To determine safe and effective exercise testing techniques for musculoskeletally handicapped individuals, 20 patients (mean age, 61 years) were studied. Types of handicaps included various degrees of paralysis of arms and legs, and leg amputations. Seventeen of the 20 patients had concurrent cardiovascular problems. All were currently hospitalized, engaged in various physical therapy exercises, and referred for exercise testing to increase rehabilitation therapy or for cardiovascular evaluation. Of 20 tests, arm ergometry was used in 15, leg or combination arm-leg ergometry in four, and tread

Two risk factors for cardiovascular disorders, parental history of hypertension and the Type A behavior pattern, were investigated concurrently with respect to cardiovascular reactivity to challenging situations. Sixty-four college males were given both the Structured Interview (SI) and Jenkins Activity Survey (JAS) for the Type A behavior pattern and a family health questionnaire to determine parental history of hypertension. The students were monitored for blood pressure (BP), heart rate (HR), and pulse transit time (PTT) response to four tasks: cold pressor, isometric handgrip exercise, a r

Acetylcholine in the central nervous system appears to be involved in some aspects of hypertension. Clonidine and methyldopa may inhibit acetylcholine (Ach) release in several brain areas. The present study was therefore designed to determine whether a depletion of brain Ach could modify the antihypertensive effect of clonidine in freely moving spontaneously hypertensive (SHR), Grollman hypertensive (GHR) and DOCA-salt hypertensive (DHR) rats. Intravenous injection of clonidine (15, 30 and 75 micrograms/kg) reduced mean arterial pressure (MAP) and heart rate (HR) in all hypertensive animals. T

The short- and long-term effects of diuretic treatment with chlorthalidone 50 mg/day on haemodynamic and hormonal parameters in patients with essential hypertension (WHO grade I) were investigated. After three days of treatment, all patients showed a rise in plasma renin activity (PRA), plasma aldosterone (PA), urinary norepinephrine excretion (UNE) and heart rate (HR), and a decrease in body weight (BW) and extracellular volume (ECV) and blood volume (BV); the change in blood pressure (BP) was variable. The changes in BP were correlated with those in BV. After three months of therapy, the sig

A 40-month retrospective review of 677 postterm pregnancies was undertaken to determine the usefulness of 24-hour urinary estrogen per gram of creatinine (E/Cr) in monitoring such pregnancies. The cutoff value between normal and low E/Cr values was chosen at the tenth percentile (18 mg/gm). The corrected perinatal mortality rate (PMR) in post-term pregnancies with normal E/Cr was 0.23% (1/426) and was not different from that at term (0.23%; 4/1,775). However, in post-term pregnancies with low E/Cr, the corrected PMR was 9.2% (4/43), representing a 40-fold increase (P less than 0.001). The inci

OBJECTIVES: Reverse Potts shunt is a promising yet high-risk therapy for pediatric pulmonary arterial hypertension. Postoperative hemodynamics is critically influenced by shunt configuration but is difficult to predict. This study aimed to quantify the effects of shunt size and location on hemodynamics to guide surgical planning.
METHODS: Based on a patient-specific model, four postoperative models with two different shunt locations [left pulmonary artery (LPA)-descending aorta (DAO) and pulmonary artery bifurcation-aortic arch] and three conduit sizes (4, 5, and 6 mm) were created. The direc

BACKGROUND: Stress in academic settings arises from the interplay between perceived demands such as exams, deadlines, and academic workload and an individual's coping resources. While academic stress (AS) is frequently examined as a separate construct, the stress encountered in an academic environment encompasses both academic and non-academic stressors that students face throughout their university experience. This study examined the longitudinal associations between stress in an academic context on key psychological, physiological, and behavioral variables in university students.

BACKGROUND: Medical students commonly experience Medical Student Syndrome (MSS), a condition where they compare their vague symptoms to the medical problems and life-threatening diseases they are learning about in medical school, even though their health profile is free. Our research aims to investigate the symptoms of MSS (anxiety-related illness) and hypochondriasis, as well as their impact on the lives of students. Additionally, we aim to study various types of personalities and, finally, investigate the demographic determinants of MSS and their interactions with various personality types a

BACKGROUND: The pediatric palliative care (PPC) team's mission is to assist and promote the highest quality of life for children with life-threatening and life-limiting illnesses and their families. The whole care of these patients implies sometimes challenging clinical and emotional situations. PPC providers may be exposed to psychological distress.
AIMS: The study's objectives were to evaluate the psychological well-being of healthcare providers working in the Regional Center for Palliative Care and Pediatric Pain Therapy, including their stress levels, levels of depression, anxiety, and ri

BACKGROUND: The etiology of exercise intolerance in heart failure with preserved ejection fraction (HFpEF) is multifactorial. Several contributing pathways may be improved by ketone ester (KE).
OBJECTIVES: This study aims to determine whether KE improves exercise tolerance in HFpEF.
METHODS: KETO-HFpEF (Ketogenic Exogenous Therapies in HFpEF) is a randomized, crossover, placebo-controlled trial of acute KE dosing in 20 symptomatic HFpEF participants. Coprimary endpoints include peak oxygen consumption (VO2) during incremental cardiopulmonary exercise testing and time to exhaustion during an

BACKGROUND: Exposure to air pollution including contemporary sources like wildland fire smoke worsens cardiovascular outcomes. Although several mechanisms for these effects have been postulated, one underexplored impact of inhaled air pollution that may mediate adverse health outcomes is sleep disruption, which is an independent risk factor for cardiovascular morbidity and a trigger of multiple biological pathways linked to disease. The purpose of this study was to determine whether cardiovascular responses to air pollution, especially excursions in blood pressure, are associated with contempo

BACKGROUND: Patients with spinal cord injury (SCI) who develop urinary tract infection (UTI) present differently than the non-SCI population. UTIs can cause loss of quality of life and even lead to life-threatening complications including urosepsis. Challenges in SCI management include distinguishing symptomatic UTI from asymptomatic bacteriuria (ASB), which occurs often in patients with SCI, and the lack of standardization in UTI diagnosis in SCI.
OBJECTIVE: This study aims to set the foundation for the development of a risk score to improve diagnostic accuracy of UTI after SCI.

AIMS: Maintenance of euvolaemia with diuretics is critical in heart failure (HF) patients with chronic kidney disease (CKD); however, it is challenging because no reliable marker of volume status exists. Fractional excretion of urea nitrogen (FEUN) is a useful index of volume status in patients with renal failure. We aimed to examine whether FEUN is a surrogate marker of volume status for risk stratification in HF patients with CKD.
METHODS AND RESULTS: We examined 516 HF patients with CKD (defined as discharge estimated glomerular filtration rate < 60 mL/min/1.73 m2 ) whose FEUN was measured

BACKGROUND: Exercise limitation in chronic obstructive pulmonary disease (COPD) is commonly attributed to abnormal ventilatory mechanics and/or skeletal muscle function, while cardiovascular contributions remain relatively understudied. To date, the integrative exercise responses associated with different cardiopulmonary exercise limitation phenotypes in COPD have not been explored but may provide novel therapeutic utility. This study determined the ventilatory, cardiovascular, and metabolic responses to incremental exercise in patients with COPD with different exercise limitation phenotypes.
