Update on paravertebral blocks.

PURPOSE OF REVIEW: The goal of this review was to update the reader on the developments and advancements that have transpired in the previous few years and to encourage an open dialogue amongst readers and researchers alike. RECENT FINDINGS: Tremendous progress has been made investigating the part paravertebral blocks play, not only in acute pain management but also in management of nonsurgical pain. It starts with discussing the recent advances pertaining to paravertebral blocks (PVBs) in breast, thoracic and cardiac surgery and then leads on to its evolving presence in pediatric surgery. Th

The impact of parenteral narcotic choice in the development of acute chest syndrome in sickle cell disease.

BACKGROUND: Acute chest syndrome (ACS) represents a serious morbidity and often fatal complication in patients with sickle cell disease. Painful episodes which require hospitalization are most often treated with opioids, which may then influence the development of ACS. Nalbuphine is a parenteral opioid which effectively treats pain and may cause less ACS. PROCEDURE: This retrospective chart review documented 988 admissions for painful episodes at 2 institutions and recorded the incidence of ACS and opioid used. RESULTS: At the Children's Hospital in St Louis, Missouri, the incidence of ACS i

[Treatment of severe non-cancer pain by oral morphine].

Although the pain-relieving effect of morphine and other opioid analgesics for various conditions (cancer or non-cancer) are established, many physicians hesitate to prescribe them for the patients suffering from severe non-cancer pain. To develop full efficacy of morphine (and other opioids), following procedures are essential for the therapy; detailed explanation about the drugs, careful measures to prevent the adverse effects (nausea, vomiting, constipation, sleepiness, etc.) and prescribing "analgesic dosage" of morphine for the patient. We have been engaged in the control of severe non-ca

Meningioma manifested as temporomandibular joint disorder: a case report.

Pain in the temporomandibular joint (TMJ) and the surrounding region constitutes a symptom of TMJ disorders. Various dental causes usually stimulate the trigeminal nerve, developing facial pain which triggers trigeminal neuralgia. However, trigeminal neuralgia may also arise from irritation of the endocranial root of the nerve, due to occult damage which has not yet manifested other symptoms, for example a meningioma. In this manner, the actual cause of pain in the ipsilateral half of the face may be interpreted incorrectly and may possibly be attributed to a TMJ dysfunction syndrome. This res

[The patello-femoral joint in osteoarthritis of the knee with gene varum (author's transl)].

The authors have treated 63 knees with osteoarthritis associated with varus deformity by valgus osteotomies. The clinical and radiological state of the patello-femoral joints has been studied before and after correction of varus deformity. No correlation was found between the severity of tibio-femoral osteoarthritis and patello-femoral osteoarthritis. Medical patello-femoral discordance was found in 12 p. 100 of the cases. Clinical evidence of pain arising from the patello-femoral joint was more often found than evidence of radiological abnormality. It is concluded that pain arising from the m

Pass legislation to expand access to nonopioid pain treatments among older adults.

The opioid epidemic is an ongoing public health emergency, and rates of prescription opioid use, misuse, and opioid use disorder continue to increase in older adults (age ≥ 65 years). One of the major challenges to reducing prescription opioid use/misuse among older adults is that alternative pain management approaches are often not accessible. In 2022, the Non-Opioids Prevent Addiction in the Nation (NOPAIN) Act was passed by Congress and signed into law by President Biden. Beginning in 2025, the NOPAIN Act will expand access to nonopioid pain treatments by establishing separate reimbursement

Cancer-induced Pain Management by Nanotechnology-based Approach.

Cancer patients frequently report experiencing pain as one of their symptoms. Cancerrelated pain is often caused by the tumor itself, especially when the tumor is pressing on nerves. In addition to the pain caused by the tumor itself, patients also experience discomfort from the treatment, such as surgery, chemotherapy, radiation therapy, and the diagnostic procedures. The majority of today's pain therapies rely on opioid analgesics, which have not been shown to be effective. The adverse effects of opioids and their addictive properties call for the development of innovative treatment techniqu

Sinus tarsi denervation: clinical results.

Traumatic neuroma of the branches of the deep peroneal nerve that innervate the sinus tarsi can be the source of recalcitrant lateral ankle pain. That these nerves can be the source of the pain can be demonstrated by nerve blocks, and this pain can be surgically treated by resection of the appropriate branch of the deep peroneal nerve. This article documents the clinical results of this approach in 13 patients with sinus tarsi syndrome. At a minimum of 6 months postoperatively, 10 patients (77%) were completely pain-free, wore normal shoes, and had returned to work. Two patients (15%) had a sm

Ultrasound-Guided Platelet-Rich Plasma (PRP) Injection for Deep Medial Collateral Ligament (MCL) Injuries of the Knee.

BACKGROUND: Deep medial collateral ligament (dMCL) injuries are a known cause of medial knee pain, which can result from either trauma or degenerative conditions. While conservative management, including bracing and physical therapy, remains the first-line treatment, persistent symptoms may necessitate additional interventions. Platelet-rich plasma (PRP) therapy has emerged as a promising treatment modality due to its ability to enhance ligament healing by promoting collagen synthesis and tissue regeneration. Recent studies highlight PRP's potential to improve pain and function in patients wit

The Cutting Edge: Interventional Strategies for Pain Management.

When caring for spine patients, accurate diagnosis is essential, as interventional treatments are only effective when the true pain generator is correctly identified. Diagnostic procedures generally fall into provocative tests, which reproduce pain, and analgesic blocks, which relieve pain through anesthetic injections. However, both approaches are limited by false positives, particularly with uncontrolled medial branch or sacroiliac blocks. Controlled comparative blocks improve accuracy, but predictive value remains modest unless combined with history, examination findings, and imaging. This

Review of the evidence: best analgesic regimen for shoulder surgery.

Multimodal pain management has been advocated in patients experiencing acute pain after surgical procedures due to tissue damage and the subsequent inflammatory response. For patients undergoing shoulder surgeries, studies have definitively shown that interscalene blocks (ISBs) via single-injection or continuous infusion can reduce the total opioid consumption and can lower pain scores after surgery. In some cases, ISBs can be used as the sole anesthetic during shoulder surgeries and spare patients of receiving general anesthesia. However, clinicians should be fully aware of potential pulmonar

Arthroscopic resection of fat pad lesions and infrapatellar contractures.

The infrapatellar fat pad (IFP), also known as Hoffa's fat pad, may be a common site of pain in the knee because of its susceptibility to injury and its vast innervation and vascular supply. Patients who have trauma to the IFP may undergo a process of hemorrhage, inflammation, and fibrosis that may become painful. Patients with Hoffa's disease in whom conservative treatment with medications, physical therapy, and injections has failed may receive significant pain relief and benefit from undergoing arthroscopic subtotal removal of the IFP. We describe a safe and effective way to perform this pr

Internal dislocation of scapula following thoracotomy for lung transplantation - a case report.

INTRODUCTION: The diagnosis of chest pain at times can be challenging and requires a detailed history, thorough physical examination and investigations including imaging in post transplantation patients. This case is an example of a rare cause of pleuritic chest pain, which was initially misdiagnosed as a haemothorax. The correct diagnosis of dislocated scapula was delayed for three days resulting in considerable discomfort for the patient. CASE PRESENTATION: We present a case of dislocation of scapula following thoracotomy for single lung transplantation. This complication should be consider

Ultrasound-guided intercostal nerve cryoablation.

Ultrasound technology has advanced regional anesthesia and pain management, by improving accuracy and reducing complication rates. We have successfully performed cryoablation of intercostal nerves with ultrasound guidance with no complications. Four patients with postthoracotomy pain syndrome had pain relief for at least 1 mo after selective cryoablation of intercostal nerves at the mid-axillary line. Visualizing the pleura during the procedure is the greatest benefit of using ultrasonography, especially in thin patients whose intercostal groove to pleural distance may be <0.5 cm. Although fur

Postherpetic ophthalmic neuralgia.

Postherpetic ophthalmic neuralgia is the final stage of a varicella zoster infection. Many years after chickenpox infection, patients can develop herpes zoster in one or more specific dermatomal regions. The ophthalmic branch of the trigeminal nerve and the thoracic nerves are most commonly affected. Younger patients are less prone to postherpetic neuralgia than the older. Patients with a depression in cell-mediated immunity are more susceptible to develop postherpetic pain. Postherpetic ophthalmic neuralgia is a neuropathic pain and can be treated by anticonvulsants and tricyclic antidepressa
