Biological studies of schizoaffective disorders.

Biological studies of the relationships between the schizoaffective disorders, the affective disorders, and schizophrenia suggest that no simple reductionist model is supported by currently available data. Thus, both affective and schizoaffective patients but not schizophrenics, manifest abnormalities such as decreased platelet serotonin (5-HT) uptake, blunted clonidine-induced increase in serum growth hormone, shortened latency of rapid eye movement (REM) sleep, and increased REM density. However, there are some types of studies which show greater similarity between schizoaffective and schizo

Opsoclonus-myoclonus syndrome attributable to hepatitis C infection.

Opsoclonus-myoclonus syndrome is a rare neurologic disorder characterized by multidirectional chaotic eye movements, myoclonus in the limbs, and ataxia. We present a 9-year-old boy with opsoclonus-myoclonus syndrome associated with hepatitis C infection. To the best of our knowledge, this is the first report of opsoclonus-myoclonus syndrome associated with hepatitis C infection.

Nonmotor complications in Parkinson's disease.

Parkinson's disease (PD) is most often considered a disorder of movement. Whereas bradykinesia, rigidity, tremor, and postural instability result in disability, nonmotor complications in PD may be of equal or greater significance in some patients. This review will discuss many of the nonmotor complications in PD, including cognitive, autonomic, sleep, and sensory difficulties that may occur.

Risus sardonicus after thalamic haemorrhage.

We describe an uncommon movement disorder after stroke. A 70-year-old man was admitted for a right thalamic haemorrhage and 1 week later developed bilateral contractions of the face. Electromyographic study revealed a bilateral facial dystonia. The association of this bilateral facial dystonia and vertical gaze palsy produced the aspect of a "risus sardonicus."

Three sibs with phalangeal anomalies, microcephaly, severe mental retardation, and neurological abnormalities.

This paper describes three children of a Pakistani first cousin marriage with a distinctive, non-progressive disorder characterised by variable phalangeal anomalies, microcephaly, pre- and postnatal growth retardation, poor vision, dystonic movements, a characteristic face, and severe mental retardation. This combination of features seems to be distinct and to represent a new autosomal recessive syndrome.

Problem kids and preventive medicine: the making of an odd couple.

This paper maintains that the preventive medical model is inappropriate for dealing with childhood developmental disorders. The history of preventive medicine is traced briefly, and its adoption by the mental hygiene, child guidance, and community mental health movements is discussed. It is concluded that a program of screening, diagnosis, and treatment for poor children will have negative consequences for those it is meant to serve.

Sleep Disorders and Sleep Problems in Patients With Tourette Syndrome and Other Tic Disorders: Current Perspectives.

Sleep disorders seem to be a frequent complaint of patients diagnosed with Tourette syndrome (TS) or chronic or persistent tic disorders (CTD or PTD). In this review, we expanded a previously used search using 4 well-known databases up to February 15, 2022, looking for the coexistence of global and/or specific sleep disorders and polysomnographic studies performed on patients with TS/CTD/PTD. The references of interest in the topic were selected by hand. Sleep disorders in general, insomnia, different arousal disorders, the persistence of tics during sleep, excessive daytime sleepiness, and pe

Reconsidering the Clinical Outcome, Effect of Sex, and Diagnostic Criterion of Primary Complex Motor Stereotypies in Children.

BACKGROUND: Complex motor stereotypies (CMS) are repetitive movements classified into "primary" for typically developing individuals and "secondary" for those with other neurological or neurodevelopmental disorders. This study evaluated children diagnosed with primary CMS to assess motor stereotypy outcomes, sex-related differences, and early neurodevelopmental histories that play a key role in diagnostic classification. METHODS: Sixty-five healthy participants diagnosed with pCMS (44 males, 21 females), mean age 9.1 ± 3.6 years (range 3-17 years), completed follow-up telephone interviews and

Sleep Bruxism in Respiratory Medicine Practice.

Sleep bruxism (SB) consists of involuntary episodic and repetitive jaw muscle activity characterized by occasional tooth grinding or jaw clenching during sleep. Prevalence decreases from 20% to 14% in childhood to 8% to 3% in adulthood. Although the causes and mechanisms of idiopathic primary SB are unknown, putative candidates include psychologic risk factors (eg, anxiety, stress due to life events, hypervigilance) and sleep physiologic reactivity (eg, sleep arousals with autonomic activity, breathing events). Although certain neurotransmitters (serotonin, dopamine, noradrenalin, histamine) h

Enhanced antisaccade abilities in children with Tourette syndrome: the Gap-effect Reversal.

Tourette Syndrome (TS) is a childhood onset disorder of motor and vocal tics. The neural networks underlying TS overlap with those of saccade eye movements. Thus, deviations on saccadic tasks can provide important information about psychopathology of TS. Tourette syndrome often coexists with Attention Deficit Hyperactivity Disorder (ADHD) and Obsessive Compulsive Disorder (OCD). Hence, we manipulated various components of a saccade task to measure its effects on saccades of children with TS-only, TS+ADHD, TS+ADHD+OCD and healthy controls. Children looked toward (prosaccade) or in the opposite

Age-related changes in human blinks. Passive and active changes in eyelid kinematics.

PURPOSE: The authors analyzed eyelid kinematics in normal aging subjects to test the hypothesis that eyelid movements exhibit age-related changes and that blink disorders prevalent among the elderly, in turn, represent an outcome of normal aging processes. METHODS: The electromagnetic search coil technique was used to study blinks in normal human subjects for each decade from 40 to 89 years. Blink metrics (amplitude, peak velocity, and duration), main sequence relationships, and conjugacy were assessed. RESULTS: Mean amplitude and peak velocity of blinks decreased with age for spontaneous bl

The association between narcolepsy and REM behavior disorder (RBD).

BACKGROUND AND PURPOSE: Rapid eye movement (REM) sleep Behavior Disorder (RBD) is a movement disorder associated with loss of REM-related muscle atonia and is characterized by complex, vigorous and frequently violent dream-enacting behavior during REM sleep. RBD is usually idiopathic or secondary to neurological problems such as Parkinson's disease. This study looked at the association of RBD with another sleep disorder, narcolepsy. PATIENTS AND METHODS: Seventy-eight questionnaires were sent to known narcoleptics chosen at random from those with contact details available at the center. The q

Sleep Disorders in the Elderly.

Sleep disturbances in the elderly may not be a result of the aging process per se, but rather are likely caused by many factors that are amenable to treatment. These factors include medical and psychiatric problems, medications, and circadian rhythm changes, all of which can cause difficulties during sleep at night, and can lead to complaints of insomnia. Other factors that cause disturbances include a high prevalence of specific sleep disorders such as sleep disordered breathing (SDB), periodic limb movements during sleep (PLMS) and rapid eye movement (REM) sleep behavior disorder (RBD). [Disorders of motor functions after removal of parasagittal meningiomas].

Specific features of motor activity disorders were studied in 87 patients before and after removal of a parasagittal meningioma. Motor disorders are most severe in localization of the tumor in the region of the middle third of the superior sagittal sinus and falx and less severe when the tumor is found in the posterior parts of the anterior third of the superior sagittal sinus. The disorders are more marked after operation with extensive resection of areas of the superior sagittal sinus involves in the tumor and with occlusion of the formed venous drainage passages. The motor disorders are par

Sick sinus syndrome: A harbinger of anti-LGI1 encephalitis.

Autoimmune encephalitis usually presents with short-duration neuropsychiatric syndromes, mainly behavioral with cognitive dysfunction, recurrent seizures, and movement disorders. Among them, anti-LGI1 encephalitis has faciobrachial dystonic seizure as a pathognomic feature. The two cases reported here exemplify another peculiar feature of anti-LGI1 encephalitis, which has rarely been reported and is therefore not typically emphasized. Both cases were initially treated for cardiac symptoms until the full-blown features of encephalitis became evident. The key to preventing adverse outcomes is ea…

Challenges of equitable access to device-aided therapies for advanced Parkinson's Disease in Poland - expert consensus and treatment recommendations.

INTRODUCTION: In Poland, not all forms of device-aided therapies for advanced Parkinson's Disease (APD) are currently available. MATERIAL AND METHODS: We aimed to produce a consensus recommendation from Polish movement disorders experts after discussing gaps in the APD care pathway in Poland. RESULTS: Rescue therapy with apomorphine (APO) PEN injection and levodopa-entacapone-carbidopa intestinal gel infusion are not included in Poland's Specialist Therapeutic Programme, and are thus not reimbursed. For APO infusion, only the medication is reimbursed but not the device. CONCLUSIONS: Consens…

Gabapentin-induced Multifocal Myoclonus.

BACKGROUND: Gabapentin is the most commonly preferred agent for neuropathic pain in general practice as it is usually well tolerated, but occasionally, its toxicity may occur at standard doses, especially in elderly individuals, even without any prior comorbidities. CASE: We present an elderly male with normal renal parameters, who was started on gabapentin for neuropathic pain. He developed multifocal myoclonus all over the body within few days after starting gabapentin and subsided completed after withdrawal of the drug. CONCLUSION: Acute hyperkinetic movement disorders such as multifocal …

How do teeth erupt?

The development of normal occlusion requires eruptive migration of teeth from their developmental position in the jaw into a functional position within the oral cavity. This process involves significant and coordinated movement in an axial direction and appropriate eruption through the gingival tissues. The mechanisms regulating these developmental events are poorly understood, and teeth retain eruptive potential throughout their lifespan. In recent years, the use of mouse models has helped to elucidate some of the underlying molecular and biological mechanisms of mammalian tooth eruption. Her

Dream recall frequency and psychosomatics.

The ability to remember dreams can depend on a wide range of factors such as personality, creativity, mental state, cognitive functions as well as somatic symptoms. In the course of their studies, medical researchers have demonstrated that about 80% patients, woken up at their Rapid Eye Movement (REM) sleep phase, can remember their own dreams, whereas, in clinical practice, young adults can remember their dreams on their awakenings only once or twice a week. Let us now come to the point: in this study we suggest some hypotheses that could explain the reason why patients suffering from psychos

"Face of a Giant Panda" and "Beating Wings" in a Young Male.

Mutations in the gene coding for ATPase copper transporting beta polypeptide (ATP7B) cause Wilson's disease, located on chromosome 13. It has mainly hepatic and neurological presentations. Movement disorders are a characteristic finding in Wilson's disease, and "wing-beating tremors" are classical characteristics found on physical examination. We came across and managed a case of Wilson's disease with primarily neurological presentation with classical wing-beating tremors and "face of a giant panda" on radiology. As the patient had very typical findings and he also improved with the treatment,…
