Interspecies scaling: predicting pharmacokinetic parameters of antiepileptic drugs in humans from animals with special emphasis on clearance.

The objective of this study was to test the interspecies-scaling approach in a series of antiepileptic drugs. Clearance, volume of distribution, and elimination half-life were scaled up from animal data obtained from literature. Four different methods were utilized to generate plots to scale up the clearance values: (i) clearance vs body weight (simple allometric equation); (ii) the product of clearance and maximum life-span potential (MLP) vs body weight (an approach recommended in literature); (iii) the two-term power equation which incorporates both body weight and brain weight suggested by

Long-term consequences of head injury.

This study was undertaken to determine the long-term consequences of head injury resulting from police reported tow-away motor vehicle crashes in the U.S. in 1993. The results are presented in terms of the Functional Capacity Index (FCI), which quantifies the relative functional capacity of a previously healthy adult 1 year postinjury, and Life-years Lost to Injury (LLI), defined as the FCI value times the injured person's life expectancy. FCI values vary from 0 for no effect to 1.0 for complete loss of function. Total LLI is indicative of the effect on society, and average LLI represents the

Impact of Intrauterine Growth Restriction and Placental Insufficiency on Nutritional Outcomes of Extremely Low Birth Weight Infants.

Introduction The aim of our study was to assess the impact of intrauterine growth restriction (IUGR) and placental insufficiency (PI) on the nutritional outcomes of extremely low birth weight (ELBW) infants. Methods We conducted a six-year retrospective case-control study that included 117 ELBW infants. Of these, 58 infants had IUGR and 59 were born appropriate-for-gestational age (AGA). Infants with IUGR were further divided based on the presence or absence of PI, as determined by umbilical arterial doppler velocimetry on serial ultrasounds. Results IUGR infants with PI had the lowest enteral

Maternal hookworm modifies risk factors for childhood eczema: results from a birth cohort in Uganda.

BACKGROUND: Worms may protect against allergy. Early-life worm exposure may be critical, but this has not been fully investigated. OBJECTIVES: To investigate whether worms in pregnancy and in early childhood are associated with childhood eczema incidence. METHODS: The Entebbe Mother and Baby Study, an anthelminthic treatment trial, enrolled pregnant women between 2003 and 2005 in Uganda. Mothers were investigated for worms during pregnancy and children annually. Eczema was doctor-diagnosed from birth to age five years. A planned observational analysis was conducted within the trial cohort to

Early palliative care for solid and blood cancer patients and caregivers: Quantitative and qualitative results of a long-term experience as a case of value-based medicine.

IntroductionCancer patients and their caregivers have substantial unmet needs, that negatively impact the clinical outcome and quality of life. However, interventions aimed to address such needs are still suboptimal, failing to answer the recent healthcare call for the adoption of value-based models of care. In the case of incurable oncologic and hematologic cancers, a value-based model of care should plan advanced care on patients' needs and include the quality of death as an outcome. The integration of early palliative care into standard oncologic care for patients with advanced cancers repr

Patient Preferences for Treatment in Relapsed/Refractory Acute Leukemia: A Multinational Discrete Choice Experiment.

PURPOSE: Acute leukemia (AL) is an aggressive cancer of white blood cells that progresses rapidly and requires intensive treatment. While first-line therapies can induce remission, this is not the case for all patients (refractory disease), and many patients experience a relapse. In these situations, treatment decisions become complex due to trade-offs between efficacy, toxicity and quality of life. Despite recognition of the importance of patient preferences in treatment decision-making, limited evidence exists on how individuals with AL value different treatment features. PATIENTS AND METHO

The mobile hermit and the city: considering links between places, objects, and identities in social psychological research on homelessness.

This article explores aspects of a homeless man's everyday life and his use of material objects to maintain a sense of place in the city. We are interested in the complex functions of walking, listening and reading as social practices central to how this man forges a life as a mobile hermit across physical and imagined locales. This highlights connections between physical place, use of material objects, imagination, and sense of self. Our analysis illustrates the value of paying attention to geographical locations and objects in social psychological research on homelessness.

Extracorporeal membrane oxygenation in pheochromocytoma-induced cardiogenic shock.

Extracorporeal membrane oxygenation has been extensively used for cardiopulmonary support in cardiogenic shock. However, its clinical value in the management of pheochromocytoma crisis remains unclear. We report a rare case of life-threatening cardiogenic shock managed with peripheral venoarterial extracorporeal membrane oxygenation combined with endovascular left ventricular venting, in a 40-year-old female patient, in the setting of unknown adrenal pheochromocytoma. We highlight the life-saving role of extracorporeal membrane oxygenation in undiagnosed endocrine emergencies, allowing cardiac

Ocular Refraction at Birth and Its Development During the First Year of Life in a Large Cohort of Babies in a Single Center in Northern Italy.

The purpose of this study was to investigate refraction at birth and during the first year of life in a large cohort of babies born in a single center in Northern Italy. We also aimed to analyze refractive errors in relation to the gestational age at birth. An observational ophthalmological assessment was performed within 24 h of birth on 12,427 newborns. Refraction was examined using streak retinoscopy after the administration of tropicamide (1%). Values in the range of between +0.50 ≤ D ≤ +4.00 were defined as physiological refraction at birth. Newborns with refraction values outside of the

Health State Utility Impact of Breast Cancer in U.S. Women Aged 18-44 Years.

INTRODUCTION: Breast cancer affects women's health-related quality of life negatively, but little is known about how breast cancer affects this in younger women aged 18-44 years. This study measures preference-based health state utility (HSU) values, a scaled index of health-related quality of life for economic evaluation, for younger women with breast cancer and compares these values with same-age women with other cancers and older women (aged ≥45 years) with breast cancer. METHODS: Data from the 2009 and 2010 Behavioral Risk Factor Surveillance System were analyzed in 2014. The sample inclu

Serum thyroid hormone levels in preterm infants born before 33 weeks of gestation and association of transient hypothyroxinemia with postnatal characteristics.

BACKGROUND: Fetal thyroid function and the hypothalamopituitary-thyroid axis continue to mature throughout pregnancy. Therefore, thyroid hormone levels of premature infants differ from those of mature ones. Our primary objective was to evaluate the reference values of serum thyroid hormones in preterm infants born before 33 wk gestation. The second objective was to define a cut-off value for transient hypothyroxinemia of prematurity (THOP) according to gestational age and association of THOP with postnatal characteristics in these infants. SUBJECTS AND METHODS: We recruited a cohort of 200 in

Care-full negotiation of hospital discharge and end-of-life care in an Indonesian palliative care unit.

How do palliative care professionals negotiate end-of-life care with family members when prognosis and dying are not openly discussed? Based on ethnographic fieldwork in an Indonesian palliative care unit, I argue that palliative care professionals employ implicit, ambiguous and culturally sensitive communication to carefully negotiate hospital discharge and discuss end-of-life care. I focus on listening to what is said and what remains unspoken in the embodied communicative practices about end-of-life care in family meetings to discuss hospital discharge. I show how palliative care profession

Neonatal hyperbilirubinemia. Evidence for a role of the erythrocyte enzyme activities involved in the detoxification of oxygen radicals.

Determinations of erythrocyte enzyme scavengers of oxygen radicals (glutathione-peroxidase, superoxide-dismutase and catalase) and determinations of erythrocyte age-dependent glycolytic activities (glucose-6-phosphate-dehydrogenase, pyruvate-kinase and glucose-phosphate-isomerase) were carried out in cord blood and in the blood taken on the 4th day of life in 152 newborn infants with different peak bilirubin levels. The enzyme activities scavenging oxygen radicals, glutathione-peroxidase and superoxide-dismutase were significantly lower in infants with peak bilirubinemia higher than 214 mumol/

WHO disability grade does not influence physical activity in Brazilian leprosy patients.

Disability caused by leprosy may be associated with stigma. The aim of this work is to describe the degree of disability, quality of life and level of physical activity of individuals with leprosy and to identify possible correlations between these factors. Ninety-seven patients from two referral centres were studied. A complete medical history was taken and the World Health Organization degree of physical disability classification (WHO-DG), the International Physical Activity Questionnaire (IPAQ) and the Medical Outcome Study 36-item Short-form health Survey (SF36) were applied. The mean age

Quality of Life in Patients With Olfactory Loss Is Better Predicted by Flavor Identification Than by Orthonasal Olfactory Function.

To date, most studies on the relationship between chemosensory performance and quality of life have focused on orthonasal measures of olfactory function. In the current investigation, we examined the predictive value of orthonasal and flavor identification indices of olfactory function on a wide spectrum of health and sociopsychological factors, including quality of life, life satisfaction, overall health, and depressive symptoms. Participants were 178 ENT patients (Mage = 58 ± 1), representing various causes of olfactory loss: idiopathic smell loss (n = 51; Mage = 63 ± 2), sinunasal disease (
