning dynamics. Between April and July 2021, we conducted an exploratory qualitative research study that compris…

Barriers and opportunities to addressing social determinants of malaria in Wakiso District, Uganda.

Malaria remains a significant public health threat in sub-Saharan Africa, particularly in Uganda, where it is the leading cause of morbidity and mortality. There is limited evidence on factors related to social determinants within ongoing malaria control efforts in the country. This study explored barriers and opportunities in addressing the social determinants of malaria in Uganda. A qualitative study was conducted in Wakiso district. Data were collected through 10 focus group discussions with community members and community health workers (CHWs), 11 in-depth interviews with heads of househol…

Nurses and Midwives Mistakes and Errors in Pediatric Health Outcomes in Southwestern Uganda.

BACKGROUND: According to the World Health Organization, nearly 1 in 10 patients suffer harm during healthcare, resulting in over 3 million deaths in low- and middle-income countries. Alarmingly, more than half of these incidents are preventable, with medication errors being the primary cause. Children are particularly vulnerable to these errors and other adverse patient events. Therefore, this study explored nurses and midwives' mistakes and errors in paediatric health outcomes in southwestern Uganda. METHODS: We adopted a phenomenological qualitative research design. Purposive and consecutiv…

A socioecological model of skilled birth attendant perspectives on gastroschisis in southwest Uganda.

BACKGROUND: Gastroschisis, a congenital abdominal wall defect, is increasingly reported in sub-Saharan Africa, yet survival remains poor in low-income and middle-income countries, such as Uganda. Barriers to care extend beyond clinical limitations, with societal stigma and cultural beliefs influencing health-seeking behavior. This study explores the sociocultural perspectives surrounding gastroschisis in southwest Uganda using a socioecological model. METHODS: A qualitative study was conducted with midwives and nurses from public and private facilities across 9 districts in Uganda. Participan…

Development of a gastroschisis teaching model to enable surgical task sharing in Uganda.

INTRODUCTION: Gastroschisis mortality is less than 5% in high-income countries, but remains 60-95% in Africa, partly due to absence of prena … s-on training. The model was piloted with nurses, midwives, pediatricians and pediatric surgeons in Uganda and Rwanda. The model was created using a plastic teaching doll, which replicates newborn anatomy and contains a hollow torso, necessary for securing the simulated intestines. A 3 cm defect was created on the anterior abdominal wall of the doll, to …

Oxygen blenders for prevention of retinopathy of prematurity are highly cost-effective in Uganda.

Blended oxygen has been shown to reduce severe retinopathy of prematurity (ROP) incidence worldwide, and more recently in Uganda. Although it has been established that ROP-driven visual impairment has a substantial societal burden and that screening and treatment programs are cost-effective in low- and middle-income countries, there is limited evidence for the economic impact of prevention with appropriate oxygen management equipment (OME) in these regions. We performed a cost-utility analysis of OME implementation comparing variations ranging between most and least favorable values. Disabilit…

Central Nervous System Toxoplasmosis is an Under-Recognized Opportunistic infection in Uganda.

In Uganda, Toxoplasma meningoencephalitis remains underdiagnosed due to the low sensitivities and specificities of available diagnostics. In our recent publication, we identified 15 cases of possible Toxoplasma gondii meningoencephalitis by cerebrospinal fluid metagenomic next-generation sequencing in patients with suspected meningitis. We herein discuss, in detail, these cases to highlight the ongoing limitations of utilizing clinical symptoms to diagnose Toxoplasma gondii meningoencephalitis, the importance of access to rapid diagnostics, and the frequency of toxoplasmosis as a possible co-i…

Prevalence and Correlates of Steatotic Liver Disease Among People With HIV in Uganda.

BACKGROUND: Liver disease is an important cause of morbidity and mortality among people with HIV (PWH). Steatotic liver disease (SLD) is a l … atio, 2.96; 95% CI, 1.00-8.70; P = .048). CONCLUSIONS: We found a low overall prevalence of SLD in Uganda, but older PWH undergoing antiretroviral therapy had a higher prevalence of SLD than those without HIV. Variation in the prevalence of SLD in the African region may be due to the variability in the methods used.

Investigating the Sudan virus outbreak in Uganda through the deployment of a mobile laboratory.

BACKGROUND: Uganda faced its sixth reported Ebola outbreak between September 2022 and January 2023, the fifth to be recorded as Sudan Virus Disease (SVD) in the country. In response to this, the Ugandan Ministry of Health (MoH), rapidly deployed a mobile laboratory from the National Health Laboratory and Diagnostics Services (NHLDS) within one week of the declared outbreak. Here we describe the deployment of the mobile laboratory to Mubende, the outbreak epicentre, as part of the national response. This provided (1) efficient diagnostics and characterization of Sudan virus cases to support nat…
