A token efficient vision framework using patch residual transformer for Alzheimer's disease diagnosis.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder, and early diagnosis using structural magnetic resonance imaging (sMRI) is crucial for timely intervention. Recently, the Vision Transformer (ViT) has been applied to sMRI-based AD diagnosis, but it often struggles with information redundancy when handling high-dimensional data, leading to focus loss and missed critical features. To address this, we proposed a Patch Residual Transformer (PRT)-a token-efficient vision framework designed for AD diagnosis and the prediction of mild cognitive impairment (MCI) conversion (i.e., st

From Harm to Harmful Acts: How Childhood Maltreatment Drives Unethical Pro-Organizational Behavior at Work.

Childhood maltreatment (CM) is a significant early-life experience that shapes cognitive, emotional, and moral development, influencing ethical decision-making in adulthood. While previous research has examined the impact of CM on personal and social behavior, its role in unethical pro-organizational behavior (UPB) remains underexplored. UPB refers to unethical actions employees undertake to benefit their organization, often rationalized through moral disengagement (MD). This study investigates the link between CM and UPB, emphasizing MD as a mediator and parenting quality (PAQ) as a moderator

The digital scalpel: navigating the future of surgical innovation.

BACKGROUND: Artificial intelligence is entering surgical practice not as an extension of the surgeon's hand, eyes, or dexterity, but of surgical judgment itself. This creates a growing informational asymmetry between well-resourced and under-resourced operative environments, with implications for training, competence, accountability, and equity. METHODS: This commentary, derived from the Karl Storz Lecture delivered at the SAGES 2026 Annual Meeting, proposes the Digital Scalpel: a five-layer conceptual architecture (data infrastructure, connected systems, computational vision, cognitive augme

Surgeon workload in supervised robotic colorectal surgery: a surgical task load index study.

BACKGROUND: Robot-assisted colorectal surgery (RAS) is increasingly adopted; however, surgeon workload during supervised training and its determinants remain unclear. METHODS: We conducted a single-center study combining a prospective surgical task load index (Surgical-TLX) assessment and a retrospective cohort analysis (October 2023-August 2025). Junior surgeons were defined as non-Japan Society for Endoscopic Surgery (JSES)-certified surgeons who had < 10 robot-assisted colorectal cases as the primary operator and were within 8 years after graduation. All junior cases were performed under d

Interpretable machine learning model based on multimodal MRI radiomics for Alzheimer's disease diagnosis.

OBJECTIVE: Alzheimer's disease (AD), the most common neurodegenerative disorder, is a leading cause of cognitive impairment and dementia in older adults. This study aimed to develop an interpretable machine learning model using multimodal MRI radiomics for the diagnosis of Alzheimer's disease. MATERIALS AND METHODS: A total of 110 subjects (48 AD, 62 healthy control subjects) underwent 3D T1WI, DWI, and T2WI scans. Radiomics features were extracted from eight AD-related brain regions and selected using a three-step approach: variance thresholding, independent t-test, and LASSO-all conducted s

Dual-Stream Fusion of Eye-Tracking and ECG Signals for Fatigue Detection in Remote Tower Air Traffic Controllers.

Fatigue detection in remote tower air traffic controllers is important for maintaining operational safety under sustained visual monitoring and high cognitive workload. This study proposes MFD-Net, a dual-stream multimodal fusion framework using eye-tracking and electrocardiogram (ECG) signals. The model separately encodes eye-tracking and ECG-derived temporal inputs, incorporates an ECG-derived RMSSD expert feature, and performs lightweight late fusion for fatigue-state classification. Under the mixed-subject random-window protocol, MFD-Net achieved an Accuracy of 85.20%, a Recall of 83.33%,

Unsupervised Robot-Assisted Therapy at Home After Stroke: a Pilot Feasibility Study.

Unsupervised robot-assisted therapy could allow increasing upper limb therapy dose for stroke survivors with minimal additional burden on the healthcare system. Thanks to the ability to actively assist movement and dynamically adapt the assistance level, actuated devices can support individuals with a wide range of deficits. However, these devices are often complex to use, and their application in a fully unsupervised setting has rarely been explored. Here, we present a pilot study investigating the feasibility of unsupervised therapy with ReHandyBot, an actuated device for upper limb rehabili

A Master Adaptive Learner Approach to Cognitive Disability Support in a US Urology Residency.

Medical education is witnessing a growing need for disability support, particularly for cognitive and psychiatric disabilities, yet many medical learners do not receive the necessary accommodations. These barriers may arise from uncertainty in how to approach multifaceted challenges, lack of disability recognition by both learners and educators, fear of stigma, and unclear or inadequate institutional policies and support systems. This case study about a urology resident in a U.S. training program, presented from the viewpoint of his competency development coach and residency program leadership

The Impact of Environment Design Bias on Working Memory.

Extended Reality (XR) is a powerful tool for training, education, and gaming. Research suggests that gender differences exist in XR environments including women having a lower sense of subjective presence and being more susceptible to motion sickness. However, the underrepresentation of women both as participants and researchers could lead to potential design biases, impacting the accuracy and inclusivity of XR systems. This work investigates subtle design differences in virtual environments on women's performance on a cognitive test. Non-male participants (n = 40) completed the Stroop Interfe

Chemogenetic Breakdown of the Dentate Gate Causes Seizures and Spatial Memory Deficits.

The dentate gyrus has often been posited to act as a gate that dampens highly active afferent input into the hippocampus. Effective gating is thought to prevent seizure initiation and propagation in the hippocampus and support learning and memory processes. Pathological changes to DG circuitry that occur in temporal lobe epilepsy (TLE) can increase DG excitability and impair its gating ability which can contribute to seizures and cognitive deficits. There is evidence that TLE pathologies and seizures may independently contribute to learning and memory deficits in TLE through distinct mechanism

Comprehensive Peri-Operative Risk Assessment and Management of Geriatric Patients.

Background: As the population ages, the prevalence of surgical interventions in individuals aged 65+ continues to increase. This poses unique challenges due to the higher incidence of comorbidities, polypharmacy, and frailty in the elderly population, which result in high peri-operative risks. Traditional preoperative risk assessment tools often fail to accurately predict post-operative outcomes in the elderly, overlooking the complex interplay of factors that contribute to risk in the elderly. Methods: A literature review was conducted, focusing on the predictive value of CGA for postoperativ
