Intramedullary Lesion Length on Postoperative Magnetic Resonance Imaging is a Strong Predictor of ASIA Impairment Scale Grade Conversion Following Decompressive Surgery in Cervical Spinal Cord Injury.

BACKGROUND: Evidence indicates that, over time, patients with spinal cord injury (SCI) improve neurologically in various degrees. We sought to further investigate indicators of grade conversion in cervical SCI. OBJECTIVE: To detect predictors of ASIA impairment scale (AIS) grade conversion in SCI following surgical decompression. METHODS: In a retrospective study, demographics, clinical, imaging, and surgical data from 100 consecutive patients were assessed for predictors of AIS grade conversion. RESULTS: American Spinal Injury Association motor score was 17.1. AIS grade was A in 52%, B in

Increased spinal cord movements in cervical spondylotic myelopathy.

BACKGROUND CONTEXT: Magnetic resonance imaging (MRI) is a very useful diagnostic test for cervical spondylotic myelopathy (CSM) because it can identify degenerative changes within the spinal cord (SC), disclose the extent, localization, and the kind of SC compression, and help rule out other SC disorders. However, the relationships between changes in cerebrospinal fluid (CSF) flow, cord motion, the extent and severity of spinal canal stenosis, and the development of CSM symptoms are not well understood. PURPOSE: To evaluate if changes in the velocity of CSF and SC movements provide additional

Radiographic analysis of the anterior to posterior open mouth (APOM) cervical spine view: frequency of atlas transverse process overlap of the inferior tip of the mastoid process.

OBJECTIVE: The purpose of this study was to measure the frequency with which the atlas transverse process is overlapped by the inferior tip of the mastoid process based upon radiographic analysis of the anterior to posterior open mouth (APOM) cervical spine view. METHODS: This is a retrospective study. Anterior to posterior open mouth radiographs (N = 120) were obtained from patient files at a chiropractic clinic. Dimensions were bilaterally measured: the vertical distance from the inferior mastoid to the superior margin of the C1 transverse process (C1TP) and the vertical distance from the i

Management of acute combination atlas-axis fractures with percutaneous triple anterior screw fixation in elderly patients.

INTRODUCTION: Patients with combined C1-2 fractures were often treated by posterior arthrodesis. However, elderly patients with multiple injuries (such as brain injury), the large surgical trauma of posterior arthrodesis will increase the risk of perioperative mortality. A minimally invasive technique may be better for them, and decrease the risk of perioperative mortality. MATERIALS AND METHODS: Seven patients with combined C1-2 fractures underwent percutaneous anterior odontoid screw and anterior C1-2 transarticular screws (percutaneous triple anterior screws fixation). The surgical techniq

Developmental anatomy of the atlas and axis in childhood by computed tomography.

OBJECT: The CT modality plays a central role in the diagnosis of cervical spine fractures. In childhood, radiolucent synchondroses between ossification centers can resemble fractures, and they can be the sites of fractures as well. Recognition of cervical spine fractures in children requires familiarity with normal developmental anatomy and common variants as they appear on CT scans. METHODS: A convenience sample of 932 CT scans of the cervical spine accessible on the picture archiving and communications system (known as PACS) at a single children's hospital was examined. Scans were excluded

[Rare cause of acute neck pain].

The acute calcific tendinitis of the longus colli muscle is a rare inflammatory process of the prevertebral muscles. The clinical picture includes acute neck pain, limited range of motion of the cervical spine with stiffness and odynophagia. The laboratory findings demonstrate inflammatory signs. The incidence of the disease peaks between the third and sixth decade. The knowledge of the characteristic radiologic findings and the self-limiting course prevents the patient from needless medical and surgical interventions.

Unilateral C-1 lateral mass sagittal split fracture: an unstable Jefferson fracture variant.

OBJECT: The object of this study was to describe an unusual fracture subtype within C-1 injuries with a propensity to result in late deformity and pain. Most patients with C-1 injuries are nonsurgically treated using external immobilization unless there is an injury of the transverse atlantal ligament. The authors describe an unusual variant involving a unilateral sagittal split with a high tendency to late deformity and pain. They also review the literature and treatment of C-1 fractures. METHODS: A retrospective review of 12,671 CT scans from a Level I trauma center over a 6-year period yie

[Colliculus atlantis--a rarely observed anatomic structure--in a transoral roentgen image of the thoracic spine].

PURPOSE: To study the time and mode of the development of the colliculus atlantis, the rate of its occurrence, the causes for its absence, and the radiological-clinical importance in the analysis of open-mouth-view radiographs. MATERIAL AND METHODS: Retrospective analysis of standardized radiographs of the cervical spine in more than 20,000 adults and 100 children. Study of 234 human skeletons of different ages and of 38 isolated adult atlases. Cadaveric dissection of 42 adults (age 48-87). Axial radiographs of isolated atlases and analysis of the bony structures of the colliculus atlantis.

Anterior zero-profile V-osteotomy: A safe and effective approach for segmental and localized-type cervical OPLL.

OBJECTIVE: Anterior cervical subtotal vertebral decompression and fusion (ACCF) provides an extensive decompression space, and the orthopedic surgeon needs to remove a large number of endplates and vertebrae to achieve sufficient decompression. In contrast, V-shaped vertebral osteotomy performed via the intervertebral space achieves effective decompression through resection of the entire pathological posterior wall tissue while allowing for better preservation of the vertebral endplates. This study therefore aims to evaluate the efficacy and safety of the surgical approach utilizing the zero-p

Postcranial pneumaticity: an evaluation of soft-tissue influences on the postcranial skeleton and the reconstruction of pulmonary anatomy in archosaurs.

Postcranial pneumaticity has been reported in numerous extinct sauropsid groups including pterosaurs, birds, saurischian dinosaurs, and, mos … n Neotheropoda (e.g., Abelisauridae, Allosauroidea), the following pattern emerges: pneumaticity of cervical vertebrae and ribs suggests pneumatization by lateral vertebral diverticula of a cervical air sac system, with sacral pneumaticity indicating the presence of caudally expanding air sacs and/or diverticula. The identification of postcranial pneumaticity in ext …

Cervical kyphosis and myelopathy. Treatment by anterior corpectomy and strut-grafting.

Between 1976 and 1984, fourteen patients who had severe cervical kyphosis and myelopathy were treated with anterior decompression and arthrodesis. Eight had had spondylosis; five, a traumatic injury; and one, a benign intradural tumor. In eight of the fourteen patients, the severe kyphosis and myelopathy had developed after a laminectomy of three, four, or five cervical vertebrae. The laminectomy had been done for the treatment of spondylosis in five patients, of a traumatic lesion in two, and of a tumor in one. Considering all fourteen patients, an average of 2.25 vertebral bodies was removed…

Time to Mobility Is Associated With Pulmonary Complications in Patients With Spine Fractures.

INTRODUCTION: Treatment of spine fractures may require periods of prolonged immobilization which prevents effective pulmonary toileting. We hypothesized that patients with longer time to mobilization, as measured by time to first physical therapy (PT) session, would have higher pulmonary complications. METHODS: We performed a retrospective review of all trauma patients with cervical and thoracolumbar spinal fractures admitted to a level 1 trauma center over a 12-month period. Demographic data collection included age, gender, BMI, pulmonary comorbidities, concomitant rib fractures, admission G

Development of a Novel Cervical Deformity Surgical Invasiveness Index.

STUDY DESIGN: Retrospective review. OBJECTIVE: The aim of this study was to develop a novel surgical invasiveness index for cervical deformity (CD) surgery that incorporates CD-specific parameters. SUMMARY OF BACKGROUND DATA: There has been a surgical invasiveness index for general spine surgery and adult spinal deformity, but a CD index has not been developed. METHODS: CD was defined as at least one of the following: C2-C7 Cobb >10°, cervical lordosis (CL) >10°, cervical sagittal vertical axis (cSVA) >4 cm, chin brow vertical angle >25°. Consensus from experienced spine and neurosurgeons s

Cervical sagittal alignment in adult hyperkyphosis treated by posterior instrumentation and in situ bending.

BACKGROUND: In the normal adult spine, a link between thoracolumbar and cervical sagittal alignment exists, suggesting adaptive cervical positional changes allowing horizontal gaze. In patients with thoracic hyperkyphosis, cervical adaptation to sagittal global alignment might be different from healthy individuals. However, this relationship has not clearly been reported in hyperkyphotic deformity. PURPOSE: The purpose of this study was to identify cervical sagittal alignment types observed on radiographs in young adults with thoracic hyperkyphosis. The relationship between cervical and thora

Rosai-Dorfman Disease of Rare Isolated Spinal Involvement: Report of 4 Cases and Literature Review.

BACKGROUND: Rosai-Dorfman disease (RDD) is a rare histioproliferative disorder that only occasionally involves the central nervous system (CNS). CASE DESCRIPTION: The diagnosis and treatment of 4 patients with isolated spinal RDD are discussed. All 4 patients were treated by total or subtotal surgical resection and none of them experienced recurrence. Histopathologic examination showed a characteristic emperipolesis, and the lymphocytes were engulfed in the S-100-protein-positive histiocytes with no expression of CD1a. CONCLUSIONS: Preoperative diagnosis of spinal RDD is still challenging be
