An unusual cause of benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo (BPPV) is a self-limiting condition characterized by vertigo and nystagmus induced by certain head positions. The most common causes of BPPV are post-traumatic following head injury and post-viral labyrinthitis. We present an interesting case of BPPV following an otherwise uneventful neurosurgical removal of a parietal osteoma using hammer and chisel. Caution should be exercised during such procedures as disabling vertigo can result for a considerable period of time.

Acoustic neuroma: postoperative vertigo and the mechanisms of compensation.

Ninety-five percent of patients with acoustic neuroma experience some degree of vertigo preoperatively, postoperatively or both. Postoperative vertigo is often a debilitating condition that results when the eighth cranial nerve is severed during surgery to resect a tumor growing around the vestibular portion of this nerve. Thus, surgery for acoustic neuroma affects the membranous labyrinth, which is the functional part of the vestibular apparatus and is primarily concerned with equilibrium. Compensation for impaired equilibrium may be by biofeedback in the vestibular pathways, visual control, …

Surgical treatment of vertigo with retrolabyrinthine vestibular neurectomy.

Results for control of vertigo and preservation of hearing in patients who have had a retrolabyrinthine vestibular neurectomy (RVN) by our group were analyzed retrospectively. This procedure consists of selective section of the vestibular nerve in the posterior cranial fossa. Vertigo was completely controlled in all but two of 31 patients, one of whom required revision surgery to control attacks. Analysis of these two cases suggests that the cause of persistent vertigo is incomplete neurectomy. With our current surgical technique in patients with Meniere's disease, hearing results were not sta…

Sensitivity and specificity of audiological tests in patients with vertigo.

In patients with vertigo a comprehensive test battery has been used for localizing pathological processes responsible for the symptoms. Audiological tests have been an integral part of the otoneurological investigation. However, in many studies the diagnostic sharpness of psychoacoustic tests in the localization of specific disease in the cranial nerve system has been called in question. Other tests, such as stapedius reflex and auditory brainstem response, have been of increasing importance in the diagnostic set-up. The purpose of this paper is to present a survey of the literature regarding …

Vertigo following aural suction: can it be prevented?

Suction toilet of mastoid cavities leads to unpleasant vertigo in many patients. We have shown that suction causes cooling within the cavity, in a series of 20 patients. This is of the same order of magnitude as a cold air caloric, showing the temperature drop to be an important factor. The relevant physical principles are examined. During suction a flow of cold, dry room air replaces the warm, moist cavity air, causing cooling both directly and by vaporization of water. Introducing humidified air at near body temperature should eliminate these effects and a study is being set up to evaluate t…

Identification and treatment of metabolic abnormalities in patients with vertigo.

Hyperinsulinism, impaired glucose tolerance, and hypertriglyceridemia may be risk factors for atherosclerotic heart disease and have also been described in patients with vertigo, whose symptoms and findings responded to appropriate dietary therapy. We studied 100 patients in an otolaryngology practice to determine the role of these abnormalities in identifying patients suitable for dietary therapy and to assess the efficacy of dietary therapy in the treatment of vertigo in such selected patients. The determination of hyperinsulinism and hypertriglyceridemia were of value as supplements to the …

The pitfalls of limiting ENG testing to patients with vertigo.

A high incidence of patients who do not experience vertigo with unilateral vestibular disease has been observed. Recognizing that many clinicians perform electronystagmographic vestibular testing on only patients who describe vertigo, a retrospective study of 229 patients was designed to study the potential pitfalls of this practice. The results confirm a preliminary study and show that 66% of the patients described no vertigo. Included were 13 intracranial lesions of which only one had the classic symptom. Differences in cortical perception and adaptation are postulated as factors responsible…

Vertigo due to whiplash injury: a neurotological approach.

A series of examinations were carried out to elucidate the characteristics of vertigo due to whiplash injury. In patients with whiplash injury, there are over-excitation of the cervical and/or the lumbar proprioceptors on one hand, and dysfunction of the central nervous system, such as the hypothalamus, the brain stem and the cerebellum, on the other hand. These two etiological factors induce disequilibrium by a trigger-and-target relationship in which the above proprioceptors act as a trigger and the central nervous system acts as a target. This postulate is applicable to the explanation of a…

Surgery for vertigo (you don't have to live with it).

Surgery for vertigo is recommended where there is objective evidence of a unilateral inner ear disorder, when medical therapy fails, and when the patient demands definitive therapy. Common surgical procedures are the endolymphatic subarachnoid shunt, retrolabyrinthine vestibular neurectomy, cochleovestibular neurectomy, and the post ampullary nerve section (singular neurectomy). Vertiginous disorders which can be surgically cured are Ménière's disease, vestibular neuronitis, chronic labyrinthitis, and benign positional vertigo. Due to the recent advances in microsurgery and the development of …

Benign paroxysmal vertigo of childhood: a migraine equivalent.

Eight patients with the syndrome of benign paroxysmal vertigo of childhood are presented. This symptom complex is characterized by attacks of vertigo in young children combined with nystagmus, ataxia and transiently decreased vestibular function, but without impaired consciousness. Diagnostic and follow-up studies revealed a close relationship to other forms of autonomic nervous system instability, particularly to the migraine symptom complex.

Lorazepam and diazepam in the treatment of benign paroxysmal vertigo.

Animal studies indicate that diazepam suppresses resting activity in the vestibular system. Despite lack of clinical studies, diazepam and lorazepam have been used empirically for relief of vertigo. However, drug use with benign paroxysmal vertigo (BPV) is controversial which led to this study to evaluate the value of diazepam and lorazepam for relief of BPV. Twenty-five BPV patients were given diazepam, lorazepam, or a placebo over four weeks using a double-blind technique. Each patient rated his dizziness after each week, and the patient's nystagmus was rated before and after treatment. The …

[Diagnostic problems in dizziness or vertigo (author's transl)].

Different causes of dizziness or vertigo can only be recognized by thorough anamnestic explorations. Following a classification in vestibular and nonvestibular causes for vertigo, a further differentiation is possible by defining different characteristic qualities of the symptoms involved. In addition to the classical vestibular forms of vertigo seen, dizziness currently results from drug overdosages, hypertension, polyneuropathy and--less commonly, but equally important--brief epileptic seizures. Psychosomatic and neurotic symptoms may also lead to unsteady gait, dizziness or vertigo, all of …
