Anatomy of the anterior sacroiliac joint with reference to lumbosacral nerves.

There are no detailed descriptions of the neural structures that may be seen during surgical interventions of the pelvis. Anatomic dissections were performed to see which nerves are endangered in approaches to the anterior sacroiliac joint for plate fixations. Sixty cadavers were dissected bilaterally. Fifty-one were male and nine were female. L4 and L5 nerve roots were followed along the sacroiliac joint from the intervertebral foramen to the entrance into the lesser pelvis. Measurements were made between the nerves and sacroiliac joint from the proximal end of the joint to the pelvic brim. T

Innervation of nociceptors in the menisci of the knee joint: an immunohistochemical study.

Using histology, we studied the innervation of nociceptors in the medial and lateral menisci of the knee joint. Specimens examined were taken from 16 patients during arthroplasty. The patients were 6 men and 10 women, with ages ranging from 14 to 76 years (mean 56 years). Immunohistochemistry with the unlabeled antibody biotin-streptavidin method was employed to detect protein gene product 9.5 (PGP 9.5) or substance P (SP) in the specimen. The antibody for PGP 9.5 detected nerve tissues in the menisci. Most but not all of the nerve fibers were associated with blood vessels. Nerve fibers and se

Tumors and tumorous masses presenting as temporomandibular joint syndrome.

OBJECTIVE: Neoplasms of the temporomandibular joint (TMJ) usually mimic common causes of TMJ syndrome, leading to delay in diagnosis. To increase awareness of TMJ neoplasms and establish guidelines for early intervention, we performed a retrospective analysis of a series of patients with neoplasms of the TMJ. STUDY DESIGN AND SETTING: A retrospective review of the records of patients with neoplasms of the TMJ from 1990 to 1997 was done. RESULTS: Six patients were identified. The neoplasms included benign and malignant neoplasms. The time from initial presentation to final diagnosis was in mo

[Corrective osteotomy in the treatment of degenerative changes in the knee joint].

Angular deformity of the knee can be corrected by high tibial osteotomy or distal femoral osteotomy. In this research we have analysed effects of corrective osteotomy in knees with varus or valgus deformity. In this research we present a long-term evaluation of 67 patients older than 40 with angular deformity of the knee in which high tibial osteotomy and distal femoral osteotomy were performed at the Department of Orthopaedic Surgery, School of Medicine, University of Zagreb, in the period 1982-1992. Operation was performed bilaterally in 14 patients. There were 66 knees with varus deformity

MR imaging of lumbar facet joint synovial cysts.

The increasing application of magnetic resonance (MR) imaging of the spine has raised the awareness of lumbar facet synovial cysts (LFSC). This well recognised, yet uncommon condition, presents with low back pain and radiculopathy due to the presence of an extradural mass. The commonest affected level is L4/5 with a mild degenerative spondylolisthesis a frequent associated finding. MR imaging is the technique of choice to detect and diagnose a LFSC. This pictorial essay, drawing on experience of 43 cases seen in 40 patients, illustrates the spectrum of appearances that can be encountered and s

Computed tomographic findings of dogs with cubital joint lameness.

The purpose of this study was to identify the incidence of abnormal computed tomography (CT) findings in dogs with lameness of one or both elbows. CT examinations of 102 elbows in 51 dogs were reviewed individually and by group consensus by 3 examiners and graded for various abnormalities. Frequently encountered abnormalities included abnormal shape and sclerosis of the medial coronoid process of the ulna, irregularity of the radial incisure of the ulna, and ulnar trochlear notch sclerosis. Other abnormalities included distinct/separate or fissure/in situ fragmentation of the medial coronoid;

Imaging of degenerative and posttraumatic disease in the shoulder joint with ultrasound.

This article reviews the examination technique of shoulder ultrasound, normal and abnormal ultrasound findings in acute (posttraumatic) and chronic (degenerative) lesions. Moreover, it reviews the effectiveness of ultrasound in relation to magnetic resonance (MR) imaging. Most authors report that full-thickness tears of the supraspinatus can reliably be diagnosed by ultrasound. However, the simple diagnosis of a full-thickness rotator cuff tear is no longer sufficient for surgical management. The precise localization and size of rotator cuff tears as well as the extent of muscle degeneration i

Surgical correction of temporomandibular joint ankylosis in two cats.

Two cats with temporomandibular joint ankylosis were examined. Difficulty in opening the mouth and malocclusion were associated with trauma and fracture around the zygomatic arch and the condyle of the mandible after an automobile accident. Unilateral condylectomy or aggressive caudal mandibulectomy was performed to improve quality of life. Excision arthroplasty of the temporomandibular joint improved mandibular function and appearance. Some malocclusion remained but had no clinical significance.

The role of aspirin for thromboembolic disease in total joint arthroplasty.

Fatal PE is a real problem and cannot be minimized. However, it must be appropriately balanced to other perioperative risks. It appears that fatal PE, as reported in the recent literature, is the same no matter which chemoprophylactic agent is used, with a rate of approximately 0.1% to 0.2%. With the use of aspirin as chemoprophylaxis after TKA, we have demonstrated the risk of fatal PE to be 0.15%. Because of our experience, the current data in the literature, and the need to carefully balance risk, we continue to use aspirin as our prophylactic agent of choice.

Warfarin responses in total joint and hip fracture patients.

Warfarin is considered as a narrow therapeutic drug-an agent for which small changes in dosage can lead to significant changes in response. The precision of warfarin treatment is especially relevant in an acute-care setting. In this study, we examined the effect of various risk factors on warfarin response in the early postoperative period after total joint arthroplasty and hip fracture fixation. A total of 260 patients placed on warfarin for thrombosis prophylaxis were retrospectively studied. Risk factors for warfarin sensitivity include age 80 years or older and hip fracture fixation. Patie

MR imaging of inflammatory joint diseases of the foot and ankle.

Pain affecting the foot and ankle is a common complaint frequently attributable to inflammatory joint diseases. Although conventional radiography is regarded as the initial step in the diagnostic investigation, MR imaging may contribute to further evaluation of these patients due to the direct visualization of the inflammatory soft tissue formed in the disease and its effects on bone, cartilage and para-articular structures. The high spatial resolution of MR imaging combined with tissue characterization often allows initial detection of inflammatory joint abnormalities at a stage that precedes

[The surgical treatment of degenerative-dystrophic diseases of the hip joint].

An analysis of long-term results of surgical treatment of 609 patients with degenerative-dystrophic diseases of the hip-joint was made. Choice of the optimum methods of surgical treatment of such patients must be based on the prognosis of effectiveness of the operative intervention taking into account general regularities associated with the degree of the arthrosis alterations, dyscongruence of the articulation surfaces and duration of the disease, the degree of disturbances of the femur head blood supply, the disturbed supporting ability and severity of the trauma. When choosing the method of

[Current problems in the diagnosis and treatment of injuries to the temporomandibular joint].

Complex examination of 2103 patients with temporomandibular joint (TMJ) injuries was carried out. This examination included contrast arthrotomography, computed tomography (CT) and MR tomography of the TMJ. It gas been established, that in mandibular condyle fractures not only bone structures of the TMJ, but also soft tissue ones (the articular disk and capsular-ligamentous apparatus) are being injured. Our investigation has shown that condylar fractures in the region of condylar base and neck are combined with the injuries of the articular disk and capsular-ligamentous apparatus in 54.1 +/- 5.

Meniscal subluxation: association with osteoarthritis and joint space narrowing.

OBJECTIVE: Since complete meniscectomy leads to knee OA, we investigated the potential links among meniscal subluxation, joint space narrowing and symptomatic OA. MATERIALS AND METHODS: 233 cases with symptomatic knee OA and 58 asymptomatic controls underwent radiography and MR imaging of the knee. Joint space narrowing was measured on weight-bearing PA fluoroscopy-positioned radiographs. The amount of medial or lateral meniscal subluxation was measured on coronal MR images. The prevalence and severity of meniscal subluxation was compared in cases and controls. We evaluated the correlation of

Upper airway compression after arthroscopy of the temporomandibular joint.

An unusual complication is presented following a temporomandibular arthroscopy carried out under general anaesthesia. Severe cervicofacial oedema occurred immediately after surgery which required prolonged endotracheal intubation. Retrospective analysis revealed a massive fluid escape in the surrounding tissues leading to laryngeal oedema.
