Sleep apnea.

Progress continues in the refinement of historical and physical examination findings predictive of sleep-disordered breathing. Home monitoring is becoming more widely accepted and validated. The most significant development in the diagnosis of sleep-disordered breathing this year was the American Academy of Sleep Medicine's report on "Recommendations for syndrome definition and measurement techniques in clinical research." In this report, hypopnea and respiratory effort related arousal are defined, and the term obstructive sleep apnea is appropriately changed to obstructive sleep apnea-hypopne

Impact of brainstem sleep mechanisms on pharyngeal motor control.

Suppression of respiratory muscle activity in sleep, particularly evident in the pharyngeal muscles, is pivotal to the pathogenesis of common sleep-related breathing disorders such as obstructive sleep apnea. Obstructive apneas are caused by sleep-related decrements in pharyngeal muscle activity that leads to snoring and airway obstruction in individuals with underlying structural narrowing of the upper airway. Since obstructive apneas occur exclusively during sleep, this disorder by definition is state-dependent and ultimately caused by the influences of brainstem sleep mechanisms on pharynge

Sleep Apnea in Hurler Syndrome: Looking Beyond the Upper Airway.

This case involves a 13-month-old male with Hurler syndrome. Due to oxygen desaturations during sleep, this patient was referred for polysomnography, which revealed severe mixed sleep apnea (apnea-hypopnea index [AHI] 72 events/h). Because sleep apnea in patients with Hurler syndrome is frequently attributed to upper airway obstruction, he was referred to otolaryngology. Prior to his evaluation by otolaryngology, he underwent ventriculoperitoneal (VP) shunt placement, which had been scheduled due to hydrocephalus on brain magnetic resonance imaging (MRI). After VP shunt placement, his oxygen d

Central Sleep Apnea.

OBJECTIVE: Understanding respiratory control during sleep is fundamental to understanding the approach to the diagnosis and management of central sleep apnea (CSA). LATEST DEVELOPMENTS: CSA is not one disease but rather a syndrome resulting from a heterogeneous group of etiologies. There are complex chemical, metabolic, mechanical, and neural contributions that can cause dysregulation in loop gain, decreased central respiratory drive, or both. The traditional dichotomy between obstructive versus central sleep apnea is overly simplistic. Conventional CSA is the extreme end along a continuum be

[Normal tension glaucoma, sleep apnea syndrome and nasal continuous positive airway pressure therapy--case report with a review of literature].

BACKGROUND: In the pathogenesis of glaucoma, besides an elevated intraocular pressure (IOP), cardiovascular risk factors, such as arterial hypotension and hypertension, vasospasms, autoregulatory defects, atherosclerosis, and diabetes mellitus are of increasing importance, especially in normal tension glaucoma. Recently, there have been several reports of an additional risk factor: obstructive sleep apnea syndrome. METHODS: Literature review (Medline) and case report. RESULTS: The authors report on a 8 1/2 years follow-up of a 60-year-old patient with normal tension glaucoma. Despite success

Effectiveness of short-term treatment with nocturnal oxygen therapy for central sleep apnea in patients with congestive heart failure.

OBJECTIVES: To evaluate the short term effects of inhalation of oxygen at night in 51 patients with congestive heart failure (CHF) and sleep apnea syndrome (SAS). METHODS: Fifty-one patients with stable CHF (31 males, 20 females, mean age 79.0 +/- 11.9 years; brain natriuretic peptide level of > 100 pg/ml) were evaluated between September 2003 and August 2004, using a Morpheus monitor. The complication rate of SAS in patients with CHF was assessed and apnea hypopnea index, oxygen desaturation index 3%, heart rate, and autonomic nerve activity under room air compared to supplemental O2 (2 l/mi

Alpha-1 antitrypsin deficiency and risk of sleep apnea: a nationwide cohort study.

OBJECTIVES: α1-Antitrypsin deficiency is a disease characterized by increased neutrophil elastase activity leading to tissue getting less elastic and robust. It is known that if tissue in the pharynx becomes less elastic and robust, it could contribute to obstructive sleep apnea. This paper seeks to investigate whether patients with α1-antitrypsin deficiency have an increased risk of sleep apnea. METHODS: We tested this hypothesis by doing a nationwide cohort study of 2702 individuals diagnosed with α1-antitrypsin deficiency compared with 26,750 individuals without α1-antitrypsin deficiency m

Radiofrequency catheter ablation of persistent atrial fibrillation decreases a sleep-disordered breathing parameter during a short follow-up period.

BACKGROUND: Obstructive sleep apnea (OSA) is often associated with atrial fibrillation (AF), but the impact of radiofrequency catheter ablation (RFCA) for AF on sleep apnea syndrome is unknown. METHODS AND RESULTS: A total of 25 patients (3 women; 61 ± 6 years) with sleep apnea syndrome who underwent RFCA for drug-refractory, persistent AF were studied. Polysomnography was also performed 1 day before and 1 week after RFCA in all patients. The total number of central or OSA or hypopnea events was analyzed and compared. Among the 25 patients who all predominantly had obstructive apnea, the apne

Sleep in postpolio syndrome.

Post-polio patients may develop additional neuromuscular and respiratory symptoms decades after the acute attack, the post-polio syndrome. We hypothesize some post-polio symptoms may be due to breathing disorders occurring during sleep. We performed polysomnography on 13 post-polio patients: group 1 (five patients) were those already on ventilatory assistance (rocking beds) and group 2 (eight patients), those without any assistance. Patients requiring new treatment were then evaluated on nasal CPAP or nasal mask ventilation. Group 1 patients, on rocking beds, demonstrated consistently poor sle

Impact of sleep-disordered breathing in patients with acute myocardial infarction: a retrospective analysis.

Sleep-disordered breathing (SDB) is associated with an increased risk of cardiovascular events. Previous studies showed that severe SDB has a negative impact on myocardial salvage and progression of left ventricular dysfunction after acute myocardial infarction (AMI). This study investigated the frequency of SDB and the effects of SDB on left ventricular function after AMI. This retrospective study enrolled all patients with AMI who had undergone cardiorespiratory polygraphy for SDB diagnosis. The apnea-hypopnea index was used as a standard metric of SDB severity. SDB was classified as mild (a

[Physiopathology of sleep-disordered breathing in children].

Sleep-disordered breathing represents a broad spectrum of alterations, ranging from habitual snoring, through obstructive hypoventilation to obstructive sleep apnea-hypopnea syndrome, and produces breathing and architecture anomalies during sleep. There are some controversies on the physiopathological mechanisms of sleep-disordered breathing and its metabolic, neurocognitive and cardiovascular effects. The causes of comorbidity in obstructive sleep apnea-hypopnea syndrome in children are not yet well known and can include blood gasometric anomalies, fragmented sleep, and local and systemic inf

Sleep apnoea detection in children using PPG envelope-based dynamic features.

Photopletysmography signal has been developed for monitoring of Obstructive Sleep Apnoea, in particular, whenever an apneic episode occurs, that is reflected by decreases in the photopletysmography signal amplitude fluctuation. However, other physiological events such as artifacts and deep inspiratory gasp produce sympathetic activation, being unrelated to apnea. Thus, its high sensitivity can produce misdetections and overestimate apneic episodes. In this regard, a methodology for selecting a set of relevant non-stationary features to increase the specificity of the obstructive sleep apnea de

Sleep of the great.

Both Lewis Carroll and William Shakespeare appear to have made clinical observations of sleep apnea syndromes long before they were discovered by medical science, and to have understood something about their physiological mechanisms. The somnolent dormouse in Alice in Wonderland indicates that his problem is one of sleep and breathing and is subject to modern treatment for obstructive apnea. Shakespeare in Henry IV presents a case of obstructive apnea along with a case of Cheyne-Stokes breathing and uses the plot of these history plays to explain by analogy the theoretical basis for periodic b

Endoscopic findings in sleep apnea associated with acromegaly.

Two acromegalic patients presented with the additional complaint of excessive daytime sleepiness. Subsequent sleep laboratory evaluation revealed that each patient had concurrent obstructive sleep apnea. In each patient, endoscopic examination of the upper airway revealed that on inspiration, the soft tissue of the posterior and lateral hypopharynx invaginated into the laryngeal vestibule before any posterior movement of the tongue. Neither enlargement of the tongue nor in movement in relation to respiration appeared to be the primary factors in the etiology of the upper airway obstruction. Af

The effects of ondansetron on sleep-disordered breathing in the English bulldog.

Serotonin and serotoninergic drugs have significant effects on respiration, at many sites throughout the nervous system, and serotonin has been implicated in the pathogenesis of obstructive sleep apnea. Thus, understanding the serotoninergic mechanisms underlying respiratory control may help discover novel pharmacotherapies for sleep-disordered breathing. Ondansetron, a serotonin (5-HT) antagonist selective for the 5-HT3 receptor subtype has recently been shown to suppress sleep-related central apneas in rats, particularly in rapid-eye-movement (REM) sleep. To evaluate the potential of ondanse

Alleviation of central sleep apnea by ventricular pacing in a patient with an implanted cardioverter defibrillator.

A 75-year-old man with a single chamber cardioverter defibrillator implanted for the management of sustained ventricular tachycardia developing after a healed myocardial infarction was admitted for evaluation of a sleep disorder. Polysomnography confirmed the presence of severe predominant central sleep apnea syndrome. The apnea-hypopnea index score decreased from 43.3 during spontaneous cardiac rhythm to 24.6 during VVI pacing at a rate of 70 beats/min. Ventricular pacing reduced the number of episodes of central sleep apnea/hypopnea without reducing the total sleep time, though it had no eff

Effects of sleep deprivation on respiratory events during sleep in healthy infants.

This study was designed to determine the effects of sleep deprivation on respiratory events during sleep in healthy infants. Ten unsedated full-term infants (1-6 mo) were monitored polygraphically during "afternoon naps" on a control day and on the day after sleep deprivation. Respiratory events, i.e., central apnea, obstructive apnea and hypopnea, and periodic breathing were tabulated. Results for respiratory events were expressed as 1) indexes of the total number of respiratory events and of specific respiratory events per hour of total sleep (TST), "quiet" sleep (QS) and "active" sleep (AS)
