Real-world treatment patterns and outcomes among patients with immune thrombocytopenia (ITP) who switched treatment from eltrombopag or romiplostim to avatrombopag in the United States: Results from the REAL-AVA 3.5 study.

Real-world data on treatment response after switching between thrombopoietin receptor agonists are limited for patients with immune thrombocytopenia (ITP). The objective of this study is to describe treatment patterns and outcomes in patients with primary ITP who switched from eltrombopag (ELT) or romiplostim (ROMI) to avatrombopag (AVA). We conducted a retrospective chart review study of adults with primary ITP who initiated ELT or ROMI on or after 1 July 2019, switched to AVA within 30 days of ELT/ROMI discontinuation and had ≥6 months follow-up after AVA initiation. Patients were followed f

Prenatal initiation of elexacaftor/tezacaftor/ivacaftor via carrier mother prevents congenital bilateral absence of vas deferens in a male infant with cystic fibrosis.

BACKGROUND: Prenatal therapy with elexacaftor/tezacaftor/ivacaftor (ETI) has recently emerged as a potential strategy to modify the early natural history of cystic fibrosis (CF). While case reports have described prevention of meconium ileus and pancreatic insufficiency, data on male reproductive outcomes remain extremely limited. CASE PRESENTATION: We report a male infant with CF (homozygous F508del) whose heterozygous carrier mother initiated ETI at 27+4 weeks of gestation after prenatal diagnosis. Pregnancy was uneventful until preterm delivery at 35+2 weeks. The neonate presented with nor

Elexacaftor/tezacaftor/ivacaftor is safe and effectively reduces sino-nasal symptoms in people with cystic fibrosis post lung transplantation.

BACKGROUND/AIM: This investigator-initiated, open-labelled study describes safety and extrapulmonary outcomes of elexacaftor/tezacaftor/ivacaftor (ETI) treatment in people with cystic fibrosis after lung transplantation (pwCF + LTX), with particular focus on sino-nasal symptoms. ETI has not been systematically administered to people with cystic fibrosis after lunge transplantation (pwCF + LTX) due to lack of data from this subgroup and concerns about potential interactions with immunosuppressive therapy. METHODS: All pwCF + LTX in Denmark were screened for eligibility and included participant

Pyrazolopyridine-Containing Compounds as Multitargeted Anti-Alzheimer Agents: Synthesis, Biological Evaluation, and In Silico Studies.

Multitargeted directed ligands (MTDLs) represent an appealing tactic to treat Alzheimer's disease (AD) as a multifactorial neurodegenerative disorder. In this study, the pyrazolopyridine scaffold was used to design three different series of integrated MTDLs that could inhibit the main causes of AD. They possess common structure features essential for such a goal. The planar pyrazolopyridine core allows binding with the catalytically active site of cholinesterase enzymes and intercalating between the residues of Aβ and tau protein. In addition, the planar ring is attached to different linkers a

Horse Spleen Apoferritin-Encapsulated Organoruthenium Complex Bearing Pyrazolyl-Pyridine Ligand for Cytotoxic and Antimicrobial Resistance Modulations.

Horse spleen apoferritin is a highly efficient nanocarrier for encapsulating and delivering metallodrugs, resulting in enhanced stability and therapeutic effectiveness. Herein, Horse spleen apoferritin nanocage(Aftn) is used to encapsulate the model organoruthenium complex bearing a pyrazolyl-pyridine ligand (Ru(bpy)) via a pH-induced disassembly/reassembly method to obtain Ru(bpy)-loaded nanoformulation (Ru-Aftn). This nanoformulation exhibits sustained release of the loaded complex over a period of 6 days with minimal burst release. Ru-Aftn also displays prominent cytotoxicity with better se

Impact of elexacaftor/tezacaftor/ivacaftor (ETI) on the prescription refill rate (PRR) for inhaled medications in people with CF: an Italian multicenter analysis.

This retrospective population-based study investigated the impact of elexacaftor/tezacaftor/ivacaftor (ETI) therapy on inhaled medication adherence in people with cystic fibrosis (pwCF). Prescription refill rate (PRR) for several inhaled medications were compared before and after ETI introduction in three major Italian CF centers. We found a significant decrease in PRR for most inhaled antibiotics and dornase-alpha after ETI implementation. This suggests that patients may be reducing their adherence to inhaled medications, potentially due to improved respiratory symptoms and quality of life. [Elexacaftor, Tezacaftor and Ivacaftor: immediate efficacy evaluation using home spirometry].

INTRODUCTION: Following two weeks of application of the triple combination therapy of Elexacaftor (E), Tezacaftor (T), and Ivacaftor (I) known as ETI, substantial pulmonary improvement in patients with cystic fibrosis is well-documented. However, few detailed data are available on the action of this treatment over the course of these first 14 days. METHODS: In this prospective study (NCT05599230), 20 patients aged≥12 years, all of them eligible for ETI, were recruited at the initiation of treatment. Home spirometry (MIR Spirobank®) was performed during the three days preceding the start of tr

Elexacaftor/tezacaftor/ivacaftor prescription in lung transplant recipients with cystic fibrosis in the US.

BACKGROUND: Elexacaftor/tezacaftor/ivacaftor (ETI) has dramatically improved pulmonary and extrapulmonary manifestations of cystic fibrosis (CF). ETI clinical trials excluded lung transplant (LTx) recipients and current post-transplant prescribing practices are not evidence-based. We sought to identify the prevalence of new ETI prescriptions among CF LTx recipients in the United States (U.S.) and factors associated with ETI prescription after LTx. METHODS: We performed a retrospective cross-sectional study of LTx recipients through December 2022 using the Cystic Fibrosis Foundation Patient Re

Development of pyrazolo[1,5-a]pyrimidine based macrocyclic kinase inhibitors targeting AAK1.

Since the outbreak of SARS-CoV-2 in recent years, our society has become more aware that zoonotic diseases pose a real threat. Therefore, the demand for small molecules that target host proteins, essential for viral entry and replication, has increased as an interesting strategy for the development of antiviral agents, as these agents may be effective against several different pathogens. NAK kinases is one such potential target family because they are involved in a variety of cellular functions, hijacked by viruses to invade host cells, such as clathrin-mediated endocytosis. A large number of

Blood platelet reduction after elexacaftor/tezacaftor/ivacaftor treatment in people with cystic fibrosis may depend on systemic inflammation reduction.

The elexacaftor/tezacaftor/ivacaftor (ETI) combination for cystic fibrosis transmembrane regulator modulators is a safe and effective treatment in both adults and children who are homozygous or compound heterozygous for the F508del variant. However, few cases involving a significant reduction in blood platelets and an increase in the alanine aminotransferase/platelet ratio have been described in adult and pediatric patients receiving ETI therapy. In the present study, we describe 272 people with cystic fibrosis (pwCF) (166 adult and 106 pediatric pwCF) who were independently followed at two ce

Transcriptomic and functional responses of the cystic fibrosis airway epithelium to CFTR modulator therapy.

Elexacaftor/tezacaftor/ivacaftor (ETI) cystic fibrosis transmembrane conductance regulator (CFTR) modulator therapy has led to rapid and substantial improvements in cystic fibrosis (CF) airway disease. Underlying molecular and cellular mechanisms, long-term efficacy, and ability to reverse airway epithelial remodeling in established disease remain unclear. Longitudinal nasal brushes from an adult CF cohort were used to evaluate gene expression, cellular composition, stem cell function, and microbiome changes at baseline and at 6 months and 2 years after ETI. The baseline to 6 month span showed

Patient-Reported Tolerability of Selpercatinib Compared to Cabozantinib/Vandetanib: A Secondary Analysis of the LIBRETTO-531 Randomized-Controlled Trial in RET-Mutant Medullary Thyroid Cancer.

Background: Progression-free survival (PFS) may not fully capture the impact of treatment on patients, especially in cancers with longer natural histories and thus, could be complemented by robust measures of patient-reported tolerability (PRT). We report the use of a novel, quantifiable PRT metric as a multiplicity-controlled endpoint to support regulatory and clinical decision-making for selpercatinib use. Comparative PRT was assessed in LIBRETTO-531 (NCT04211337), a randomized phase 3 trial of selpercatinib versus vandetanib/cabozantinib (control) in advanced RET-mutant medullary thyroid ca

Tumor-intrinsic interferon signaling drives pancreatic cancer resistance to tumor mucin1-targeted CAR T cell therapy.

Pancreatic cancer (PC) remains one of the most challenging cancers and has the worst prognosis. Tumor-associated MUC1 (tMUC1) is overexpressed and aberrantly glycosylated in over 80% of human pancreatic ductal adenocarcinoma (PDA). Chimeric antigen receptor (CAR) engineered T cells are an emerging cancer immunotherapy strategy and recently, we successfully engineered tMUC1-specific human and mouse CAR T cells and demonstrated their effectiveness as monotherapy against PDA in vitro and in vivo. In this study, we observed varying sensitivity among human PDA cell lines in response to tMUC1-target

People with cystic fibrosis with high sputum neutrophil elastase on elexacaftor-tezacaftor-ivacaftor exhibit worse pulmonary function and pro-inflammatory airway milieu.

Cystic fibrosis (CF) is a genetic condition affecting over 100,000 individuals worldwide. Lung disease is the main cause of mortality in CF, with chronic neutrophilic inflammation as a hallmark. Elexacaftor/tezacaftor/ivacaftor (ETI) is a combination of highly effective CF transmembrane conductance regulator (CFTR) modulators, conferring significant pulmonary gains to treated patients. However, the extent to which ETI controls inflammation is uncertain. To investigate this, we quantified effector proteins, cytokines, and metabolites in sputum fluid supernatant from ETI-treated (n = 32) and tre

Population Pharmacokinetics of Elexacaftor, Tezacaftor and Ivacaftor in a Real-World Cohort of Adults with Cystic Fibrosis.

BACKGROUND AND OBJECTIVES: Elexacaftor-tezacaftor-ivacaftor (ETI), a combination of cystic fibrosis transmembrane conductance regulator (CFTR) modulators, has become the therapeutic standard of care for most people with cystic fibrosis (pwCF). People with cystic fibrosis exhibit differences in CFTR genotypes and have important differences in phenotypic characteristics including age, body weight, pancreatic status, disease severity, and comorbidities. While these differences predict large interindividual variability (IIV) in ETI exposure, there is a unique dose regimen recommended for adults. T

Combining immunosuppressive therapy with low dosage eltrombopag in Chinese patients with severe aplastic anemia: mild aggravation of hepatic injury.

Eltrombopag (EPAG) is an oral thrombopoietin receptor agonist analog with the potential risk to induce liver injury. This prospective registry study evaluated the prevalence and severity of hepatic injury in Chinese patients with severe aplastic anemia undergoing low-dose EPAG treatment (75 mg/day) in the context of standard immunosuppressive therapy (IST). The incidence of acute drug-induced liver injury was slightly higher in the IST + EPAG group than in the IST group at the 1st and 2nd month, but no statistically significant difference was observed: 10% vs 5% (p = 0.400), 9% vs 8% (p = 1.00
