OK-432 and lymphatic malformations in children: the Starship Children's Hospital experience.

BACKGROUND: Surgery has previously been the mainstay of treatment for lymphatic malformations but has attendant problems of marked scarring, high chance of recurrence and potential nerve damage. Alternative management for these lesions involves the intralesional injection of OK-432. The present paper reviews OK-432 use in lymphatic malformations in children. METHODS: A retrospective chart review was carried out of children undergoing intralesional OK-432 therapy from the Departments of Paediatric Surgery, Paediatric Otolaryngology and Plastic Surgery at Starship Children's Hospital, Auckland.

A Curious Case Report: Should a Clinician Be Worried About Bone Resorption Under a Chin Augmentation Site?

Oftentimes patients are concerned about their facial esthetics and appearance. To achieve the desired look, patients have several augmentation procedures available to them. Appearance of the chin and its morphology plays a crucial role in facial aesthetics. It is an important anatomic component that not only helps in the definition of the jawline and face but is also required from a functional aspect. It is not uncommon in the field of plastic surgery for patients to undergo chin reconstruction and recontouring due to the chin deformities such as microgenia and jaw asymmetry. The treatment opt

Mycobacterium chelonae wound infections after plastic surgery employing contaminated gentian violet skin-marking solution.

From April 1 to October 31, 1985, postoperative surgical-wound infections due to rapidly growing mycobacteria developed in eight patients undergoing cosmetic plastic surgery performed by one surgeon. All infections followed either face-lift or augmentation-mammoplasty procedures performed in the surgeon's office; no infections occurred after surgical procedures performed at the hospital or after other surgical procedures performed at the office. An epidemiologic investigation implicated a gentian violet skin-marking solution as the source of the infections (P less than 0.001). Among patients e

[The parascapular fasciocutaneous flap in the surgical treatment of deep burns in the axillary area].

An original operative approach to severe defects of the axilla--periscapular fasciocutaneous flap according to Pontaine--is recommended. It is a matter of a procedure which surgeons dealing with the problems of plastic and reconstructive surgery become acquainted with for the first time. A classification of axillary defects, contributing to the most adequate choice of operative approach, is also suggested. Typical cases where this operative method is used are illustrated.

How Patient Decision-Making Characteristics Affect Satisfaction in Facial Plastic Surgery: A Prospective Pilot Study.

BACKGROUND: The main objective of this study was to prospectively analyze which personality traits, clinical psychiatric states, and patient decision-making characteristics predict who will be less satisfied after facial plastic surgery. METHODS: This prospective study enrolled 60 adult subjects into one of three groups: aesthetic, functional, and reconstructive facial plastic surgery procedures (n = 20 in each group) from November of 2011 to February of 2016. Self-report surveys of personality traits (i.e., NEO Personality Inventory-Revised), psychiatric state (i.e., Patient Health Questionn…

Gynecomastia: The 4Dx technique.

In surgery for gynecomastia, it is not sufficient to just remove the gland or do a liposuction that addresses the front of the chest only as it is not aesthetically pleasing for the patient and the surgeon alike. Most patients expect to achieve a sculpted look, which includes not only the breast area but also the surrounding areas such as the sides, the axillae, and the infraclavicular region. To tackle these areas and achieve a well-sculpted and aesthetic look, we describe the 4Dx (4 Directions) technique of liposuction for gynecomastia. It also makes the procedure more objective for plastic

Timing of hard palatal closure: a critical long-term analysis.

The controversy about timing of cleft palate surgical procedures is focused on early palatoplasty for improved speech versus delayed hard palate repair for undisturbed facial growth. Timing and technique of palate repair are the most important influences on speech and facial growth, yet there is no consensus on the age or technique for surgery. The Oxford Cleft Palate Study was initiated to evaluate critically the long-term follow-up of 44 patients with early versus late closure of the hard palate. A multidisciplinary approach was used to determine the incidence of speech deficiencies, palatal

Non-embolic complications of gluteal fat transfer: A systematic review of the literature.

BACKGROUND: Gluteal fat grafting is a key component of aesthetic body contour surgery and ranks among the most frequently performed cosmetic procedures worldwide. While thromboembolic complications, such as fat embolism, dominate the literature due to their severe and often fatal outcomes, non-embolic complications have received less attention and standardization. These include infections, fat cysts, sciatic nerve injuries, and fat necrosis, which, despite being less severe, can significantly impact patient outcomes. OBJECTIVES: To determine the prevalence of non-embolic complications in glut

Does the participation of a senior plastic surgeon improve conditions of hospitalisation in craniosynostosis repair surgery?

INTRODUCTION: The real impact of the participation of other surgical specialties together with neurosurgeons on perioperative care in craniosynostosis repair surgery has not been determined. The purpose of this study was to determine whether the participation of a second senior surgeon (plastic surgeon) during surgical repair of pediatric monosutural craniosynostosis improved perioperative medical care. MATERIAL AND METHODS: The authors retrospectively reviewed 2 cohorts of patients who had consecutively undergone primary repair surgery for trigonocephaly and unicoronal craniosynostosis. Infa

The "midface-lift" as a misnomer for correctly identifying procedures designed to lift and rejuvenate the cheeks and malar regions of the face.

OBJECTIVE: To demonstrate that a classic temporal cheek rhytidectomy results in substantial and acceptable rejuvenation of the so-called midface and that additional surgery is not necessary to improve a sagging cheek, the melolabial fold, and the position of the corner of the mouth and the lateral corner of the eye. DESIGN: A retrospective observational study of 53 patients seen at the McCollough Plastic Surgery Clinic between 2005 and 2007. Each patient underwent temporal and cheek face-lifting surgery for various indications. All procedures were performed by the same surgeon, and the surgic

Functional Anatomic Computer Engineered Surgery Protocol for the Management of Self-Inflicted Gunshot Wounds to the Maxillofacial Skeleton.

PURPOSE: Virtual surgical planning (VSP) is an indispensable aid in craniomaxillofacial reconstruction, yet no protocol is established in facial gunshot wounds. We review our experience with computer-aided reconstruction of self-inflicted facial gunshot wounds (SIGSW'S) and propose a protocol for the staged repair. METHODS: A retrospective case series enrolling patients with SIGSW's managed with the Functional Anatomic Computer Engineered Surgical protocol (FACES) was implemented. Subjects were evaluated at least one month postoperatively. Outcome variables were jaw position, facial projectio

Sialendoscopy as treatment of face aesthetic surgery complications: technical note.

OBJECTIVE: The demand for cosmetic surgery on the face and body has increased worldwide. Brazil is currently the world leader in open plastic surgery and second on facelift, buccal fat removal, injectables (fillers), and thread lifts. The incidence of injuries to salivary glands, ducts, and facial nerves related to cosmetic procedures is uncommon but devastating. Treatment includes dressings, botulinum toxin, anticholinergics, drainages and salivary gland resection. Sialendoscopy has been successfully described for the treatment of ductal diseases with gland preservation. We present five cases

Double plastic stenting for inoperable malignant biliary stricture among cirrhotic patients as a possible cost-effective treatment: a pilot study.

BACKGROUND AND STUDY AIM: Endoscopic retrograde cholangiopancreatography (ERCP) has evolved as the main therapeutic intervention for hepatobiliary disorders. Palliative stenting for inoperable cases is associated with better morbidity and mortality than surgery. This work aimed at assessing the effect of insertion of two plastic stents in inoperable malignant biliary stricture among cirrhotic patients regarding stent patency, quality of life (QOL), and cost. PATIENTS AND METHODS: This multicenter study included 72 cirrhotic patients presented for ERCP with an inoperable malignant biliary stri

Diplopia following cosmetic surgery.

Cosmetic blepharoplasty is the most often performed procedure in plastic surgery and ophthalmic plastic surgery. Many complications have been reported including ectropion, exposure keratitis, hemorrhage, epiphora, infection, vision loss, and, very rarely, diplopia. Although uncommon, there have been several case reports and case series published evaluating possible causes of this problem, including aggressive excision of fat, wound-related inflammation, conjunctival edema, and temporary muscle disturbances. Although the inferior oblique muscle is most susceptible to injury, the inferior rectus

Financial Toxicity in Plastic Surgery: A Scoping Review.

INTRODUCTION: Financial toxicity (FT) describes the physical, psychosocial, and financial burden of health care-related costs on patients' well-being. Plastic surgery patients may be susceptible to FT due to high procedure and adjunct therapy costs. Despite these risks, FT resulting from plastic and reconstructive surgery remains unclear. This scoping review assessed the prevalence and impact of FT on plastic surgery patients. METHODS: MEDLINE, Embase, Web of Science Core Collection, Scopus, and the Cumulative Index to Nursing and Allied Health Literature were searched from inception through

The incidence, morbidity, and mortality of surgical procedures after orthotopic heart transplantation.

OBJECTIVE: The authors present their experience with patients having undergone orthotopic heart transplantation (OHT) in whom surgical conditions subsequently developed that required operative intervention. The incidence, morbidity, and mortality of these procedures are reported. SUMMARY BACKGROUND DATA: Several studies have evaluated the management options of biliary tract disease after OHT. Multiple reports of patients having undergone OHT who subsequently underwent peripheral vascular reconstructions, plastic reconstructive, and thoracic procedures also have been published. Applications of navigation for orthognathic surgery.

Stereotactic surgical navigation has been used in oral and maxillofacial surgery for orbital reconstruction, reduction of facial fractures, localization of foreign bodies, placement of implants, skull base surgery, tumor removal, temporomandibular joint surgery, and orthognathic surgery. The primary goals in adopting intraoperative navigation into these different surgeries were to define and localize operative anatomy, to localize implant position, and to orient the surgical wound. Navigation can optimize the functional and esthetic outcomes in patients with dentofacial deformities by identify

Body lift: an account of 200 consecutive cases in the massive weight loss patient.

BACKGROUND: The epidemic of obesity in the United States has led to a rapid increase in the number of bariatric procedures performed over the past several years. The dramatic changes to the torso following massive weight loss are only partially addressed by routine procedures such as abdominoplasty and liposuction. Circumferential body lifts or simultaneous abdominoplasty, thigh, and buttock lifts are becoming the method of choice for treating the postbariatric condition. In this article, the authors review the senior author's experience of 200 body lifts with massive weight loss individuals.

Effect of enhanced recovery after surgery on older patients undergoing transvaginal pelvic floor reconstruction surgery: a randomised controlled trial.

BACKGROUND: Prospective trial evidence is lacking regarding the application of enhanced recovery after surgery (ERAS) in transvaginal pelvic floor reconstruction surgery among older patients. Our study aimed to investigate whether implementing the ERAS protocol could enhance post-operative recovery in this patient population. METHODS: Older patients undergoing elective transvaginal pelvic floor reconstruction surgery were randomly assigned to either the ERAS group or the conventional group. The primary outcome was post-operative length of stay (LOS). The secondary outcomes encompassed other p
