The work analyzes alterations of gastric secretion in 687 patients subjected to various kinds of vagotomy in combination with or without draining operations on the stomach for ulcer of the duodenum. Draining operations were established to decrease activity of gastric secretion. During a 4 year period, we found no objective evidence that parietal cell vagotomy contributes to the production of gastroesophageal reflux. The fact that patients without reflux preoperatively achieved the same satisfactory clinical results whether parietal cell vagotomy was or was not accompanied by an antireflux procedure supports this view. Therefore, in the absence of reflux, we cannot recommend the prophylactic use of an antireflux procedure in combination with parietal cell vagotomy in the treatment of patients with duodenal ulcer. Antral mucosal immunoreactive gastrin (IRG) and somatostatin-like immunoreactivity (SLI) were measured in separate prepyloric mucosal canine specimens obtained before and after selective proximal vagotomy (SPV). A reduction in volume and free and total acidity of gastric content was noted along with reduction in ulcer index, with a shift of the site of ulceration from fundus to the glandular part of the stomach, following vagotomy in pylorus-ligated rats. Low volume and acidity explains the absence of ulcers in the fundus, but the increased involvement of glandular part in ulceration is possibly due to weakening of the mucosal barrier following vagotomy.

Biopsy specimens have been taken from five standard sites in the stomach and from the duodenal bulb in order to investigate the association of gastritis and duodenitis with duodenal ulcer. Twenty patients with chronic duodenal ulcer were investigated in this manner and in addition had gastric secretion tests and a radio-immune assay of serum gastrin under differing conditions. Gastric emptying of isotopically labelled solid meals was studied in normal human subjects and in 30 patients who had had an operation for chronic duodenal ulcer. Each patient had a vagotomy of the whole stomach combined with either a Finney pyloroplasty to produce a large gastric outlet or a Heineke-Mikulicz pyloroplasty to produce a relatively small outlet. Carotid bodies of ducks show no detectable changes in ultrastructure up to periods of four weeks following the removal of 2-3 cm of the cervical vagus (decentralization). This indicates that the majority, and possibily all the nerves terminating on the glomus cells are afferent (sensory) in nature. These nerve endings are in reciprocal synaptic contact with the glomus cells and therefore have efferent and afferent functions. Theories concerning the carotid body receptor mechanism are discussed with particular reference to those theories which ascribe functions to the reciprocal synapses.

The immediate and delayed effects of different types of vagotomy on the antral myoelectrical activity have been studied in 52 patients who had previously undergone either highly selective vagotomy (HSV) or truncal vagotomy and pyloroplasty (TV + P) for the treatment of chronic duodenal ulceration up to eight years previously.
