VSP stabilization of lumbar neoplasms: technical considerations and complications.

Variable screw placement (VSP) plates and pedicle screw fixation were used to stabilize eleven lumbar neoplasms. Blood loss and complications were comparable to other methods of posterior segmental fixation, although operative times were longer. Fewer levels were fused than for systems using sublaminar hooks or wires, with 8/11 patients treated with two level fixation. Four preoperatively irradiated patients experienced 43% of all complications and had 70% of the major complications. Wound infections occurred in 18%, vascular injuries in 18%, and transient neurologic deficits in 36% of our pat

[Avulsion fractures of the distal part of the radius with anterior displacement in children].

The authors report 7 cases of fractures of the distal part of the radius, with a frontal displacement with children. This fracture is rare in pediatric traumatology. There is no particular type of mechanism that will provoke that fracture; in five cases the child was falling down on the palm of his hand and in two cases in a forced flexion. The problem about this fracture is the stability of the reduction. A proper function of the wrist is impossible with an imperfect result. The orthopedic treatment is often unable to stabilize the fracture. The authors need a surgical management using a plat

Lower extremity salvage and reconstruction by free-tissue transfer. Analysis of results.

The application of soft tissue or bone free-tissue transfers to lower extremity salvage or reconstruction was assessed in a review of 86 consecutive procedures done during a 36-month period. This group included 65 cases of free skin or muscle flap transfer and 21 cases of free vascularized bone transfer. In terms of tissue viability, the success rate was 80.2% (78.5% for the soft tissue group and 85.7% for the bone group). With due consideration of indications, the incidence of secondary sepsis in patients with chronic osteomyelitis, and other potential complications, free-tissue transfers are

[Reconstruction prosthesis after upper epiphyseal and diaphyseal resection of the femur for tumor. Results of 16 cases and a biomechanical study].

The authors have treated 16 cases of tumours of the upper end of the femur by resection over at least 12 centimetres and insertion of a massive prosthesis. The tumour was secondary in 9 cases and primary in 7. This treatment was used for metastases where conservative surgery was impossible or had failed. In primary tumours, when the excision was very extensive, the main problem was of stability of the prosthesis and its long-term future. A new type of total massive prosthesis, designed by the authors after biomechanical studies, is described.

[Surgical fixation of the spine in fractures in the thoracic and lumbar segments].

Posterior alloplastic fixation of a damaged spinal portion was accomplished in 68 patients. In 20 cases fracture was associated with spinal dislocation. In such cases when a compressive spinal fracture is accompanied with insignificant signs of spinal concussion one should follow an expectation policy, since an early operation may be inherent in some additional trauma and result in inhibition of regenerative processes. For spinal fixation Lavsan was employed in 23 patients, a tightening device-in 28, metallic CITO plates-in 17. In 6 objects fixation with the plates was associated with arthrode

Sexually dimorphic effects of prenatal alcohol exposure on the murine skeleton.

BACKGROUND: Prenatal alcohol exposure (PAE) can result in lifelong disabilities known as foetal alcohol spectrum disorder (FASD) and is associated with childhood growth deficiencies and increased bone fracture risk. However, the effects of PAE on the adult skeleton remain unclear and any potential sexual dimorphism is undetermined. Therefore, we utilised a murine model to examine sex differences with PAE on in vitro bone formation, and in the juvenile and adult skeleton. METHODS: Pregnant C57BL/6J female mice received 5% ethanol in their drinking water during gestation. Primary calvarial oste

Clavicle Shaft Non-Unions-Do We Even Need Bone Grafts?

Background: The surgical treatment of bony non-unions is traditionally performed with additional bone grafts when atrophic and/or stronger implants when hypertrophic. In the case of the clavicle shaft, however, in our experience, a more controversial method where no additional bone graft is needed leads to equally good consolidation rates, independent of the non-union morphology. This method requires the meticulous anatomical reconstruction of the initial fracture and fixation according to the AO principle of relative stability. Methods: A retrospective review following the STROBE guidelines w

Biomechanical comparison of a 3.5 mm anterior locking plate to cannulated screws with anterior tension band wiring in comminuted patellar fractures.

PURPOSE: To date, surgically treated multifragmentary patellar fractures are still associated with high rates of complications, such as i.e. secondary fracture displacement. Osteosynthesis is most frequently performed with screws and cerclages. To increase primary stability, locking plates have been introduced. However, there is still a lack of biomechanical data supporting the superiority of plate fixation compared to screw fixation with cerclages in these cases. The goal of the present study was to conduct biomechanical comparison of these two techniques under dynamic loading conditions. ME

Stability of Lisfranc injury fixation in Thiel Cadavers: Is routine fixation of the 1st and 3rd tarsometatarsal joint necessary?

BACKGROUND: There is debate as to whether a home run screw (medial cuneiform to 2nd metatarsal base) combined with k-wire fixation of the 4th & 5th tarsometatarsal joints is sufficient to stabilise Lisfranc injuries or if fixation of the 1st and 3rd tarsometatarsal joints is also required. Unlike the 2nd, 4th and 5th tarsometatarsal joints, stabilisation of the 1st and 3rd requires either intra-articular screw or an extra-articular plate which risk causing chondrolysis and/or osteoarthritis. The aims of this cadaveric study were to determine if routine fixation of the 1st and 3rd tarsometatars

Factors of mineralization dynamics - A mathematical model for microstructural bone remodeling.

Bone mineralization critically influences bone strength and fracture risk, yet the dependency on bone porosity across the range from cortical to trabecular bone remains incompletely understood. We present a novel mathematical framework that explicitly models bone mineralization dynamics as a function of bone turnover, which can be linked to porosity. Unlike existing fixed-porosity models, our approach incorporates porosity-dependent cellular activity based on remodeling surface availability, enabling analysis across the entire porosity range. We tested three biologically motivated hypotheses t

Self-sustained biomimetic bioelectronic accelerated metabolic reprogramming of bone regeneration.

Bionic bioelectronics has promising applications in bone defect repair, with current research primarily focusing on the development of electroactive biomaterials and self-powered systems, which can mimic the electrophysiological microenvironment of natural bone tissue, accelerating bone healing by promoting osteoblast proliferation and differentiation through electrical stimulation. However, the biological mechanisms of bionic electrical stimulation in bone defect repair remain incompletely understood. Here, the study developed a self-sustained biomimetic bioelectronic system comprising a trib

Craniogenetic studies in Sus scrofa: With emphasis on the 'orbitosphenoid' problem.

The orbitosphenoid is a skeletal element of the endocranium of extant mammals. However, it has also been described in many of their fossil ancestors. Craniogenetic studies show that it is composed of two types of bone: first, the cartilaginous ala orbitalis and parts of the trabecular plate are transformed by endochondral ossification; second, so-called 'appositional bone' ('Zuwachsknochen') arises directly from the perichondrium of the two optic pilae and spreads in all directions and overlays the remaining cartilage and the endochondral ossifications. For some time, both bone types can be di

Osteoclasts secrete leukemia inhibitory factor to promote abnormal bone remodeling of subchondral bone in osteoarthritis.

BACKGROUND: Osteoarthritis (OA) is a common chronic degenerative joint disease. At present, there is no effective treatment to check the progression of osteoarthritis. Osteochondral units are considered to be one of the most important structures affecting the occurrence and development of osteoarthritis. Osteoclasts mediate an increase in abnormal bone remodeling in subchondral bone in the early stage of osteoarthritis. Here, alendronate (ALN) that inhibit osteoclasts was used to study the regulatory effect of osteoclast-derived leukemia inhibitory factor (LIF) on early abnormal bone remodelin

Dietary Complex and Slow Digestive Carbohydrates Promote Bone Mass and Improve Bone Microarchitecture during Catch-Up Growth in Rats.

Catch-up growth is a process that promotes weight and height gains to recover normal growth patterns after a transient period of growth inhibition. Accelerated infant growth is associated with reduced bone mass and quality characterized by poor bone mineral density (BMD), content (BMC), and impaired microarchitecture. The present study evaluated the effects of a diet containing slow (SDC) or rapid (RDC) digestible carbohydrates on bone quality parameters during the catch-up growth period in a model of diet-induced stunted rats. The food restriction period negatively impacted BMD, BMC, and micr

Effect of Low-Intensity Pulsed Ultrasound (LIPUS) on Alveolar Bone during Maxillary Expansion Using Clear Aligners.

The present study evaluated the possible effect of low-intensity pulsed ultrasound (LIPUS) on buccal bone plate thickness and height after maxillary arch expansion using clear aligners. The cone beam computed tomography (CBCT) of before and immediately after maxillary arch expansion (3 mm per side) of 28 adult patients (18 in LIPUS group and 10 in control) (average age 36.2 ± 13.2 years old) was analyzed. The wearing protocol of clear aligners in the LIPUS group was to change the aligners every 4 to 5 days, while the wearing protocol in the control group (without LIPUS) was to change the align

Partially implantable bone conduction hearing aids without a percutaneous abutment (Otomag): technique and preliminary clinical results.

INTRODUCTION: Patients with air-bone gaps who cannot be corrected successfully by tympanoplasty or with mixed hearing loss may be treated with bone conduction hearing aids. Their disadvantages are the obvious external fixation components or the biological and psychosocial problems of open implants. We have developed new partially implantable bone conduction hearing aid without a percutaneous abutment and have been using them clinically for 4 years. The principle of these bone conduction hearing aids is a magnetic coupling and acoustic transmission between implanted and external magnets. The go

Secondary aneurysmal bone cyst in fibrous dysplasia of the proximal femur.

Fibrous dysplasia of bone is a congenital skeletal disorder characterized by proliferation of spindle cells inside an intramedullary collagenous stroma and by formation of metaplastic bone. An aneurysmal bone cyst is a lesion of unknown etiology. Macroscopically it appears like a blood-filled cavity that expands the affected bone. Histologically, fibrous septa are bordered by macrophages, giant cells, inflammatory cells, and areas of bone production. Hemorrhagic areas can rise into fibrous dysplasia. The lesion appears as an aggressive and destructive process that is difficult to diagnose prop

Modeling human skeletal development using human pluripotent stem cells.

Chondrocytes and osteoblasts differentiated from induced pluripotent stem cells (iPSCs) will provide insights into skeletal development and genetic skeletal disorders and will generate cells for regenerative medicine applications. Here, we describe a method that directs iPSC-derived sclerotome to chondroprogenitors in 3D pellet culture then to articular chondrocytes or, alternatively, along the growth plate cartilage pathway to become hypertrophic chondrocytes that can transition to osteoblasts. Osteogenic organoids deposit and mineralize a collagen I extracellular matrix (ECM), mirroring in v

[Hematogenous osteomyelitis due to clavicular fracture : A rare complication after conservative treatment].

We present a case of a nondislocated clavicular fracture that was conservatively treated for 4 weeks with a sling. The patient was a smoker and suffered from type 2 diabetes mellitus. After 2 months a suppurative infection occurred due to an escalation based on a clavicular osteomyelitis. Through antibiotic treatment and an aggressive surgical approach the infection was cured but ended up with a painful malunion of the clavicle. The bone reconstruction was achieved with a long locking plate with an additional vascularized medial femoral condyle free flap. After 18 months the patient was subjec

Stress-Induced, Aseptic Osteolysis of the Mid-Tibia in a Revision Hinged Total Knee Arthroplasty Mimicking Infection.

In this report, we present the case of an 80-year-old female with pain located over the tip of her cemented tibial stem in a revision hinge total knee arthroplasty with localized osteolysis that looked suspicious for infection. A thorough workup was negative for infection. We postulate that the osteolysis at the end of her tibial stem was initiated by a modulus of elasticity mismatch at the stem tip, which generated a focal area of increased sagittal bone bending and microparticle generation. She was treated with lesional exploration, debridement, synthetic bone grafting, and tibial plating to
