Simultaneous operations for gastric cancer and aortic aneurysm: a case report.

BACKGROUND: Gastric cancer is the second highest cause of morbidity among malignant tumors of the gastrointestinal tract and fifth in overall cancer statistics. Diseases of the cardiovascular system are the leading causes of death in the world. Aneurysm of the abdominal aorta is the most common type of vascular aneurysm, while in 75% of the cases it is asymptomatic. The risk of rupture of aneurysm of the abdominal aorta increases progressively depending on its diameter and the age of the patient. CASE PRESENTATION: A 56-year-old male patient underwent treatment for complaints of pain and disc

An unusual case of Streptococcus pyogenes infective endocarditis demonstrating the usefulness of repeated echocardiography.

Infective endocarditis (IE) is most often caused by alpha-hemolytic streptococci or Staphylococcus aureus and is characterized by the presence of vegetations on the heart valves. Here we present a case of IE caused by Streptococcus pyogenes, a distinctly uncommon IE-pathogen, where no vegetations could be visualized on repeated echocardiography. Diagnosis was instead evident from septic embolizations and progressive aortic insufficiency and IE was verified upon heart valve surgery. The patient was a 69-year-old man who presented with a two-day-history of fever and confusion. Several lesions on

[Hydrogel posterior chamber lenses--an improvement?].

Although the design of PMMA intraocular lenses has reached a stage of sophistication at which further modifications are unlikely to result in any major improvements in implant surgery, new materials with possibly improved mechanical and biologic properties should be evaluated carefully. The hydrogel lens (containing 38% poly-HEMA) is a soft, flexible, hydrophilic "single-piece" posterior chamber lens which could help to minimize intraoperative and postoperative IOL trauma. Fifty hydrogel intraocular lenses were implanted during a ten-month period using a standard extracapsular cataract extract

Intramedullary pinning with tension-band wiring for surgical neck fractures of the proximal humerus in elderly patients.

Most proximal humeral fractures in the elderly population are related to osteoporosis. Several methods have been proposed to treat surgical neck fractures of the proximal humerus in elderly people. This study investigates a new method of intramedullary pinning with tension-band wiring. From June 1998 to March 2001, 10 female patients with a mean age of 73.0 years and displaced two- or three-part surgical neck fractures of the proximal humerus were studied. Two intramedullary pins were used with tension-band wiring via a deltopectoral approach with minimum dissection. The mean follow-up was 20.

"Optimizing IOL centration in cataract surgery: Narrative review of techniques, challenges, and outcomes".

Intraocular lens (IOL) centration is pivotal in determining the visual quality and refractive success after cataract surgery, especially with premium IOLs such as multifocal, extended depth of focus (EDOF), and toric lenses. Despite the surgical and technological advancements, achieving perfect centration of the IOL remains a challenge, influenced by anatomical variations and intraoperative variables. Different preoperative variables, such as the patient's refractive status, the technique of axis marking, calculation of IOL power, IOL material and design, and measurement of visual angles, have

CAIRS and nomograms: a narrative review.

CAIRS (Corneal Allogeneic Intrastromal Ring Segments) is an emerging new treatment for keratoconus that involves implantation of allogeneic corneal tissue into the periphery of the keratoconic cornea, resulting in flattening of the cone, regularisation of the central cornea, and ultimately, improved vision. Published results have been encouraging and consequently, the procedure is gaining significant traction around the world. Several nomograms have been developed to predict visual outcomes and improve accuracy and predictability. We review and compare the results of the different nomograms (e

Hybrid optogenetic and electrical stimulation of retinal ganglion cells for artificial vision.

INTRODUCTION: Millions of adults worldwide experience severe visual impairment due to photoreceptor loss from retinal diseases such as retinitis pigmentosa and macular degeneration. Retinal prostheses that provide artificial vision by stimulating the surviving retinal ganglion cells (RGCs) have emerged as a promising therapy. However, all clinically approved retinal prostheses that use electrical stimulation face the issue of electrical spread. As such, the quality of restored vision provided by existing devices has been limited. Optogenetic approaches provide greater spatial precision, howeve

Four-flanged, intrascleral fixation of foldable intraocular lens using straight-needled 7-0 polypropylene suture - A surgical technique.

The occurrence of intraocular lens (IOL) implantation failure in phacoemulsification is relatively uncommon. However, because cataract surgery is the most prevalent surgical procedure worldwide, the aggregate instances of post-surgical aphakic status are significant. Though several alternatives, including anterior chamber IOL, iris-fixated IOL, and scleral-fixated IOL (SFIOL), exist for secondary IOL implantation, recent trends favor SFIOL implantation. In the realm of SFIOLs, all sutureless variants necessitate the exteriorization of the haptic through the sclera, implying a larger sclerotomy

[Research advances on light-adjustable intraocular lens].

Refractive error is a key determinant of visual acuity post-cataract surgery. Traditional methods for addressing refractive errors have been invasive, posing the risk of permanent surgical complications. Thus, the pursuit of non-invasive techniques for correcting refractive errors after cataract surgery is of significant clinical importance. Recently, a novel class of intraocular lenses (IOL), termed light-adjustable IOL, has emerged as a potential solution for refining refractive outcomes. These IOL possess the unique ability to alter their dioptric power in vitro through exposure to specific

Managing low corneal astigmatism in patients with presbyopia correcting intraocular lenses: a narrative review.

Cataract surgery has become a refractive procedure in which emmetropia is the goal, with the implantation of extended depth-of-focus or multifocal intraocular lenses (IOLs) being the commonly selected option to restore vision beyond the far distance. The selection criteria for implanting these lenses can differ from those for monofocal IOLs and even between technologies, as eye characteristics can affect postoperative visual performance. Corneal astigmatism is an eye characteristic that can affect visual performance differently, depending on the implanted IOL. The magnitude of corneal astigmat

Optimizing outcomes with multifocal intraocular lenses.

Modern day cataract surgery is evolving from a visual restorative to a refractive procedure. The advent of multifocal intraocular lenses (MFIOLs) allows greater spectacle independence and increased quality of life postoperatively. Since the inception in 1980s, MFIOLs have undergone various technical advancements including trifocal and extended depth of vision implants more recently. A thorough preoperative workup including the patients' visual needs and inherent ocular anatomy allows us to achieve superior outcomes. This review offers a comprehensive overview of the various types of MFIOLs and
