The Case of a 15-Year-Old With Non-Parasitic Chyluria.

Chyluria is defined by the presence of chyle in urine, and is caused by a communication between the lymphatic and urinary system. Commonly, it is characterised by the excretion of milky white urine, which is present in up to 70% of chyluria cases. The prevalent cause for chyluria in Asia is filariasis with Wuchereria bancrofti. Non-parasitic chyluria is more common in western countries and is usually subsequent to traumatic factors, infections, or tumours. The occurrence of non-parasitic chyluria in the absence of trauma, iatrogenic or other, is exceedingly rare. The lymphatic system is rather…

Host melatonin secretion is a timing signal for the release of W. bancrofti microfilaria into the circulation.

Infection of the lymphatic system by Wuchereria bancrofti microfilaria is the most common cause of elephantiasis, a disease that affects over 100 million people. The nightly release of microfilaria into the circulation coincides with the feeding activity of its mosquito vector, a synchronization that is presumably an evolutionary adaptation favoring survival and transmission of the parasite. During the daytime hours, the microfilaria are thought to be sequestered in the lungs, and because they are not exposed to the solar light/dark cycle, there must be some other cue that entrains their night…

Lymphatic filariasis: detection of circulating and urinary antigen and differences in antibody isotypes complexed with circulating antigen between symptomatic and asymptomatic subjects.

A two-site immunoradiometric assay using a monoclonal antibody (MoAb) against Brugia malayi microfilariae allowed the detection of parasite molecules both in the serum and the urine of patients from Sri Lanka infected with Wuchereria bancrofti. Whereas 50% of patients had no antigen in their serum, all of them excreted detectable amounts of antigen in their urine, the levels being higher in symptomatic than in asymptomatic patients. The poor detection in serum appeared to be related to the presence of circulating immune complexes. It was shown that the isotype of the antibodies complexed with …

Primary retroperitoneal filariasis: a common disease of tropics with uncommon presentation and review of literature.

Lymphatic filariasis is caused by nematode filariae Wuchereria bancrofti, Brugia malayi or Brugia timori. It is commonly seen in tropical and subtropical regions of the world and affects the lymphatic system of humans, who are the definitive host while mosquito is the intermediate host. The most common manifestation of the disease is hydrocele followed by lower limb lymphoedema and elephantiasis. Although filariasis is much more common entity in north India, its presentation as retroperitoneal cyst is very rare with reported incidence rate of 1/105 000. We present a case of primary retroperito…

Spatial modelling of individual-level parasite counts using the negative binomial distribution.

We present a spatial model for the mean and correlation of highly dispersed count data, and apply it to individual-level counts of the nematode Wuchereria bancrofti, a parasite of humans which causes the disease lymphatic filariasis. Our model uses the negative binomial distribution, whose shape parameter is a convenient index of over-dispersion. Spatial association is quantified in terms of a characteristic length, which has an intuitive interpretation as the distance over which correlation decreases by half. Demographic surveillance and mapping enable us to include individual-level covariate…

Efficacy of five annual single doses of diethylcarbamazine for treatment of lymphatic filariasis in Fiji.

Annual single-dose treatments with diethylcarbamazine citrate (DEC) at a dose of 6 mg/kg have been reported effective in reducing microfilar … e to high-endemicity areas. The results also suggest that such treatment affected juvenile forms of Wuchereria bancrofti and may prevent them from reproducing.

Anopheles flavirostris incriminated as a vector of malaria and Bancroftian filariasis in Banggi Island, Sabah, Malaysia.

Seven villages in Banggi Island, Sabah, Malaysia, were surveyed four times to evaluate the roles of local mosquitoes as vectors of malaria a … irostris and four of 365 An. balabacensis harboured L2 or L3 filarial larvae identified as those of Wuchereria bancrofti. This is the first record of An. flavirostris as a natural vector of malaria and W. bancrofti in Sabah.

The lethal effects of the cibarial and pharyngeal armatures of mosquitoes on microfilariae.

Microfilariae of Wuchereria bancrofti and Brugia pahangi were killed by the chewing action of the cibarial and pharyngeal armatures and other papillae and spines in the fore-gut of mosquitoes. The proportion of ingested microfilariae that were killed was largely dependent on the presence and shape of the cibarial armature. Anopheles farauti No. 1 and Anopheles gambiae species A and B have well developed cibarial armatures and killed 36 to 96% of the ingested microfilariae. Culex pipiens fatigans has a poorly developed cibarial armature and killed only 6% of the microfilariae. Aedes aegypti and…

Lymphatic filariasis transmission on Mafia Islands, Tanzania: Evidence from xenomonitoring in mosquito vectors.

INTRODUCTION: Lymphatic filariasis (LF) is a chronic nematode infection transmitted by mosquitoes and in sub-Saharan Africa it is caused by Wuchereria bancrofti. The disease was targeted for global elimination by 2020 using repeated community-wide mass drug administration (MDA) distributed in endemic areas. However, recently, there has been a growing recognition of the potential role of including vector control as a supplement to MDA to achieve elimination goal. This study was carried out to determine mosquito abundance and transmission of bancroftian filariasis on Mafia Islands in Tanzania as…

Tanzania filariasis project: studies on microfilaraemia and selected clinical manifestations of Bancroftian filariasis.

In a total of 1977 males aged 5 years and over examined in 21 cluster samples on the coast of Tanzania, the overall rate for microfilariae of Wuchereria bancrofti was 32%. Hydrocoele and elephantiasis rates were 39% and 3%, respectively. The rates and severity of these 2 clinical manifestations increased in the older age groups. Both the microfilarial density and prevalence increased rapidly in the early age groups, followed by a levelling off and then a slight fall in old age. There was no association between microfilarial densities or rates and the overall prevalence of hydrocoele, but a neg…

Absence of lymphatic filariasis infection among secondary-school children in Oman.

The endemicity status of lymphatic filariasis in Oman is uncertain, with only sporadic cases reported, mostly imported. Immunochromatographic card test surveys were carried out to assess the presence of circulating Wuchereria bancrofti antigenaemia as a marker for active infection in children from suspected high-risk areas of Oman (South Batinah and Dhofar). Lot quality assurance sampling surveys were carried out on a minimum of 250 secondary-school children aged 17-18 years in each of 8 districts from February 2004 to March 2004. All tested students were negative for circulating W. bancrofti …

T-lymphocytes from individuals with filarial inflammatory disease have increased transendothelial migration in vitro.

The in vitro transendothelial migration of circulating filarial antigen-specific T-cells was examined in Wuchereria banerofti infection. Circulating T-cells from individuals with filaria-induced lymphatic pathology (LP) had significantly greater migration through unstimulated HUVEC monolayers than did T-cells from asymptomatic infected (MF) individuals (P = 0.04). In contrast to the MF individuals where no effect was seen, transendothelial migration of 48-hr filarial antigen stimulated T-cells from LP individuals was significantly (P = 0.01) greater than migration of 48-hr media-stimulated T-c…

Extralymphatic Filariasis.

Filariasis caused by nematodes affects the structure and function of lymphatic vessels. Lymphedema due to lymphatic blockage and lymphadenitis is the usual mode of presentation of filariasis. However, rarely extralymphatic filariasis has been reported in uncommon sites, including skin and soft tissue. Wuchereria (W.) bancrofti is the most common nematode causing extralymphatic filariasis. We report a rare case of a 25-year-old man with live extralymphatic filarial infestation presenting as a facial subcutaneous soft-tissue swelling confirmed by ultrasonography (USG), which revealed the filaria…

A multidisciplinary study on bancroftian filariasis in Jakarta.

A study on bancroftian filariasis in Jakarta has indicated that one person in one year could be exposed to 223,000 bites of Culex pipens fatigans and to 1,941 infective-stage larvae of Wuchereria bancrofti. Blood surveys with 20 mm3 samples revealed a microfilaria rate of 6%. Although some cases of hydrocele (4% of 272 males examined) were found, there was little evidence of severe filarial disease in either males or females. Amongst wild-caught mosquitoes only 0.3% contained infective larvae, but much higher levels of vectorial competence were established under laboratory conditions. High dai…

Imported bancroftian filariasis discovered in a patient infected with Plasmodium falciparum: First case of concomitant parasitism in the Al-Buraimi Governorate, Oman.

Malaria and lymphatic filariasis (LF) are endemic in tropical and subtropical regions of sub-Saharan Africa, Asia, and Central and South America. Ninety-two percent (200 million) of malaria cases in 2017 were detected in the WHO African Region. This accounts for approximately 30% of the global burden of LF disease and includes 405.9 million people at risk in 39 out of 46 member countries. Anopheles species of mosquitoes transmit Wuchereria bancrofti in most parts of Africa. Our case of a 23-year-old Nigerian woman highlights incidental laboratory findings showing the first malaria/filariasis c…

Filarial abscess in the submandibular region.

Filariasis is a parasitic infectious disease caused by filarial nematode worms. These worms mainly dwell in subcutaneous tissues and lymphatics of the human host, with a predilection for lower limbs, retroperitoneal tissues, spermatic cord, and epididymis. Oral or perioral involvement of the filarial nematode is rare. This case report describes a filarial abscess in the right submandibular region. Fine needle aspiration cytology of the abscess revealed the presence of microfilaria of Wuchereria bancrofti species. The parasite was also present in the peripheral blood smear.
