PURPOSE: The binding of hematogenously borne malignant cells that express the carbohydrate sialyl Lewis X (sLe(X)) to selectin adhesion receptors on leukocytes, platelets, and endothelial cells facilitates metastasis. The glycosylation inhibitor, per-O-acetylated GlcNAcbeta1,3Galbeta-O-naphthalenemethanol (AcGnG-NM), inhibits the biosynthesis of sLe(X) in tumor cells. To evaluate the efficacy of AcGnG-NM as an antimetastatic agent, we examined its effect on experimental metastasis and on spontaneous hematogenous dissemination of murine Lewis lung carcinoma and B16BL6 melanoma cells.

Medullary thyroid carcinoma (MTC) is a neuroendocrine malignancy of the thyroid C cells. Spread of MTC commonly occurs to cervical and mediastinal lymph nodes. MTC cells do not concentrate radioactive iodine, and are not sensitive to hormonal manipulation. Because of these features, the treatment of metastatic or recurrent MTC is different from the treatment of differentiated thyroid cancer. The effectivity of the treatment of malignant adrenal tumors (MAT) depends on primary complete tumor resection with its regional metastases (RM). The possible regional metastases of malignant adrenal tumors in 257 patients have been studied. One third of the patients were diagnosed with regional metastases. We describe a patient with adenocarcinoma of the lung who developed hydronephrosis secondary to compression by right common iliac lymph node metastases. The most common primary sites of cancers causing ureteral obstruction are the cervix, prostate, bladder and colo-rectum. To date, few reports of ureteral obstruction attributable to lung cancer have been published. Although rare, physicians should be aware that hydronephrosis can complicate the course of patients with non-small cell lung cancer.

A 38-year-old man underwent a left lower lobectomy and lingulectomy for bronchiectasis. On macroscopic examination the lungs were bronchiectatic. Microscopically, multiple tumorlets were detected in the lung parenchyma. One peribronchial lymph node contained a tumorlet. We discuss the case and review the literature on this rare entity.

Clinical features and the management of adrenal insufficiency are considered with reference to etiology, diagnosis, and treatment. Specifically considered are the management of patients with Addison's disease and metastases. Adrenal insufficiency may go unrecognized in the cancer patients because features such as nausea, vomiting, anorexia as well as orthostatic hypotension are not specific and may be mistakenly attributed to advancing malignancy or side effects of therapy. Prompt recognition and treatment of adrenal insufficiency can avert potentially life-threatening situations.

The technique of extended cervical mediastinoscopy is described in detail as well as its indications for use. The author has found extended cervical mediastinoscopy extremely valuable in staging lung carcinoma with regard to level V and VI lymph node involvement if a standard cervical mediastinoscopy fails to demonstrate metastatic disease and a CT scan suggests subaortic lymph node involvement. This technique has been found to be safe and accurate and only adds minimal operating time to a cervical mediastinoscopy without the need for a second incision.

We report a 55-year-old man with angiosarcoma of the cecum who presented with generalized colicky abdominal pain with a mass in the periumbilical region. The lesion at surgery was a colo-colic intussusception extending to the midtransverse colon. A radical right hemicolectomy was done. Metastases to regional lymph nodes were noted in the specimen.
