Disease management of COPD with pulmonary rehabilitation.

Pulmonary rehabilitation is a set of tools and disciplines that attends to the multiple needs of the COPD patient. It extends beyond standard care by addressing the disabling features of chronic and progressive lung disease. It centers on self-management, exercise, functional training, psychosocial skills, and contributes to the optimization of medical management. Exercise enables other components by building strength, endurance, confidence, and reducing dyspnea. Patients who have undergone rehabilitation often enjoy a reduced need for health-care utilization. On the downside, rehabilitation i

[Pain--from the physiological and internal medicine viewpoint].

Physiology of pain is a manifold and very complex phenomenon that is far from being understood. It cannot be explained without reference to psychosocial conditions. Pain has the function of a warning system, but the system is far from perfect, because a number of chronic diseases (e.g., arterial hypertension or malignant neoplasms) begin slowly and nearly painless. The role of pain in internal medicine will be exemplified by thoracic and abdominal pain. With regard to diagnoses both types of pain represent ambiguous symptoms. Their anatomic and physiologic substrates often cannot be ascertaine

A multidisciplinary approach to chronic low-back pain in Western Australia.

Fifty patients with chronic low-back pain were subjected to extensive medical, psychiatric, and psychosocial assessment by a comprehensive Low Back Pain Clinic. Most patients were found to have easily identifiable environment factors which influenced the persistence of the symptomatology. All patients were significantly functionally impaired, and whilst neurologic findings were uncovered in only 12 patients, most had restricted movement of the lumbar spine. The initiating cause of the low-back pain was usually from a minor injury or no injury at all. Thirty-two patients were given psychiatric

Digital Discourse, Secondary Victimization, and Psychological Harm: Mixed-Methods Analysis of System Justification in the #MeToo Movement.

BACKGROUND: The #MeToo movement, initiated in 2006 and amplified on social media in 2017, mobilized women worldwide to share experiences of sexual harassment and assault online. While the movement increased awareness, it also revealed deep social divisions in digital spaces. Supportive discussions promoted solidarity and healing, whereas antagonistic responses reinforced backlash and secondary victimization. In India, the Indian Entertainment Industry (IEI) became a focal point where survivors' disclosures highlighted structural gender inequalities. These polarized reactions function as digita

Smell and taste function after completion of childhood cancer treatment: Follow-up of a prospective cohort study.

BACKGROUND & AIMS: Smell and taste changes are common and bothersome treatment symptoms during childhood cancer treatment, consequently affecting food intake and quality of life. Patients frequently ask whether and when smell and taste function recover after childhood cancer treatment. However, this question is difficult to answer as available evidence is sparse and limited to cross-sectional designs. The current study aims to describe the prevalence of (both objective and subjective) smell and taste dysfunction, and its relation with dietary intake and Health-related Quality of Life (HRQoL) w

Contemporary pharmacological strategies for acute peripheral facial palsy: a narrative review with clinical decision considerations.

Acute peripheral facial palsy (APFP) sometimes known as Bell's palsy is a common neurological condition and is marked by the acute onset of lower motor weakness on one side of the face. Whereas spontaneous recovery is widespread, there is a significant rate of incomplete recovery, synkinesis, or enduring functional and psychosocial disability of patients. The past decades witnessed the improvement of the diagnostic and therapeutic plans, particularly the pharmacological and adjunctive ones, due to the developments in pathophysiological and clinical trials and the creation of guidelines. This n

Scoping review of perceived injustice in mixed trauma and spinal cord injury/disorder samples.

PURPOSE/OBJECTIVE: Perceived injustice, the appraisal of injury-related losses as severe, permanent, and unfairly caused by others, is a significant predictor of poor outcomes in musculoskeletal and pain conditions. This scoping review investigates the landscape of perceived injustice research in mixed traumatic injury and spinal cord injury/disorder populations. RESEARCH METHOD/DESIGN: This scoping review followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews (PRISMA-ScR) reporting guidelines. We reviewed 41 studies published between 2009 and 2024, in

Let's Talk About Sex: A Systematic Review of What Childhood Cancer Patients and Survivors Really Want to Know About Sex.

BACKGROUND: Childhood cancer treatment often leads to persistent, complex psychosexual late effects, which impact survivors' sexual function, body image, and relational development. Despite these pervasive and serious consequences, sexual health communication remains consistently neglected across the cancer care continuum for childhood cancer patients and survivors. AIMS: This systematic review synthesies empirical evidence to determine childhood cancer patients and survivors' specific preferences for sexual health communication: What, how, when, where, and with whom do they want to discuss s

Neuropsychiatric Symptoms in Dementia: Prevalence, Impact, and Implications for Care and Carers - A Systematic Review.

BackgroundNeuropsychiatric symptoms (NPS), including agitation, depression, anxiety, psychosis, apathy, and sleep disturbances, are highly prevalent in dementia and significantly contribute to carer burden, accelerated cognitive decline, and increased healthcare utilisation. Despite their clinical relevance, NPS often remain under-recognised and sub-optimally managed. This review synthesises current evidence on the prevalence of NPS in dementia, their functional and psychosocial impacts, and pharmacological and non-pharmacological management approaches.MethodsA systematic search of PubMed, Emb

Fast and slow gene expression changes in blood following acute social stress.

Social stress is a risk factor for psychiatric disorders and also influences immune function. While it is known that acute social stress impacts the number of immune cells in circulation, the temporal dynamics of stress induced immune-related transcriptional changes in human blood remain unclear. To investigate changes in gene expression, we exposed 27 adults to the Trier Social Stress Test (TSST), and collected blood at baseline, as well as 5, 30, 60 and 90 min after stress. Whole-blood gene expression was profiled using a 5' targeted RNA-sequencing method (STRT). Differential expression was

The Relationship Between Menopause, Perimenopause and Alcohol Use: A Scoping Review.

BACKGROUND: The relationship between menopausal transition, alcohol use and women's health remains poorly understood despite its relevance in psychiatry and women's health. Alcohol and menopausal symptoms can overlap, which can lead to misattribution and a barrier to access appropriate care. The objectives are to map the existing literature on the relationship between menopause, perimenopause and alcohol use. We also aim to synthesise current evidence and identify knowledge gaps across 3 domains: alcohol and impact on menopausal physiology and symptoms; menopausal transition and impact on alco

Effectiveness of the interprofessional community-based rehabilitation versus standard care on quality of life in community-dwelling adults' post-stroke: systematic review protocol.

OBJECTIVE: This review aims to assess and synthesize evidence on the effectiveness of interprofessional, community-based rehabilitation (IP-CBR) compared with standard care and other non-specialized rehabilitation models in improving quality of life (QoL) among community-dwelling people living with stroke. INTRODUCTION: Stroke is a leading cause of long-term disability among older adults, resulting in functional limitations and reduced QoL. IP-CBR may improve outcomes compared to standard or non-specialized care. ELIGIBILITY CRITERIA: Randomized and quasi-experimental studies involving adult

Fear-Avoidance and Overactive Bladder: Toward a Mechanism-Informed Model of Care.

AIMS: Overactive bladder (OAB) is associated with substantial psychological burden, yet existing biopsychosocial frameworks acknowledge that psychological factors shape outcomes without explaining the cognitive and behavioral mechanisms through which that influence operates. This paper aimed to propose the fear-avoidance as a missing explanatory framework for OAB symptom maintenance, and to introduce a phenotyping model for clinical application. METHODS: We synthesized literature on fear-avoidance theory, cognitive appraisal and coping, and OAB epidemiology and psychosocial burden to construc

Correlation of spirituality with short-term outcomes after lumbar spine surgery: a prospective observational study.

PURPOSE: Spirituality and religiosity have repeatedly been discussed as potential psychosocial resources that may influence recovery in chronic or life-threatening illnesses. Evidence in surgical settings, particularly within neurosurgery, is scarce and inconsistent. This study investigated whether daily spiritual experiences - as measured by the Daily Spiritual Experience Scale (DSES) - are associated with postoperative outcomes in patients undergoing lumbar spine surgery. METHODS: In this prospective observational study, patients scheduled for lumbar decompression or fusion surgery complete

The Impact of Irritable Bowel Syndrome on Spine Surgery Outcomes: A Comprehensive Narrative Review.

Background/Objectives: Irritable bowel syndrome (IBS) is among the most prevalent disorders of gut-brain interaction, yet its implications for spine surgery remain poorly characterized. This narrative review examines how IBS influences symptom presentation and postoperative outcomes in spine surgery patients. Methods: We synthesized the neurobiologic, epidemiologic, and perioperative literature linking IBS with musculoskeletal pain, spine-related symptomatology, and surgical outcomes, drawing on spine-specific data where available and on related surgical and chronic-pain populations where it w

[Ipsilateral venous puncture and catheter placement after axillary lymphadenectomy].

After axillary lymphadenectomy (ALND) or radiotherapy for breast cancer or malignant melanoma, damage to the lymphatic tissue can reduce the lymphatic transport capacity and increase interstitial fluid accumulation. This can lead to swelling, pain, progressive trophic skin changes and an elevated risk of infections. Moreover, functional impairment of the affected limb can result in significant psychosocial consequences. Affected patients are usually informed about their increased lifetime risk of developing lymphedema and current guidelines from numerous international and national societies re

The interplay between recovery and immunity in infectious and non-communicable diseases.

BACKGROUND: The interplay between recovery and immune responses is increasingly recognized but remains under-integrated in clinical and research frameworks. Recovery extends beyond physiological repair to include restoration of function, psychological adaptation, and social reintegration, while immunity provides dynamic protection and regulation. Emerging evidence suggests that recovery trajectories shape immune competence and long-term resilience. SCOPE AND APPROACH: This review synthesizes evidence on bidirectional recovery-immunity interactions across infectious and non-communicable diseas
