Eliminating the Toynbee phenomenon in patients with nasal packs.

Nasal packing is a common procedure in otolaryngology, but is not a completely innocuous procedure. Besides aggravating mouth breathing, complete nasal obstruction results in an annoying Toynbee phenomenon. In patients with bilateral anterior nasal packing, we recorded high pressure gradients in the nasopharynx and, in consequence, in the middle ear. Patients in whom rubber tubes were inserted between the gauze layers of the anterior nasal packs suffered no such inconvenience and showed no such gradients. Our work proves the value of using these tubes for pressure equalization when nasal packs

WE-C-217BCD-03: Restricted Data Set Reconstruction Based on Respiration Quality to Improve Prospectively Gated in Vivo Micro-CT of Mice.

PURPOSE: Micro-CT is commonly employed for lung imaging of mice; prospective gating allows for in-vivo imaging of free-breathing subjects. While this technique is successfully executed for healthy animals, results are less consistent for some disease models whose symptoms include irregular or unstable respiration. The purpose of this work is to repair the quality of high-blur images that arise from respiration instability using a retrospective method of motion reduction which identifies the individual x-ray projection images contributing most to the motion blur. Reconstructions were performed

Upper airway collapsibility: measurement techniques and therapeutic implications.

Several techniques are currently available that aim to characterize upper airway function/mechanics during wakefulness or sleep. Based on the concept of a Starling resistor, we propose a standardized protocol to measure the critical pressure (Pcrit) (an indicator of upper airway collapsibility) during sleep. The effect of therapeutic interventions such as weight loss, positional changes or uvulopalatopharyngoplasty on Pcrit is illustrated by data from the literature. We propose that measurement of Pcrit become implemented in the diagnostic work-up of selected patients with sleep-related breath

Cerebrovascular reactivity assessment with O2-CO2 exchange ratio under brief breath hold challenge.

BACKGROUND: Hypercapnia during breath holding is believed to be the dominant driver behind the modulation of cerebral blood flow (CBF). However, increasing evidence show that mild hypoxia and mild hypercapnia in breath hold (BH) could work synergistically to enhance CBF response. We hypothesized that breath-by-breath O2-CO2 exchange ratio (bER), defined as the ratio of the change in partial pressure of oxygen (ΔPO2) to that of carbon dioxide (ΔPCO2) between end inspiration and end expiration, would be able to better correlate with the global and regional cerebral hemodynamic responses (CHR) to

Bicycle ergometry and gas exchange measurements in neuromuscular diseases.

Using bicycle ergometry with computerized respiratory gas exchange measurements, we compared exercise capacities in patients with various neuromuscular diseases to those in normal controls. As expected, male and female patients had significantly reduced maximum work capacities (kilopond-meters per minute per kilogram of body weight) and maximal oxygen consumptions. The oxygen cost of exercise was normal in the majority of patients, although some appeared to have abnormally high oxygen consumptions during exercise. Breathing patterns during exercise, particularly in regard to onset of hypervent

Post-contrast abdominal magnetic resonance imaging of critically ill patients using compressed sensing free-breathing golden radial angle imaging.

Obtaining diagnostic-quality magnetic resonance imaging (MRI) of the abdomen in critically ill patients can be difficult due to challenges with breath-holding and the inability to follow technologist instructions. Protocols that harness advances in commercially available MRI techniques provide a potential solution, particularly using the golden radial angle sparse parallel (GRASP) technique for dynamic post-contrast T1-weighted imaging. The GRASP technique uses a combination of free-breathing, a stack-of-stars radial acquisition, and compressed sensing reconstruction acquired over several minu

Dyspnea in a 57-Year-Old Man With Recent Viral Illness.

A 57-year-old man presented to the ED with a 1-month history of nonproductive cough and shortness of breath. The patient had been in his usual state of health until 2 months before presentation, when he experienced an episode of nonproductive cough and shortness of breath. He was diagnosed clinically with an upper respiratory tract infection; the symptoms resolved after 7 days with conservative therapy alone. One month later, the nonproductive cough and shortness of breath returned. He was treated with a 5-day course of oral azithromycin without any improvement in his symptoms. On presentation

Hyperoxic attenuation of exercise-induced bronchospasm in asthmatics.

To investigate the mechanism of exercise-induced bronchospasm, we measured specific airway conductance before and after exercise in 7 healthy normals, 12 asthmatics with intact carotid bodies, and 5 asthmatics who had had bilateral carotid body resection. The subjects breathed either air or oxygen (randomly assigned) during cycle ergometer exercise. Post-exercise bronchodilation was the usual pattern in normals, whereas post-exercise bronchospasm occurred in all asthmatics who breathed air during exercise. Oxygen breathing during exercise markedly attenuated the post-exercise bronchospasm in t

Respiratory bi-directional pressure and flow data collection device with thoracic and abdominal circumferential monitoring.

Non-invasive pressure and flow data from Venturi-based sensors can be used with validated models to identify patient-specific lung mechanics. To validate applied respiratory models a secondary measurement is required. Rotary encoder-based tape measures were designed to capture change in circumference of a subject's thorax and diaphragm. Circumferential changes can be correlated to measured or modelled change in lung volume and associated muscular recruitment measures (patient work of breathing). Hence, these simple measurement devices can expedite respiratory research, by adding low-cost, acce…

Common health predictors of early retirement: findings from the English Longitudinal Study of Ageing.

BACKGROUND: facing the costs of population ageing, many governments aim to keep older people in employment for longer. Summary health measures predict early retirement, but more specific symptoms and conditions need to be identified to guide efforts to delay retirement. OBJECTIVE: to identify common symptoms and conditions that predict early work exit, at the population level. DESIGN: cohort study of community dwelling respondents to the English Longitudinal Study of Ageing. SETTING AND PARTICIPANTS: a total of 1,693 workers aged 50 and over at baseline who were younger than the contemporan

Accelerated liver water T1 mapping using single-shot continuous inversion-recovery spiral imaging.

PURPOSE: Liver T1 mapping techniques typically require long breath holds or long scan time in free-breathing, need correction for B 1 + inhomogeneities and process composite (water and fat) signals. The purpose of this work is to accelerate the multi-slice acquisition of liver water selective T1 (wT1) mapping in a single breath hold, improving the k-space sampling efficiency. METHODS: The proposed continuous inversion-recovery (IR) Look-Locker methodology combines a single-shot gradient echo spiral readout, Dixon processing and a dictionary-based analysis for liver wT1 mapping at 3 T. The se

Comparison of internal dose measures of solvents in breath, blood and urine and genotoxic changes in aircraft maintenance personnel.

Solvents and fuels are in widespread use both in civilian and military populations. 1,1,1-trichloroethane (TCA), xylene, toluene, methyl ethyl ketone (MEK) and methylene chloride are found in a variety of compounds including degreasing agents, paints, coatings, pesticides and paint strippers. Toluene and xylene are also found in fuels, which are complex mixtures of hundreds of agents. The purpose of this investigation was twofold. The first was to determine the optimum medium to measure internal dose of solvents comparing blood, urine and breath. The second was to determine if low level exposu

Neurometabolic Response to Apneic Stimuli Tracks Global Grey Matter Volume Deficits in Patients With Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) has been linked to cortical atrophy and increased risk of neurodegeneration, a process potentially mediated by neurometabolic dysregulation. Recent work using high temporal-resolution metabolic MRI found greater changes in the cerebral metabolic rate of oxygen consumption (CMRO2) during cued breath-hold stimuli simulating spontaneous apneas in OSA patients than in healthy people. Here, we conducted a secondary analysis on this work to assess whether the observed neurometabolic responses were associated with grey matter volume fraction. We examined previously colle

The respiratory system as an exercise limiting factor in normal sedentary subjects.

The present study was undertaken to investigate the respiratory system as an exercise limiting factor. Breathing and cycle endurance (i.e. the time until exhaustion at a given performance level) as well as physical working capacity 170 (i.e. the exercise intensity corresponding to a heart rate of 170 beats.min-1 on a cycle ergometer) were determined in four healthy sedentary subjects. Subsequently, the subjects trained their respiratory system for 4 weeks by breathing daily about 90 l.min-1 for 30 min. Otherwise they continued their sedentary lifestyle. Immediately after the respiratory traini

Artificial Intelligence for the Detection of Patient-Ventilator Asynchrony.

Patient-ventilator asynchrony (PVA) is a challenge to invasive mechanical ventilation characterized by misalignment of ventilatory support and patient respiratory effort. PVA is highly prevalent and associated with adverse clinical outcomes, including increased work of breathing, oxygen consumption, and risk of barotrauma. Artificial intelligence (AI) is a potentially transformative solution offering capabilities for automated detection of PVA. This narrative review characterizes the landscape of AI models designed for PVA detection and quantification. A comprehensive literature search identif…

Applied physiology of diving.

Recreational diving is a popular sport, although human ability to stay in and under water is severely limited physiologically. An understanding of these limitations enhances safety and enjoyment of sports diving. Breath-hold diving involves head-out water immersion, apnoea and submersion, exercise, cold stress, and pressure exposure. Each of these components, by itself, elicits prominent and specific physiological effects. Combination of these factors produces a unique and interesting physiological response generally known as diving reflex. Humans display weak diving responses, but exhibit no

High-Precision Synchronous Detection of Breath CO, CH4, and CO2 Using an NIR-WMS Sensor Based on a Dual-Channel Multipass Cell.

TDLAS-based breath sensors hold significant promise for clinical diagnostics because of their high selectivity, fast response, and operational simplicity. However, their application in high-precision, multicomponent breath analysis is often hindered by system complexity, high cost, and limited detection accuracy. To overcome these challenges, this study presents a near-infrared TDLAS system for the simultaneous monitoring of carbon monoxide (CO), carbon dioxide (CO2), and methane (CH4) in breath. The system integrates a spectral line optimization and selection strategy with a novel, miniaturiz
