Assessment of the scientific basis for genetic testing of railroad workers with carpal tunnel syndrome.

In 2000, approximately 20 railroad track workers who filed injury reports or compensation claims for carpal tunnel syndrome were tested by their employer for two genetic traits to determine the work relatedness of the condition. The testing involved deletions, variants, or mutations in the genetic coding for peripheral myelin protein (PMP22) and transthyretin (TTR). This article is an assessment of whether there is a scientific basis for such testing. A review of the scientific literature indicated that neither the scientific basis nor the population validity of the PMP22 or TTR tests for carp

Lag Times in Reporting Injuries, Receiving Medical Care, and Missing Work: Associations With the Length of Work Disability in Occupational Back Injuries.

ObjectiveThe aim of this study is to examine the associations between lag times following occupational low back injury and the length of work disability.MethodsIn a retrospective cohort study using workers' compensation claims, random effects Tobit models were used to explore how disability length relates to three lag times: the number of days from the date of injury to reporting the injury, the number of days from the date of injury to medical care, and the number of days from the date of injury to initiating work disability.ResultsIn general, shorter lag times for each of the different lags …

Compensated back injury in New Zealand.

Back injury is one of the commonest claims for accident compensation. A survey carried out over three months in 1984 identified 420 claimants having details of their accidents. Labourers, freezing workers, coal miners and railway workers were at high risk. Over half (54.7%) developed back pain whilst lifting and for almost two-thirds (63.6%) there was a sudden strain. Half had a previous history of back pain and the incidence rose to 70% for those aged 40-59 years. Eighty-two percent returned to work within four weeks. Most were treated by a general practitioner and physiotherapy was increasin

Recovery expectations predict recovery in workers with back pain but not other musculoskeletal conditions.

STUDY DESIGN: Prospective cohort study. OBJECTIVE: We examined whether recovery expectations predict future return-to-work in workers filing injury claims for a variety of musculoskeletal conditions. SUMMARY OF BACKGROUND DATA: Recovery expectations seem to influence recovery and return-to-work after back pain, but their role in other compensable conditions is uncertain. METHODS: All workers' compensation claimants receiving time-loss benefits for a musculoskeletal condition and undergoing return-to-work assessment were enrolled. Claimants completed a work-related recovery expectations ques…

The false promise of return to work for migrant workers injured on the job in Canada: When public policies intersect to create exclusion.

BackgroundMigrant workers in Canada work in the most dangerous industries, with documented difficulties with occupational health and safety (OHS) and workers' compensation (WC). Little is known, however, about migrant workers' experiences with "return-to-work", the ultimate objective of WC for those who recover from their illness or injury.ObjectiveThe objective of this study was to understand migrant workers' "return-to-work" experiences after facing occupational injury/illness for in Québec, Canada.MethodsPart of a broader multi-disciplinary project exploring how Canadian policies can ensure…

Ionizing radiation and chronic lymphocytic leukemia.

The U.S. government recently implemented rules for awarding compensation to individuals with cancer who were exposed to ionizing radiation while working in the nuclear weapons complex. Under these rules, chronic lymphocytic leukemia (CLL) is considered to be a nonradiogenic form of cancer. In other words, workers who develop CLL automatically have their compensation claim rejected because the compensation rules hold that the risk of radiation-induced CLL is zero. In this article we review molecular, clinical, and epidemiologic evidence regarding the radiogenicity of CLL. We note that current u

"'Not just a job": Organizational Culture and the Sustainability of the Peer and Lived Experience Workforce.

Peer recovery support specialists represent one of the most rapidly expanding sectors of the behavioral health care workforce, yet organizational conditions that support or undermine this workforce remain underexplored. To better understand the experience of providing peer support and identify organizational factors that facilitate or hinder effective and sustainable peer work, we conducted semi-structured qualitative interviews with a convenience sample of 17 peer workers in Connecticut. Interviews were recorded, transcribed, and thematically analyzed. We identified five themes, reflecting bo

Arthroscopic repair of chronic lateral ankle instability.

BACKGROUND: The current "gold standard'' for treatment of chronic lateral ankle instability is the Broström-Gould procedure. Most authors recommend this type of operation even after an arthroscopic inspection of the joint. We review our results with an arthroscopic method of lateral ankle ligament repair. MATERIALS AND METHODS: A method of arthroscopic lateral ligament repair with an anchor placed in the fibula is described. We used this technique on 31 consecutive patients (28 were available for followup). Twenty-one patients had work or traffic accidents, four suffered casual falls. and thr

Transforaminal epidural steroid injections prevent the need for surgery in patients with sciatica secondary to lumbar disc herniation: a retrospective case series.

BACKGROUND: The median orthopedic surgery wait time in Canada is 33.7 weeks, thus alternative treatments for pathologies such as lumbar disc herniations (LDH) are needed. We sought to determine whether transforaminal epidural steroid injections (TFESIs) alleviate or merely delay the need for surgery. METHODS: We retrospectively reviewed the charts of patients with LDH who received TFESIs between September 2006 and July 2008. Patient demographics, level and side of pathology, workers' compensation status, levels injected, treatment outcome and time from referral to treatment were evaluated. Th…

Work status as a predictor of surgical outcome of discogenic low back pain.

STUDY DESIGN: Prospective observational study. OBJECTIVES: To evaluate the role of work status as a predictor of outcome from anterior lumbar fusion. SUMMARY OF BACKGROUND DATA: Many psychosocial factors have been identified as predictors of chronic disability and of outcomes of surgery. Workers' Compensation and job satisfaction are two of the strongest and most evaluated factors. Work status at the time of intervention may also be relevant but has rarely been studied independently in patients having lumbar fusion. METHODS: A total of 106 patients with discogenic low back pain were treated…
