Pitfalls in shoulder MRI: part 2--biceps tendon, bursae and cysts, incidental and postsurgical findings, and artifacts.

OBJECTIVE: The purpose of this article is to review frequently encountered pitfalls as they pertain to the biceps tendon, bursae and cysts around the shoulder, incidental findings, postsurgical findings, and frequently encountered imaging artifacts. CONCLUSION: Imaging pitfalls in and around the shoulder are not limited to normal anatomy and anatomic variants. Radiologists must be cognizant of the vast variability of structures in the shoulder and of the incidental and postsurgical findings and artifacts affecting them.

Acute achilles tendon ruptures: a comparison of minimally invasive and open approach repairs followed by early rehabilitation.

We retrospectively compared the outcomes of early functional weight-bearing after use of 2 different approaches (minimally invasive, standard) for surgical repair of the Achilles tendon. We reviewed the cases of 63 consecutive patients who underwent repair of an acute closed Achilles tendon rupture and had follow-up of at least 6 months. Of these 63 patients, 33 were treated with a minimally invasive posterolateral approach (minimal group), and 30 were treated with a standard posteromedial approach (standard group). Two weeks after surgery, each patient was allowed to weight-bear as tolerated

Anatomic double-bundle reconstruction with free tendon graft for chronic ulnar instability of the thumb metacarpophalangeal joint.

Failed primary treatment of thumb metacarpophalangeal ligament injuries may lead to chronic instability. Different treatments have been described such as secondary ligament suture, fusion, dynamic or static procedures. Techniques to restrain the joint stability with various grafts of different configurations have been published, but most of these address only the proper collateral ligament. Experimental data revealed the importance of the accessory collateral ligament in stabilizing the metacarpophalangeal joint. In a former, true anatomic ligament reconstruction a bone-tendon graft was used;

Unstable osteochondral fragment of the lateral femoral condyle containing the popliteus tendon origin. Report of three cases.

We report a series of three cases of unusual locations of chronic osteochondral fragments arising from the posterolateral aspect of the lateral femoral condyle, presenting acutely in young adults, in which the unstable fragment contained the origin of the popliteus tendon, with MRI and arthroscopic correlation. Although atypical, we hypothesize that these cases represent adult osteochondritis dissecans with extension to a popliteus tendon origin. Preoperative diagnosis of popliteus tendon involvement may influence clinical management in patients with an unstable osteochondral fragment in this

Acute double flexor tendon ruptures following injection of collagenase clostridium histolyticum (Xiapex) for Dupuytren's contracture.

We report a case of acute (24 h) double flexor tendon rupture of the little finger after a single injection of collagenase clostridium histo … r, but our test on an awake patient showed that when a 27-gauge needle was inserted into the flexor tendons through a thick palmar cord, the syringe did not move significantly when the patient moved the finger, and therefore this test does not minimise the risk of iatrogenic tendon injury when using collagenase (Xiapex) for Dupuytren's contracture.

Flexor hallucis longus tendon transfer in the reconstruction of extensive insertional Achilles tendinopathy in elderly: an improved technique.

BACKGROUND: Insertional Achilles tendinopathy is a degenerative disease associated with disabling posterior heel pain, gait dysfunction and significant morbidity. The aim of this prospective study was to evaluate the outcomes of complete excision of the pathological tendo-achilles segment in elderly patients with extensive involvement, and reconstructing the defect using a modified technique which was proposed to allow early weight-bearing and rehabilitation. PATIENTS AND METHODS: Thirteen patients (mean age 58.2 years) with extensive insertional Achilles tendinopathy (seven with spontaneous

Modification of side-locking loop suture technique using an antislip knot for repair of Achilles tendon rupture.

The 2-strand side-locking loop suture technique provides high tensile strength and stiffness immediately after surgery, and good clinical results have been reported in the treatment of Achilles tendon rupture. However, it is assumed that major differences exist among surgeons with regard to the optimal tension of the side-locking loop suture. We report a detailed technique to ensure application of a standard tension with the use of the side-locking loop suture in the clinical setting.

Simultaneous surgical repair of a tibialis anterior tendon rupture and diabetic charcot neuroarthropathy of the midfoot: a case report.

The combination of simultaneous rupture of a tibialis anterior tendon and Charcot neuroarthropathy of the midfoot in a diabetic patient is a rare and challenging condition that can lead to major complications if not addressed appropriately. This article discusses a tibialis anterior tendon rupture that may have developed before or after the incidence of the diabetic Charcot neuroarthropathy midfoot deformity and raises awareness to potential spontaneous tendon ruptures that may be associated with the diabetic Charcot foot.

Reconstruction using an autograft with near complete preservation of the extensor retinaculum for chronic tibialis anterior tendon disruption.

We present the case of a 42-year-old woman with chronic disruption of the tibialis anterior tendon who was treated by reconstructive surgery using autologous semitendinosus tendon grafting. Two incisional approaches, proximal and distal of the degenerative scar tissue between each ruptured stump, made it possible to preserve most of the extensor retinaculum, except for the inferior Y-shaped medial segment that was excised to prevent adhesion between the reconstructed tendon and the extensor retinaculum. Furthermore, by placing the proximal and distal suture knots far from the ends of the exten

Greater glycosaminoglycan content in human patellar tendon biopsies is associated with more pain and a lower VISA score.

BACKGROUND: People with patellar tendinopathy experience chronic pain and activity limitation, but a pertinent biochemical marker correlated … s controls with no history of knee pain (n=4). The quantity of sulphated GAGs in the human patellar tendons was determined with a dimethyl methylene blue (DMMB) assay; this method was first validated with rat tendon tissue. The extent and distribution of GAG species and proteoglycans (decorin, versican and aggrecan) in the human tendon biopsies were exami …

Clinical spectrum of early onset cerebellar ataxia with retained tendon reflexes: an autosomal recessive ataxia not to be missed.

Autosomal recessive cerebellar ataxias are a heterogeneous group of neurological disorders. In 1981, a neurological entity comprised by early onset progressive cerebellar ataxia, dysarthria, pyramidal weakness of the limbs and retained or increased upper limb reflexes and knee jerks was described. This disorder is known as early onset cerebellar ataxia with retained tendon reflexes. In this article, we aimed to call attention for the diagnosis of early onset cerebellar ataxia with retained tendon reflexes as the second most common cause of autosomal recessive cerebellar ataxias, after Friedrei

Incidence of talar dome lesions with concomitant peroneal tendon pathologic features: a magnetic resonance imaging evaluation.

Few studies have evaluated the incidence of talar dome lesions and injuries to the peroneal tendons occurring concomitantly. The purpose of our research was to evaluate the incidence of osteochondral lesions of the talus (OLT) with peroneal tendon pathologic features according to the magnetic resonance imaging (MRI) findings. A database search was conducted in the Department of Radiology at the Western Pennsylvania Hospital and Forbes Regional Campus for all MRI examinations ordered by attending physicians of the Department of Foot and Ankle Surgery from 2008 to 2010. A total of 810 MRI report…
