Cardiorespiratory Model-Based Data-Driven Approach for Sleep Apnea Detection.

Obstructive sleep apnea (OSA) is a chronic sleep disorder affecting millions of people worldwide. Individuals with OSA are rarely aware of the condition and are often left untreated, which can lead to some serious health problems. Nowadays, several low-cost wearable health sensors are available that can be used to conveniently and noninvasively collect a wide range of physiological signals. In this paper, we propose a new framework for OSA detection in which we combine the wearable sensor measurement signals with the mathematical models of the cardiorespiratory system. Risk factors of chronic periodontitis on healing response: a multilevel modelling analysis.

BACKGROUND: Chronic periodontitis is a multifactorial polygenetic disease with an increasing number of associated factors that have been identified over recent decades. Longitudinal epidemiologic studies have demonstrated that the risk factors were related to the progression of the disease. A traditional multivariate regression model was used to find risk factors associated with chronic periodontitis. However, the approach requirement of standard statistical procedures demands individual independence. Multilevel modelling (MLM) data analysis has widely been used in recent years, regarding thor

Particle Filtering and Sensor Fusion for Robust Heart Rate Monitoring Using Wearable Sensors.

This paper describes a novel methodology leveraging particle filters for the application of robust heart rate monitoring in the presence of motion artifacts. Motion is a key source of noise that confounds traditional heart rate estimation algorithms for wearable sensors due to the introduction of spurious artifacts in the signals. In contrast to previous particle filtering approaches, we formulate the heart rate itself as the only state to be estimated, and do not rely on multiple specific signal features. Instead, we design observation mechanisms to leverage the known steady, consistent natur

Joint Inertial Sensor Orientation Drift Reduction for Highly Dynamic Movements.

Inertial sensor drift is usually corrected on a single-sensor unit level. When multiple sensor units are used, mutual information from different units can be exploited for drift correction. This study introduces a method for a drift-reduced estimation of three dimensional (3-D) segment orientations and joint angles for motion capture of highly dynamic movements as present in many sports. 3-D acceleration measured on two adjacent segments is mapped to the connecting joint. Drift is estimated and reduced based on the mapped accelerations' vector orientation differences in the global frame. Algor

Facilitating informed decisions about breast cancer screening: development and evaluation of a web-based decision aid for women in their 40s.

BACKGROUND: Expert groups and national guidelines recommend individualized decision making about screening mammography for women in their 40s at low-to-average risk of breast cancer. We created Breast Screening Decisions (BSD), a personalized, web-based decision aid, to help women decide when to start and how often to have routine screening mammograms. We evaluated BSD in a large, prospective pilot trial of women and their clinicians. METHODS: Women ages 40-49 were invited to use BSD before a scheduled preventive care visit. One month post-visit, users were asked about decisional conflict, kn

Mobile Technology for Community Health in Ghana: what happens when technical functionality threatens the effectiveness of digital health programs?

BACKGROUND: Despite the growing use of technology in the health sector, little evidence is available on the technological performance of mobile health programs nor on the willingness of target users to utilize these technologies as intended (behavioral performance). In this case study of the Mobile Technology for Health (MOTECH) program in Ghana, we assess the platform's effectiveness in delivering messages, along with user response across sites in five districts from 2011 to 2014. METHODS: MOTECH is comprised of "Client Data Application" (CDA) which allows providers to digitize and track ser

Substance deposition assessment in obstructed pulmonary system through numerical characterization of airflow and inhaled particles attributes.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) and asthma are considered as the two most widespread obstructive lung diseases, whereas they affect more than 500 million people worldwide. Unfortunately, the requirement for detailed geometric models of the lungs in combination with the increased computational resources needed for the simulation of the breathing did not allow great progress to be made in the past for the better understanding of inflammatory diseases of the airways through detailed modelling approaches. In this context, computational fluid dynamics (CFD) simulations acco

Disparities in the use of mobile phone for seeking childbirth services among women in the urban areas: Bangladesh Urban Health Survey.

BACKGROUND: In Bangladesh, similar to its other South Asian counterparts, shortage of health workers along with inadequate infrastructure constitute some of the major obstacles for the equitable provision of reproductive healthcare services, particularly among the marginalized and underserved neighbourhoods. However, given the rapidly expanding broadband communication and mobile phone market in the country, the application of eHealth and mHealth technologies offer a window of opportunities to minimise the impact of socioeconomic barriers and promote the utilization of maternal healthcare servi

Safe Opioid Prescription: A SMART on FHIR Approach to Clinical Decision Support.

Background Prescription opioid pain medication overuse, misuse and abuse have been a significant contributing factor in the opioid epidemic. The rising death rates from opioid overdose have caused healthcare practitioners and researchers to work on optimizing pain therapy and limiting the prescriptions for pain medications. The state of New York has implemented a prescription drug monitoring program(PDMP), amended public health law to limit the prescription of opioids for acute pain and utilized the resources of the state and county health departments to help in curbing this epidemic. The rece

Asthmatic Wheeze Detection From Compressively Sensed Respiratory Sound Spectra.

Quantification of wheezing by a sensor system consisting of a wearable wireless acoustic sensor and smartphone performing respiratory sound classification may contribute to the diagnosis, long-term control, and lowering treatment costs of asthma. In such battery-powered sensor system, compressive sensing (CS) was verified as a method for simultaneously cutting down power cost of signal acquisition, compression, and communication on the wearable sensor. Matching real-time CS reconstruction algorithms, such as orthogonal matching pursuit (OMP), have been demonstrated on the smartphone. However,

Ultrasound-Based Sensing Models for Finger Motion Classification.

Motions of the fingers are complex since hand grasping and manipulation are conducted by spatial and temporal coordination of forearm muscles and tendons. The dominant methods based on surface electromyography (sEMG) could not offer satisfactory solutions for finger motion classification due to its inherent nature of measuring the electrical activity of motor units at the skin's surface. In order to recognize morphological changes of forearm muscles for accurate hand motion prediction, ultrasound imaging is employed to investigate the feasibility of detecting mechanical deformation of deep mus

Leveraging public health's participation in a Health Information Exchange to improve communicable disease reporting.

BACKGROUND: Infectious diseases can appear and spread rapidly. Timely information about disease patterns and trends allows public health agencies to quickly investigate and efficiently contain those diseases. But disease case reporting to public health has traditionally been paper-based, resulting in somewhat slow, burdensome processes. Fortunately, the expanding use of electronic health records and health information exchanges has created opportunities for more rapid, complete, and easily managed case reporting and investigation. To assess how this new service might impact the efficiency and

Evaluation of Three Algorithms for the Segmentation of Overlapping Cervical Cells.

In this paper, we introduce and evaluate the systems submitted to the first Overlapping Cervical Cytology Image Segmentation Challenge, held in conjunction with the IEEE International Symposium on Biomedical Imaging 2014. This challenge was organized to encourage the development and benchmarking of techniques capable of segmenting individual cells from overlapping cellular clumps in cervical cytology images, which is a prerequisite for the development of the next generation of computer-aided diagnosis systems for cervical cancer. In particular, these automated systems must detect and accuratel

An Effective Occipitomental View Enhancement Based on Adaptive Morphological Texture Analysis.

This paper aims to present an algorithm that specifically enhances maxillary sinuses using a novel contrast enhancement technique based on the adaptive morphological texture analysis for occipitomental view radiographs. First, the skull X-ray (SXR) is decomposed into rotational blocks (RBs). Second, each RB is rotated into various directions and processed using morphological kernels to obtain the dark and bright features. Third, a gradient-based block segmentation decomposes the interpolated feature maps into feature blocks (FBs). Finally, the histograms of FBs are equalized and overlaid local

Generalized Feature Extraction for Wrist Pulse Analysis: From 1-D Time Series to 2-D Matrix.

Traditional Chinese pulse diagnosis, known as an empirical science, depends on the subjective experience. Inconsistent diagnostic results may be obtained among different practitioners. A scientific way of studying the pulse should be to analyze the objectified wrist pulse waveforms. In recent years, many pulse acquisition platforms have been developed with the advances in sensor and computer technology. And the pulse diagnosis using pattern recognition theories is also increasingly attracting attentions. Though many literatures on pulse feature extraction have been published, they just handle

Prediction and detection models for acute kidney injury in hospitalized older adults.

BACKGROUND: Acute Kidney Injury (AKI) occurs in at least 5 % of hospitalized patients and can result in 40-70 % morbidity and mortality. Even following recovery, many subjects may experience progressive deterioration of renal function. The heterogeneous etiology and pathophysiology of AKI complicates its diagnosis and medical management and can add to poor patient outcomes and incur substantial hospital costs. AKI is predictable and may be avoidable if early risk factors are identified and utilized in the clinical setting. Timely detection of undiagnosed AKI in hospitalized patients can also l

Measuring the operational impact of digitized hospital records: a mixed methods study.

BACKGROUND: Digitized (scanned) medical records have been seen as a means for hospitals to reduce costs and improve access to records. However, clinical usability of digitized records can potentially have negative effects on productivity. METHODS: Data were collected during follow-up outpatient consultations in two NHS hospitals by non-clinical observers using a work sampling approach in which pre-defined categories of clinician time usage were specified. Quantitative data was analysed using two-way ANOVA models and the Mann-Whitney U test. A focus group was held with clinicians to qualitativ
