Intracardiac pulsed field ablation: Proof of feasibility in a chronic porcine model.

BACKGROUND: Radiofrequency (RF) has become an accepted energy source for myocardial ablation but may result in discontinuous lesions and nontargeted tissue injury. We examined the feasibility and safety of lesion formation using high-amplitude, bipolar pulsed electric fields delivered from a multielectrode array catheter. OBJECTIVE: The purpose of this study was to compare duty-cycled radiofrequency ablation (RFA) to pulsed field ablation (PFA) in terms of acute electrical effects, 2-week lesion formation, and injury to nontargeted tissues. METHODS: Intracardiac ablations were performed in 6

Alterations of motor cortical excitability and anatomy in Unverricht-Lundborg disease.

Unverricht-Lundborg disease is the most common form of progressive myoclonus epilepsies. In addition to generalized seizures, it is characterized by myoclonus, which usually is the most disabling feature of the disease. Classically, the myoclonus has been attributed to increased excitability of the primary motor cortex. However, inhibitory cortical phenomena have also been described along with anatomical alterations. We aimed to characterize the relationship between the excitability and anatomy of the motor cortex and their association with the severity of the clinical symptoms. Seventy geneti

Effect of thoracoscopic splanchnic denervation on pain processing in chronic pancreatitis patients.

BACKGROUND: Central sensitisation due to visceral pancreatic nociceptive input may play an important role in chronic pancreatitis pain. Using quantitative sensory testing (QST), this first study investigates whether thoracoscopic splanchnic denervation (TSD), performed to reduce nociceptive visceral input, affects central sensitisation in chronic pancreatitis patients. PATIENTS AND METHODS: We studied 19 chronic pancreatitis patients (11 men, 8 women on stable opioid medication) and 18 healthy volunteers as preoperative controls. Preoperatively and 6 weeks after TSD, pain numeric rating score

A new method using neuromagnetic stimulation to measure conduction time within the cauda equina.

Using principles derived from electric field measurements and studies of phrenic nerve in vitro, neuromagnetic stimuli in humans were predicted to excite selective low threshold sites in proximal and distal cauda equina. Physical models, in which induced electric fields were recorded in a segment of human lumbosacral spine immersed in a saline filled tank, supported this prediction. Conclusions from the model were tested and confirmed in normal human subjects. Ipsilateral motor evoked potentials were elicited in lower limb muscles and striated sphincters by magnetic coil (MC) stimulation of bo

Computational Insights Into Neuromodulation: How Electric Fields Shape Motor Neuron Excitability.

Amyotrophic Lateral Sclerosis (ALS) is an incurable neurodegenerative disease characterized by the selective loss of spinal motor neurons (MN). Trans-spinal direct current stimulation (tsDCS) has emerged as a promising noninvasive neuromodulation technique that could provide neuroprotection and slow down disease progression. However, the mechanisms by which tsDCS affects individual MNs remain poorly understood. This study uses computational modeling to explore how low-intensity extracellular electric fields (EEFs), generated by tsDCS, influence the electrophysiological behavior of MNs. Morphol

NIR-Activated Eutectic Gels Producing Centripetal Thermoelectric Currents and CO Release to Modulate Calcium-Associated Repair of Infected Wounds.

Bacterial infection disrupts the transepithelial potential (TEP) and dysregulates the wound immune microenvironment, impairing tissue regeneration. However, current electrical stimulation strategies often lack stable directional output, wound-conformal integration, and simultaneous control over infection-associated inflammation. Here, we developed a near-infrared (NIR)-responsive eutectic gel (CBD, 1.25 S/m at 25°C) that generates centripetal thermoelectric stimulation and enables controlled carbon monoxide (CO) release under a single NIR input. Upon 808 nm NIR irradiation, CBD rapidly generat…

Cognitive control: exploring the causal role of the rTPJ in empathy for pain mediated by contextual information.

Empathy determines our emotional and social lives. Research has recognized the role of the right temporoparietal junction (rTPJ) in social cognition; however, there is less direct causal evidence for its involvement in empathic responses to pain, which is typically attributed to simulation mechanisms. Given the rTPJ's role in processing false beliefs and contextual information during social scenarios, we hypothesized that empathic responses to another person's pain depend on the rTPJ if participants are given information about people's intentions, engaging mentalizing mechanisms alongside simu

Using gamma-band transcranial alternating current stimulation (tACS) to improve sleep quality and cognition in patients with mild neurocognitive disorders due to Alzheimer's disease: A study protocol for a randomized controlled trial.

BACKGROUND: Sleep disturbances are highly prevalent in patients with age-related neurodegenerative diseases, which severely affect cognition and even lead to accumulated β-amyloid. Encouraging results from recent studies on transcranial direct current stimulation (tDCS) showed moderate positive effects on sleep quality in preclinical Alzheimer's disease (AD). Compared to tDCS, transcranial alternating current stimulation (tACS) enables the entrainment of neuronal activity with optimized focality through injecting electric current with a specific frequency and has significant enhancement effect

The Influence of Electrode Properties on Induced Voltage Gradient Along the Rat Optic Nerve.

Significant interest exists in the potential of electric field (EF) application to be developed into a technology to direct neuronal regeneration. In vitro, EFs were shown to direct the growth of retinal ganglion cell (RGC) axons, the neurons that make up the optic nerve. As larger EF gradients were shown to direct more efficient growth, investigations into the most effective stimulation strategies that can generate the greatest voltage gradient are needed before EF application can be developed into a technology to direct optic nerve regeneration in vivo. We performed ex-vivo experiments to co

The rubber hand illusion is a fallible method to study ownership of prosthetic limbs.

Enabling sensory feedback in limb prostheses can reverse a damaged body image caused by amputation. The rubber hand illusion (RHI) is a popular paradigm to study ownership of artificial limbs and potentially useful to assess sensory feedback strategies. We investigated the RHI as means to induce ownership of a prosthetic hand by providing congruent visual and tactile stimuli. We elicited tactile sensations via electric stimulation of severed afferent nerve fibres in four participants with transhumeral amputation. Contrary to our expectations, they failed to experience the RHI. The sensations w…

Therapeutic application of rTMS in neurodegenerative and movement disorders: A review.

Transcranial magnetic stimulation (TMS) is a non-invasive form of brain stimulation that makes use of the magnetic field generated when an electric current passes through a magnetic coil placed over the scalp. It can be applied as a single stimulus at a time, in pairs of stimuli, or repetitively in trains of stimuli (repetitive TMS, rTMS). RTMS can induce changes in brain activity, whose after-effects reflect the processes of long-term potentiation and long-term depression, as certain protocols, namely those using low frequencies (≤1 Hz) seem to suppress cortical excitability, while those usin

Contribution of the corticospinal tract to motor impairment in spina bifida.

We aimed to disentangle the proportional contributions of upper and lower motor neuron dysfunction to motor impairment in children with spina bifida. We enrolled 42 children (mean age, 11.2 years; standard deviation, 2.8 years) with spina bifida and 36 control children (mean age, 11.4 years; standard deviation, 2.6 years). Motor impairment was graded to severity scales in children with spina bifida. We recorded motor evoked potentials after transcranial and lumbosacral magnetic stimulation and compound muscle action potentials after electric nerve stimulation. Regarding lower motor neuron func

Consonant recognition as a function of the number of stimulation channels in the Hybrid short-electrode cochlear implant.

Consonant recognition was measured as a function of the number of stimulation channels for Hybrid short-electrode cochlear implant (CI) users, long-electrode CI users, and normal-hearing (NH) listeners in quiet and background noise. Short-electrode CI subjects were tested with 1-6 channels allocated to a frequency range of 1063-7938 Hz. Long-electrode CI subjects were tested with 1-6, 8, or 22 channels allocated to 188-7938 Hz, or 1-6 or 15 channels from the basal 15 electrodes allocated to 1063-7938 Hz. NH listeners were tested with simulations of each CI group/condition. Despite differences

A single session of emotional stress produces anxiety in Wistar rats.

The sensorial stimulation arising from a physically stressed (PS) subject may produce emotional stress in a witnessing partner (WP). Both members of the pair develop functional changes. We tested changes in locomotor activity (crossing) and in the defensive burying test in WP, and PS adult male Wistar rats having been submitted to a single 10 min session in a two-compartment cage. During this session, the WP rats received auditory and olfactory stimulation coming from a PS pair submitted to unavoidable electric footshocks (1 mA, dc, 0.5s, 0.5c/s, 10 min). This experiment was replicated in othe

Pain control in recovering alcoholics: effects of local anesthesia.

OBJECTIVE: The medical literature suggests that alcoholics may present greater challenges to achieving clinical pain control than nonalcoholics. This study was undertaken to test the hypotheses that: (1) significant differences exist between alcoholics and nonalcoholics on detection and pain thresholds during electric tooth stimulation; (2) group differences exist in the depth and time course of pulpal anesthesia; and (3) responses to tooth stimulation are associated with severity of alcoholism and/or other psychological factors. METHOD: Male alcoholics (n = 22) in aftercare treatment (mean l

Millisecond-Resolved Electronic pH Microdomains Synchronize Calcium Amplification in Cell Clusters.

A central goal for bioelectronic interfaces is to convert electrical inputs into local chemical commands that regulate cell signaling with defined space and time. However, imposing spatially confined extracellular pH (pHo) cues across cell populations with externally programmed millisecond timing remains difficult. Here, we show that an electronically generated pHo microdomain can act as a timed biochemical command that synchronizes intracellular Ca2+ amplification in HL-1 cardiomyocyte clusters. Using a microfabricated electrochemical interface, electronic stimulation produced a spatially con

A neuronal architecture underlying autonomic dysreflexia.

Autonomic dysreflexia is a life-threatening medical condition characterized by episodes of uncontrolled hypertension that occur in response to sensory stimuli after spinal cord injury (SCI)1. The fragmented understanding of the mechanisms underlying autonomic dysreflexia hampers the development of therapeutic strategies to manage this condition, leaving people with SCI at daily risk of heart attack and stroke2-5. Here we expose the neuronal architecture that develops after SCI and causes autonomic dysreflexia. In parallel, we uncover a competing, yet overlapping neuronal architecture activated

Conducting 1D nanostructures from light-stimulated copper-metalated porphyrin-dibenzothiophene.

Control over the dimensionality of stimulated organic semiconductors has aroused significant interest in organic electronics; however, the design of such materials still remains to be decided. Herein, we have developed three dibenzothiophene-appended freebase, zinc-metalated and copper-metalated porphyrin derivatives (PFb-DBT, PZn-DBT and PCu-DBT) in which PCu-DBT leads to an anion-binding complex in chloroform upon the application of light, resulting in self-assembled 1D nanostructures with high electrical conductivity. Nevertheless, light-stimulated freebase and zinc-metalated P-DBT undergo
