Eosinophils are seldom noted in inflammatory synovial fluids but are reported to infiltrate the synovial tissue in inflammatory arthritides. To elucidate a possible role for eosinophils in inflammatory joint reactions the concentrations of eosinophil cationic protein (ECP)--a specific granule protein from eosinophils--were measured by radioimmunoassay in 90 synovial fluids from patients with various inflammatory arthritides (rheumatoid arthritis, reactive and crystal arthritides, Reiter's disease and psoriatic arthropathy). In the same specimens lactoferrin was measured as an indicator of neut…

The object of this investigation was to determine the prevalence of anti-T cell antibodies in 66 children with various connective tissue diseases. Anti-T cell antibodies were found in 43/44 juvenile rheumatoid arthritis (JRA) patients (mean cytotoxicity 15.0%) and in 10/10 children with systemic lupus erythematosus (mean cytotoxicity 20.0%), but in only 1/15 normal controls and in none of 12 children with other arthritides. There was no significant difference in mean percent cytotoxicity among the JRA subclasses. In the JRA patients, the percent cytotoxicity was positively correlated with the

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by persistent synovitis and progressive cartilage and bone destruction. Synovial macrophages are central coordinators of inflammatory signaling, stromal activation, and osteoclastogenesis, yet their pathogenic functions cannot be explained by extracellular cytokine networks alone. Increasing evidence indicates that macrophage activation is coupled to cell-intrinsic mitochondrial metabolic reprogramming. Within the hypoxic, nutrient-variable, and inflammatory RA synovium, macrophages undergo coordinated changes in glycolysi

This review explores the critical role of mitochondria in the immunometabolic processes underlying rheumatoid arthritis (RA) and osteoarthritis (OA). It examines the interplay between immune cells, metabolic demands, and tissue environments, emphasizing the impact of bioenergetics on immune responses and disease progression. Mitochondrial dysfunction in chondrocytes and immune cells contributes to OA and RA through mechanisms such as oxidative stress, disrupted calcium homeostasis, and inflammasome activation. In OA, mitochondrial dysfunction in chondrocytes results in impaired energy producti

Accurate quantification of C-reactive protein (CRP) in serum and synovial fluid is critical for diagnosing and monitoring localized inflammatory arthritis, but it remains challenging due to the complexity of the matrix and the lack of robust and sensitive assays. To overcome this critical challenge, a novel self-validating triple-modal immunosensing platform was developed based on a rationally designed multifunctional cobalt-ferrocene metal-organic framework (CoFcMOF) nanozyme, which ensures robust detection reliability through a synergistic signal fusion mechanism. The innovation lies in the

Macrophages, key players in the immune system, exhibit remarkable plasticity, polarizing into pro-inflammatory (M1) or anti-inflammatory (M2) phenotypes in response to microenvironmental cues. This functional dichotomy is pivotal in the pathogenesis and progression of a wide array of diseases, including cancer, autoimmune disorders like rheumatoid arthritis, atherosclerosis, and tissue injury. Consequently, modulating macrophage polarization has emerged as a promising therapeutic strategy. Metal-based nanoparticles (MNPs) have garnered significant attention in this field due to their unique ph

SIGNIFICANCE: Non-invasive optical measures for evaluating rheumatoid arthritis (RA) remain limited. Clinical scores such as the 28-joint Disease Activity Score (DAS28) and ultrasound are operator-dependent and do not directly quantify optical tissue changes. Short-wave infrared (SWIR) hyperspectral imaging (HSI) provides sensitivity to inflammation-associated spectral changes in biological tissues.
AIM: The aim is to develop and evaluate a quantitative HSI soft abundance scoring (HSISAS) method for assessing treatment response in RA patients using SWIR-HSI.
APPROACH: Eleven RA patients who

BACKGROUND: We aimed to investigate the prevalence and risk factors of acute kidney injury (AKI) at juvenile idiopathic arthritis (JIA) onset and its impact on long-term kidney outcomes.
METHODS: In this multicenter study, we retrospectively reviewed 192 children diagnosed with JIA (1998-2025). AKI was defined according to KDIGO serum creatinine criteria. Kidney damage (KD) was defined as chronic kidney disease (CKD) and/or hypertension at follow-up. Logistic and Cox regression analyses were used to identify risk factors and to calculate odds ratio (OR) and hazard ratio (HR), respectively. Ka

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by persistent inflammation, excessive oxidative stress, and progressive joint destruction. Although upadacitinib (UP) monotherapy exhibits robust anti-inflammatory efficacy, its systemic administration is limited by a narrow target profile and hepatotoxicity. To address these challenges, we rationally designed a biomimetic nanodrug (HA-M@SHK-Fe@BSA-UP NPs) co-delivering UP with shikonin (SHK), a natural naphthoquinone with antioxidant activity and hepatoprotective effects. This nanoplatform self-assembled via iron-mediated

Rheumatoid Arthritis (RA) is a crippling autoimmune disease characterized by gradual cartilage loss, bone degeneration, and persistent joint inflammation. Widespread adverse effects and ineffective drug distribution hamper the traditional treatment modalities. Recent progress in RA treatment has been advanced by nanocarrier-based phototherapies, including photodynamic therapy (PDT) and photothermal therapy (PTT). These therapies work by inducing necrosis or apoptosis in inflammatory cells through the generation of reactive oxygen species via PDT or localized heat production by PTT. This also l

The differential diagnosis of erosive osteoarthritis of the hand (EHOA) and psoriatic arthritis (PsA) is challenging, especially considering the absence of specific diagnostic biomarkers. The aim of the present study was to evaluate whether a pattern of microRNAs (miRNAs) (miR-21, miR-140, miR-146a, miR-155, miR-181a, miR-223), pro-inflammatory cytokines [interleukin (IL)-1β, IL-6, IL-17a, IL-23a, and tumor necrosis factor (TNF)-α], and adipokines (adiponectin, chemerin, leptin, resistin, and visfatin) could help to differentiate EHOA from PsA. Fifty patients with EHOA, fifty patients with PsA

This study aimed to explore the role of CD11b, CD38, and HLA-DR in saliva, linking rheumatoid arthritis (RA) and periodontitis. We hypothesized that their expression reflects shared immunological pathways and correlates with clinical periodontal variables. An observational case-control study was conducted with RA patients (cases), healthy participants, and individuals with degenerative chronic joint pain (controls). Salivary CD11b, CD38, and HLA-DR levels were measured using flow cytometry for the first time. Periodontal assessments included PPD, in a periodontal lesion severity index (PIRIM),

An 89-year-old male developed a persistent high fever (around 39 °C) approximately two weeks following endoscopic reduction of sigmoid volvulus. He had no history of hypercalcemia but was using diuretics and proton pump inhibitors. Renal and thyroid status were normal. He was largely bedridden and asymptomatic except for fever. Laboratory tests demonstrated elevated C-reactive protein (4.75 mg/dL), but some tumor markers (including CEA, CA19-9, and CA125), anti-nuclear antibodies, MPO-ANCA, PR3-ANCA, β-D-glucan, and interferon-gamma release assay were all negative. Urinalysis was unremarkable.

Isolated splenic tuberculosis (TB) is rare. Here, we report a case of isolated splenic TB in a 70-year-old man with rheumatoid arthritis (RA). The patient presented to the emergency department with a 3-day history of epigastric pain and hematemesis. For RA treatment, a combination of methotrexate (8 mg) and baricitinib (4 mg) had been initiated 2 years prior. Abdominal computed tomography (CT) scan revealed upper gastrointestinal hemorrhage and intrasplenic involvement. Following endoscopic hemostasis, we performed endoscopic ultrasound-guided fine-needle aspiration via the stomach due to susp

Antibiotics have been traditionally used to treat patients with infectious diseases. However, recent investigations have highlighted their immunomodulating features. Additionally, they have been used to treat patients with rheumatologic diseases of proven infectious etiology. Thus, an emerging body of literature is developing on the potential role of antibiotics in managing patients with rheumatologic diseases, which are primarily characterized by autoimmune-driven inflammation. We critically review the potential use of antibiotics in rheumatology, focusing on both their direct antimicrobial a
