The global health burden of infection-associated cancers in the year 2002.

Several infectious agents are considered to be causes of cancer in humans. The fraction of the different types of cancer, and of all cancers … mphotropic virus type I (0.03%) and the liver flukes (0.02%). There would be 26.3% fewer cancers in developing countries (1.5 million cases per year) and 7.7% in developed countries (390,000 cases) if these infectious diseases were prevented. The attributable fraction at the specific sites varies from 100% of cervix cancers attributable to the papilloma viruses to a t …

Recent Antimicrobial Resistance Situation and Mechanisms of Resistance to Key Antimicrobials in Enterotoxigenic Escherichia coli.

Enterotoxigenic Escherichia coli (ETEC) is a major cause of diarrhea in developing countries and is regularly imported into developed countries as a major cause of traveler's diarrhea. ETEC is usually self-limiting and not necessarily treated with antimicrobials, although antimicrobial treatment is recommended in malnourished children, severe cases, and traveler's diarrhea. However, resistant strains to representative therapeutic agents such as ciprofloxacin and azithromycin have been reported in recent years, and multidrug-resistant ETEC has also emerged. This review discusses the recent anti…

Vaping and smokeless tobacco control in South Asia: A policy review.

High prevalence of tobacco use is seen in low- and middle-income countries (LMIC). In the recent years, tobacco users have switched to alternatives falling under the framework of electronic nicotine delivery systems (ENDS). This review provides an overview of tobacco control-related policies in Bangladesh, India, and Pakistan, and suggested recommendations to bridge this gap to address Tobacco-free Nations. This paper's findings are relevant for developing countries worldwide that have a high tobacco-related health burden, a complex landscape of use, and inadequate resources to offer tobacco c…

A Rare Cause of Acute Appendicitis: Enterobius vermicularis.

Acute appendicitis is the most common intra-abdominal pathology that requires emergency surgery in general surgery clinics. The aetiology of acute appendicitis includes both infectious and non-infectious causes. Although parasitic diseases are rare in developed countries, they are more common in developing countries. The association between acute appendicitis and parasitic infections is unclear, and whether parasites are the cause of acute appendicitis is still under debate. In addition, many appendix parasites are encountered after appendectomy. In this study, we report a rare case of acute a…

Clinical hypoxic-ischemic encephalopathy score of the Iberoamerican Society of Neonatology (Siben): A new proposal for diagnosis and management.

Hypoxic ischemic encephalopathy is a major complication of perinatal asphyxia, with high morbidity, mortality and neurologic sequelae as cerebral palsy, mostly in poor or developing countries. The difficulty in the diagnosis and management of newborns in these countries is astonishing, thus resulting in unreliable data on this pathology and bad outcomes regarding mortality and incidence of neurologic sequelae. The objective of this article is to present a new clinical diagnostic score to be started in the delivery room and to guide the therapeutic approach, in order to improve these results.

Salmonella Enterica Serotype Paratyphi B - An Unusual Pathogen in Sepsis Neonatorum.

Sepsis remains a significant cause of morbidity and mortality in newborns especially in the developing countries. Salmonellosis, a global public health problem is common in tropical countries. However Salmonella enterica serotype Paratyphi B causing neonatal sepsis is rare and the survival depends on high index of suspicion and appropriate empiric therapy. We here report a case of sepsis neonatorum due to Salmonella enterica serotype Paratyphi B in a four day old baby girl. Salmonella infections should be considered in the differential diagnosis of neonatal sepsis, especially in endemic areas.

Anaesthetic management of a neonate with right sided congenital diaphragmatic hernia.

Neonates who are born with CDH suffer from sub-stantial morbidity and mortality. Modalities of treatment are said to influence outcome, apart from pre-natal and post-natal factors. Reports from developed countries have described improved survival with High Frequency Oscillatory Ventilation (HFOV), Extra Corporeal Membrane Oxygenation (ECMO) and inhaled Nitric Oxide (NO). Many centres in developing countries lack these neonatal advanced care facilities and outcome is expected to be different. In this case, we have focused on anaesthetic management of RCDH in a 5-day-old neonate, by using conven…

An unusually extensive case of cutaneous anthrax in a patient with type II diabetes mellitus.

Naturally acquired anthrax infection remains an important public-health problem in developing countries. Turkey is one of the countries in which the zoonotic form of anthrax may still be encountered. The most frequent portal of entry for anthrax spores is the skin. Although cutaneous anthrax is usually self-limiting, complications may arise in untreated cases. Underlying systemic disorders such as diabetes mellitus may confound the clinical picture and lead to atypical presentations. We present an unusually extensive case of cutaneous anthrax in a patient with newly diagnosed diabetes mellitus…

Immunogenic and Genetic Factors in Rheumatic Fever.

Although rheumatic fever has virtually disappeared in many developed countries, it remains the number one acquired heart disease among children in developing countries. Many hypotheses advanced to explain the pathogenic mechanisms of this disease include the concept of an abnormal humoral and cellular response within the host to streptococcal antigens cross-reacting with mammalian tissues. This report examines the most recent evidence supporting this concept. It emphasizes the importance of the cellular arm of the immune response in cardiac damage, as wall as possible genetic disposition to th…

Candidate parasitic diseases.

This paper discusses five parasitic diseases: American trypanosomiasis (Chagas disease), dracunculiasis, lymphatic filariasis, onchocerciasis and schistosomiasis. The available technology and health infrastructures in developing countries permit the eradication of dracunculiasis and the elimination of lymphatic filariasis due to Wuchereria bancrofti. Blindness due to onchocerciasis and transmission of this disease will be prevented in eleven West African countries; transmission of Chagas disease will be interrupted. A well-coordinated international effort is required to ensure that scarce reso…

Severe rheumatic mitral valve disease in children. Evaluation by echocardiography.

Severe rheumatic mitral stenosis with or without mitral insufficiency is virtually unseen in children in the affluent countries of the western world, but is not uncommon in developing countries. Fifteen cases of rapidly progressive rheumatic mitral valve disease in children are presented to illustrate the value of echocardiography in the diagnosis and evaluation of this lesion. Cardiac catheterization was performed in nine and simply confirmed the diagnosis made noninvasively. Our experience demonstrates that thorough echocardiographic examination in these cases frequently enables the examiner…

An interdisciplinary clinic in rural Tanzania - observations on chiropractic care in a developing nation.

It appears that a great many chiropractors and chiropractic institutions are involved in health care initiatives in developing countries. Developing nations present extraordinary opportunities to do good, but also carry risks, for practitioners and organizations, which may not be obvious prior to actual local engagement. This paper describes the guiding principles under which one international collaboration has evolved in rural Tanzania, a so-called 'low resource' setting where the majority of families subsist in extreme poverty. Several challenges to effective care are also identified.

Searching for the elusive typhoid diagnostic.

Typhoid (enteric) fever is still a common disease in many developing countries but current diagnostic tests are inadequate. Studies on pathogenesis and genomics have provided new insight into the organisms that cause enteric fever. Better understanding of the microorganisms explains, in part, why our current typhoid methodologies are limited in their diagnostic information and why developing new strategies may be a considerable challenge. Here we discuss the current position of typhoid diagnostics, highlight the need for technological improvements and suggest potential ways of advancing this a…

In many parts of the world particularly sub-Saharan Africa, congenital syphilis is a significant public health problem. Though it is rare in … rs with penicillin and in affluent countries it should be strengthened among those at high risk. In developing countries antenatal care screening needs to be strengthened by implementing point-of-care decentralised screening and treatment, also alternative novel approaches to control congenital syphilis should be looked at and utilized. [Sudden infant death syndrome (SIDS) and its forensic investigation].

During the past two decades tremendous efforts have been made by the medical community, especially in the fields of forensic medicine and pediatrics, to better understand the etiology, epidemiology and pathophysiology of SIDS. There have been many SIDS reports from developed countries, but few from developing Asian countries. Despite a recent significant decrease in the incidence of SIDS in many developed countries, SIDS continues to be the most common cause of post-neonatal infant death in these countries. This article analyzes the SIDS data (1990-2006) from the Office of the Chief Medical Ex
