We describe a child with the rare clinical entity of female pseudohermaphroditism, accessory phallic urethra, and posterior cloaca who was successfully treated with posterior sagittal anorectovaginourethroplasty. Masculinization was limited to the external genitalia, and no chromosomal, metabolic, or adrenal abnormalities were detected. Associated pathology included bilateral vesicoureteric reflux, a non functioning dysplastic kidney, and bicornuate uterus. The investigation and surgical management of this particularly challenging combination of anomalies is detailed.

Leiomyoma of the urethra is a relatively rare condition often presenting as an anterior vaginal wall mass or a mass that protrudes from the urethral meatus. We report on the urodynamic changes after vaginal surgery to remove a suburethral leiomyoma, which protruded from the vaginal orifice resulting in dysuria and dyspareunia. Between 1982 and 1990, 4 males with extensive intraurethral warts presented with marked inflammation of the external genitalia. When sepsis had settled all patients underwent ventral urethrotomy, in which the urethra was divided median as far back as was necessary to expose all of the warts. Surgical and electro-excision were the preferred methods of removal. Of 3 patients who developed recurrences 2 were lost to follow-up. Twelve patients with traumatic posterior urethral stricture have been treated using a combined transpubic-transperineal approach. The strictured segments were long and associated with complicated problems in 4 patients. Three of them have had bulbo-vesical anastomosis operation, and iatrogenic urethro-rectal fistula was encountered in the fourth patient. Combined approach provided the best chance for success. Our results of urethral lumen patency and continence of urine were excellent. However, we encountered 7 patients dissatisfied with their erection.
