Quantitative objective markers for upper and lower motor neuron dysfunction in ALS.

OBJECTIVE: To investigate the value of objective biomarkers for upper (UMN) and lower (LMN) motor neuron involvement in ALS. METHODS: We prospectively studied 64 patients with ALS and its subsets using clinical measures, proton MR spectroscopic imaging ((1)H MRSI), diffusion tensor imaging, transcranial magnetic stimulation, and the motor unit number estimation (MUNE) at baseline and every 3 months for 15 months and compared them with control subjects. RESULTS: (1)H MRSI measures of the primary motor cortex N-acetyl-aspartate (NAA) concentration were markedly reduced in ALS (p = 0.009) and a…

Inhibitory circuits and the nature of their interactions in the human motor cortex a pharmacological TMS study.

Inhibitory circuits are crucial in modulating corticospinal output in the primary motor cortex (M1). Relatively little is known about how these inhibitory circuits interact. Here we measured three forms of inhibition in M1 by paired-pulse transcranial magnetic stimulation: short-interval intracortical inhibition (SICI), long-interval intracortical inhibition (LICI) and short-interval interhemispheric inhibition (SIHI). We specifically tested their interactions under pharmacological challenge with a single oral dose of diazepam, a positive allosteric modulator of the gamma-aminobutyric acid typ…

Neuroactive steroids as modulators of depression and anxiety.

Certain neuroactive steroids modulate ligand-gated ion channels via non-genomic mechanisms. Especially 3alpha-reduced pregnane steroids are … s on neuroactive steroid concentrations during major depression. Neither partial sleep deprivation, transcranial magnetic stimulation nor electroconvulsive therapy affected neuroactive steroid levels irrespectively of the response to these treatments. These studies suggest that the changes in neuroactive steroids observed after antidepressant pharmacotherapy more likely reflect di …

Organization of action sequences and the role of the pre-SMA.

To understand the contribution of the human presupplementary motor area (pre-SMA) in sequential motor behavior, we performed a series of finger key-press experiments. Experiment 1 revealed that each subject had a spontaneous tendency to organize or "chunk" a long sequence into shorter components. We hypothesized that the pre-SMA might have a special role in initiating each chunk but not at other points during the sequence. Experiment 2 therefore examined the effect of 0.5-s, 10-Hz repetitive transcranial magnetic stimulation (rTMS) directed over the pre-SMA. As hypothesized, performance was di…

Central motor reorganization in cerebral palsy patients with bilateral cerebral lesions.

Transcranial magnetic stimulation (TMS) has been used to describe cortical plasticity after unilateral cerebral lesions. The objective of this study was to find out whether cortical plasticity occurs after bilateral cerebral lesions. We investigated central motor reorganization for the arm and leg muscles in cerebral palsy (CP) patients with bilateral cerebral lesions using TMS. Seventeen patients (12 with spastic diplegia, 1 with spastic hemiplegia, and 4 with athetoid CP) and 10 normal subjects, were studied. On CT/MRI, bilateral periventricular leukomalacia was observed in all spastic patie…

iTBS Stimulation of the Bilateral IFG/IPL Alters the Oscillatory Pattern in ASD.

BACKGROUND: Autism Spectrum Disorder (ASD) is a neurodevelopmental condition characterized by impairments in social communication, reciproci … s of imitation, empathy, and social cognition in ASD. Non-invasive neuromodulation using repetitive transcranial magnetic stimulation (rTMS) has shown modest behavioral benefits in ASD. However, most studies apply the conventional protocols targeting the dorsolateral prefrontal cortex. The effects of intermittent theta-burst stimulation (iTBS), a potent excitatory rTMS protocol ta …

Motor cortical hyperexcitability in idiopathic scoliosis: could focal dystonia be a subclinical etiological factor?

The aetiology of idiopathic scoliosis (IS) remains unknown; however, there is a growing body of evidence suggesting that the spine deformity … ay shed some insight into the dystonia hypothesis regarding the pathophysiology of IS. Paired pulse transcranial magnetic stimulation was used to study cortico-cortical inhibition and facilitation in nine adolescents with IS, five teenagers with congenital scoliosis (CS) and eight healthy age-matched controls. Neural network dysfunction in bipolar depression: clues from the efficacy of lamotrigine.

One strategy to understand bipolar disorder is to study the mechanism of action of mood-stabilizing drugs, such as valproic acid and lithium … t lamotrigine had a facilitatory effect on the BOLD (blood-oxygen-level-dependent) response to TMS (transcranial magnetic stimulation) of the prefrontal cortex. This effect was in contrast with an inhibitory effect of lamotrigine when TMS was applied over the motor cortex. In a follow-up study, a similar prefrontal specific facilitatory effect was observed in a larger cohort of he …

Self-Controlled Feedback on Motor Learning and Neuroplastic Changes in People With Parkinson Disease.

BACKGROUND AND PURPOSE: People with Parkinson disease (PD) experience impaired motor learning. Although numerous studies have shown that self-controlled feedback can facilitate motor learning in healthy young adults, whether similar benefits can be observed in people with PD has been less investigated. This study thus aimed to examine whether self-controlled feedback could benefit motor learning for individuals with PD, and to determine the associated behavioral and neurophysiological mechanisms. METHODS: Thirty-two participants with PD, matched in pairs, were recruited into a self-control gr

Operant Conditioning of the Motor Evoked Potential in Forearm Extensors of Individuals with Chronic Incomplete Cervical Spinal Cord Injury: Case Studies.

BACKGROUND: Operant up-conditioning of the motor evoked potential (MEP) elicited by transcranial magnetic stimulation can increase corticospinal excitability for the ankle dorsiflexor and improve its activation in individuals with foot drop due to chronic incomplete spinal cord injury (SCI). It is unknown whether MEP up-conditioning can work similarly in the upper limb (UL). OBJECTIVES: As the first step in testing the possibility of MEP up-conditioning enhancing UL neurorehabilitation, we examined the effects of forearm extensor MEP up-conditioning in 6 adults with chronic incomplete cervica…

Very low-amplitude muscle activity increases probability of motor evoked potentials in healthy individuals and in amyotrophic lateral sclerosis.

Muscle contraction increases motor evoked potential (MEP) amplitude, decreasing motor threshold (MT). Correspondingly, trials where baseline EMG amplitude exceeds a specified threshold are often rejected. We aimed to investigate the influence of motor activity below such a threshold of MEP amplitude. We retrospectively analysed TMS-EMG data collected during resting MT (RMT) measurement in 45 healthy control subjects (1794 data points) and 35 people with amyotrophic lateral sclerosis (ALS; 1229 data points). Trials with de-meaned root mean squared (RMS) EMG amplitude of >10 µV throughout the 20

Electric field intensity and EEG microstate dynamics in obsessive-compulsive disorder: a secondary analysis of a randomized sham-controlled HD-tDCS trial.

BACKGROUND: Obsessive-compulsive disorder (OCD) affects 2-3% of the population and is often accompanied by sleep disturbances, which can aggravate symptoms and hinder treatment. High-definition transcranial direct current stimulation (HD-tDCS) shows promise for alleviating both OCD symptoms and sleep-related issues, though its mechanisms remain unclear. This study aimed to explore the role of electric field (EF) intensity and EEG microstates in HD-tDCS outcomes. METHODS: This study is a secondary analysis based on a previously published clinical trial. Forty-four drug-naïve OCD patients parti
