The use of non-steroidal anti-inflammatory drugs for patent ductus arteriosus closure in preterm infants.

Over the last four decades, non-steroidal anti-inflammatory drugs have been widely used to induce closure of the patent ductus arteriosus (PDA) in preterm infants. Evidence to support this practice is lacking, despite performance of >50 randomized trials. The credibility of those trials may have been compromised by high rates of open treatment in controls, era of study prior to advent of modern practices, or inclusion of insufficient numbers of very immature infants. Meta-analyses show little impact of those factors on main conclusions. Essentially all trials reporting important long-term outc

Association of Rho-kinase Gene Polymorphisms with Respiratory Distress Syndrome in Preterm Neonates.

BACKGROUND: Respiratory distress syndrome (RDS) of the newborn is one of the most common causes of morbidity and mortality in preterm infants. Our objective was to determine the association between Rho-kinase (ROCK1 and ROCK2) gene polymorphisms and RDS in preterm neonates. METHODS: A total of 193 preterm infants with RDS and 186 preterm infants without respiratory problems were included in this study. Polymorphisms were analyzed in genomic DNA using a BioMark 96.96 dynamic array system. RESULTS: We observed that ROCK1 gene rs2271255 (Lys222Glu) and rs35996865 polymorphisms, and ROCK2 gene r

Pulmonary hypertension in bronchopulmonary dysplasia.

Pulmonary hypertension is common in bronchopulmonary dysplasia and is associated with increased mortality and morbidity. This pulmonary hypertension is due to abnormal microvascular development and pulmonary vascular remodeling resulting in reduced cross-sectional area of pulmonary vasculature. The epidemiology, etiology, clinical features, diagnosis, suggested management, and outcomes of pulmonary hypertension in the setting of bronchopulmonary dysplasia are reviewed. In summary, pulmonary hypertension is noted in a fifth of extremely low birth weight infants, primarily those with moderate or

Appendicitis in infancy.

Acute appendicitis is the most common cause of abdominal pain requiring surgery in children. But it is an uncommon entity in young children and rare in infants. During a 10-year period, between January 1991 and December 2000, 7 infants (age from 17 days to 8 months) were treated for acute appendicitis at Changhua Christian Hospital. All of the preoperative symptoms and signs, the duration between admission and operation, pathology reports, and laboratory data were reviewed. There were no specific clinical signs and symptoms. The duration between admission and operation ranged from 2 hours to 5

Spontaneous gastrointestinal perforation in very-low-birth-weight infants--a rare complication in a neonatal intensive care unit.

Over a 6-year period (1989-1995), gastrointestinal (GI) perforation was diagnosed in nine preterm infants (mean gestational age 27 weeks, mean birth weight 872 g). Three presented with necrotizing enterocolitis (NEC), two with indwelling-tube-induced perforation of the stomach, one with small-left-colon syndrome, and another with meconium ileus. Spontaneous intestinal perforation occurred in two similar very-low-birth-weight (VLBW) infants, in the distal ileum, on days 8 and 9 of life, respectively. The only clinical sign was extensive abdominal distension, and abdominal X-ray studies revealed

Delivery of the very low birthweight breech: what is the best way for the baby?

A retrospective analysis was done to determine whether vaginally vs. cesarean section-born breech infants in the very low birthweight range are at increased risk for morbidity and mortality. Eighty-three viable singleton breech infants weighing 700-1,600 g, who were delivered in our medical center during the period 1980 through 1993, were followed for up to 5 years of age. Analysis of data after correction for weight, gestational age and other confounding variables such as antepartum complications revealed that survival rates in the 700-1,000 g group were similar in both routes of delivery. In

Anaesthetic management of bone marrow transplant recipients less than two years of age.

The charts and anaesthetic records of 97 infants less than two years of age who underwent bone marrow transplantation at the University of Minnesota from 1978-1992 were retrospectively reviewed. These infants underwent 564 general anaesthetics. There were 48 perioperative complications, most (39) involving the airway. There were 20 difficult intubations occurring in 13 patients. The causes of the difficult intubations were anatomical abnormalities (12), mucositis (4), pharyngeal oedema (3) and emesis upon induction of anaesthesia (1). Four intraoperative deaths occurred. The deaths were caused

Infections in newborn infants in a special care unit. A changing pattern of infection.

An analysis of infections in the intensive and intermediate care nurseries (special care unit) of the Hospital of the University of Pennsylvania was made over a three year period. From January 1982 to September 1984, 98 bacteremias were identified in 2571 infants, giving an overall incidence of 3.8 per 100 infants admitted to the special care unit and 11.8 per 1000 hospital births; 7 of 98 infants died, all from early onset disease. Fifty-eight percent of bacteremias were nosocomial. Gram positive organisms accounted for 89 percent of all bacteremias, with coagulase negative staphylococcus and

Effects of indomethacin in premature infants with patent ductus arteriosus: results of a national collaborative study.

Among 3559 newborn infants with birth weight less than 1750 gm, 421 developing a hemodynamically significant patent ductus arteriosus were entered into a randomized trial to evaluate the role of indomethacin in the management of PDA. Indomethacin given concurrently with usual medical therapy at the time of diagnosis resulted in ductal closure in 79%, versus 35% with placebo (P less than 0.001). Indomethacin as backup to usual medical treatment resulted in similar closure rates. To assess overall effects through hospital discharge, three management strategies were compared. Although mortality d

[Analysis of dipalmitoyl-phosphatidyl-choline (DPPC) in amniotic fluid: a new sensitive method for surfactant determination].

The surface active alveolar system (surfactant) is of central importance for normal lung function in newborn infants and its lack remains a frequent cause of perinatal mortality and morbidity in premature infants, as well as those born at term. The most important substance of this system is the lecithin dipalmitoyl-phosphatidyl-choline (DPPC). Many different methods to obtain information concerning lung maturity through analysis of phospholipids in the amniotic fluid have been tested, but all of them give only indirect assessment of the DPPC content. A new method for the separation and quantif

Surgical versus medical treatment with cyclooxygenase inhibitors for symptomatic patent ductus arteriosus in preterm infants.

BACKGROUND: Patent ductus arteriosus (PDA) with significant left to right shunt in preterm infants increases morbidity and mortality. Early closure of the ductus arteriosus may be achieved pharmacologically using cyclooxygenase inhibitors or by surgery. The efficacy of both treatment modalities is well established. However, the preferred initial treatment of a symptomatic PDA in a preterm infant, surgical ligation or treatment with indomethacin, has not been well established. OBJECTIVES: To compare the effect of surgical ligation of PDA vs. medical treatment with cyclooxygenase inhibitors (us

Isolated deep orbital haemangioma in an infant presenting to the emergency department with right-sided proptosis.

Proptosis in early childhood can be an alarming and stressful situation for parents and paediatricians. Different types of anatomical structures can be found in the orbit, such as nerves, muscles, veins, arteries, glandular and connective tissue and hence the differential diagnosis of unilateral proptosis can be vast. In some cases, orbital disease has the potential to cause substantial morbidity and mortality. Appropriate medical history, examination and diagnostic evaluation with imaging can assist with a prompt diagnosis and management. In young infants, a heightened level of suspicion shou

Interactions between Hypertension and Breastfeeding: What Do We Know?

Substantial evidence indicates that breastfeeding reduces mortality and morbidity in infants. However, social changes in the 20th century resulted in a considerable decline in breastfeeding rates in many countries. Breast milk is crucial because of its nutritional, immunological, and emotional benefits and economic value. Approximately 10% of pregnancies are complicated by hypertensive syndromes, which are the most commonly diagnosed conditions during pregnancy. This perspective aims to explore how hypertension may interfere with the quality of human breast milk. While numerous studies have in

Meconium aspiration syndrome: from pathophysiology to treatment.

Meconium aspiration syndrome (MAS) is a clinical condition characterized by respiratory distress in neonates born through meconium-stained amniotic fluid (MSAF). Despite advances in obstetric practices and perinatal care, MAS remains an important cause of morbidity and mortality in term and post-term newborns. Since the 1960s, there have been significant changes in the perinatal and postnatal management of infants born through MSAF. Routine endotracheal suctioning is no longer recommended in both vigorous and non-vigorous neonates with MSAF. Supportive care along with new treatments such as su

Pseudomonas aeruginosa necrotizing bronchopneumonia.

Extremely low birth weight (ELBW) infants are at particularly high risk for infection due to an immature immune system, invasive procedures such as endotracheal intubation, intravascular catheterization, and other factors. Neonatal infections in this population are associated with a high mortality, poor growth, and neurodevelopmental outcomes. Pseudomonas aeruginosa (P. aeruginosa) infection is an uncommon but potentially devastating cause of pneumonia and sepsis in the ELBW population. P. aeruginosa is an important cause of healthcare-associated infections (HAI) or nosocomial infections. P. a

Management of severe hypertension in the newborn.

Blood pressure is considered a vital sign, as values too low or too high can be related with serious morbidity and mortality. In neonates, normal blood pressure values undergo rapid changes, especially in premature infants, making the recognition of abnormal blood pressures more challenging. Severe hypertension can occur in neonates and infants and is a medical emergency, often manifesting with congestive heart failure or other life-threatening complications. The cause or risk factors for the hypertension can usually be identified and may guide management. Most classes of antihypertensive medi

Pulmonary interstitial emphysema after resolution of relapsing pneumothorax and discontinuation of mechanical ventilation. An atypical case in a preterm infant.

UNLABELLED: Pulmonary interstitial emphysema (PIE) is a frequent complication in preterm-ventilated infants. These newborns have an increased risk of mortality and bronchopulmonary dysplasia. Various treatments for PIE have been proposed: high-frequency ventilation, postural therapy, selective bronchial intubation or surgery. We present an atypical case of a preterm infant with respiratory distress syndrome complicated by pneumothorax, who after recovery and discontinuation of mechanical ventilation, developed unilateral PIE which was treated with selective bronchial intubation. CONCLUSIONS:

Brachial thrombosis in a premature neonate. A case report.

Arterial injuries in infants are rare and in most cases are of iatrogenic origin as a consequence of catheterization, venipuncture or arterial blood sampling. These lesions require an accurate, noninvasive clinical diagnosis and prompt exploration and reconstruction using microvascular techniques to restore perfusion and to avoid morbidity and even mortality. We present a 3-day-old infant with a brachial artery thrombosis by a white thrombus as a consequence of an unsuccessful attempt to introduce an intravascular catheter. A microvascular reconstruction was performed, with complete restoratio

Exposure to mercury during the first six months via human milk and vaccines: modifying risk factors.

Breastfeeding is the best natural protection infants have against morbidity and mortality, and the development of safe and effective vaccines has made it possible to immunize children against infectious disease. Both of these mechanisms for ensuring good health in children may be compromised by contact with mercury (Hg). Maternal exposure to environmental Hg during pregnancy can predispose nursing children to neurodevelopmental disorders. Despite the World Health Organization assurance that thimerosal-preserved vaccines are safe to use in infants, the United States, the European Union, and doz

Pertussis in infants, children, and adolescents: diagnosis, treatment, and prevention.

Pertussis, or "whooping cough," caused by the gram-negative pleomorphic bacillus Bordetella pertussis, is a highly contagious, potentially life-threatening respiratory tract illness that has re-emerged worldwide as a cause of substantial morbidity and mortality in infants, children, and adolescents, despite high vaccination rates. Increased awareness and reporting, in addition to the availability of better diagnostic tests, partially explain the recent resurgence of pertussis. However, waning immunity after childhood immunization has resulted in a growing pool of susceptible adolescents and ad
