Intralesional microbial community signatures across histopathologic categories of tongue lesions.

BACKGROUND: The tissue-resident microbiome of oral epithelial lesions remains incompletely characterized, with most prior studies relying on saliva or surface sampling. This study aimed to characterize intralesional microbial communities across histopathologic categories of tongue lesions using formalin-fixed paraffin-embedded (FFPE) tissue. METHODS: This cross-sectional study characterized the intralesional microbiome of 63 formalin-fixed, paraffin-embedded tongue tissues, including fibroma (n = 15), low malignant potential dysplasia (LMP; n = 24), high malignant potential dysplasia (HMP; n

A communication phenotype for varying information needs among caregivers of persons with dementia.

BACKGROUND AND OBJECTIVES: Informal caregivers of persons with Alzheimer's disease and Alzheimer's disease-related dementias (AD/ADRD) have limited knowledge of the disease, yet they have immense responsibility, such as medication administration, managing symptoms, assisting with activities of daily living, and making end-of-life decisions. Our objective was to explore information seeking among informal caregivers to learn about their information needs. RESEARCH DESIGN AND METHODS: We interviewed informal caregivers of persons with AD/ADRD who passed away in the last 2 years. Interviews occur

Diagnostic discordance and differential diagnoses of community-acquired pneumonia in the emergency department.

BACKGROUND: Community-acquired pneumonia (CAP) is a common reason for presentation to the emergency department (ED), but accurate diagnosis is challenging due to overlapping and often nonspecific clinical features. Data on the frequency and spectrum of alternative diagnoses in patients initially diagnosed with CAP in the ED are limited. The aim of this study was to evaluate the diagnostic concordance of an initial ED diagnosis of CAP, characterize common alternative discharge diagnoses, and analyze clinical, laboratory, imaging utilization, and vital sign parameters associated with confirmed C

Fungal extracellular vesicles mediate conserved cross-species communication and immunomodulation.

Extracellular vesicles (EVs) play crucial roles in fungal communication and host immune modulation, representing potential therapeutic targets for fungal infections. This study investigated the role of fungal EVs in both intra- and interspecies communication, focusing on their effects on virulence and immune responses. Co-incubation experiments were performed using EVs derived from Candida albicans and Candidozyma auris to assess interactions with C. albicans planktonic cells and biofilms, as well as Cryptococcus neoformans and Cryptococcus gattii EVs interacting with C. neoformans cultures. Clinician Communication About Uncertainty During Parkinson Disease Diagnostic Consultations.

BACKGROUND AND OBJECTIVES: Parkinson disease (PD) entails much uncertainty, regarding diagnosis, disease course, and treatment. How clinicians communicate about such uncertainty may affect patients' understanding of the information and their well-being. However, clinicians rarely receive formal training in how to optimally discuss uncertainty during diagnostic consultations. We assessed (1) how clinicians communicate about uncertainty, (2) how their communication about uncertainty predicts patients' feelings of uncertainty and trust, and (3) the moderating effect of clinicians' and patients' t

Caenorhabditis elegans in neuroscience: from neural communication to neurodegenerative disease modeling.

Consistent with humans and other metazoans, the nematode Caenorhabditis elegans (C. elegans) undergoes progressive structural and functional decline during aging. Possessing highly conserved genetic pathways that share extensive homology with human genes, and characterized by a streamlined, fully mapped connectome, C. elegans has emerged as a robust model for dissecting the mechanisms underlying neuronal aging and degeneration. In this review, we summarize the intrinsic advantages of C. elegans as a model organism, highlighting its readily quantifiable behavioral phenotypes, short lifespan, an

Protecting School Community Health During Extreme Heat and Wildfire Events.

Extreme heat and wildfire events are becoming more frequent across the United States, creating emerging environmental health challenges for school communities. Exposure to high temperatures and wildfire emissions can adversely affect physical and emotional health, attendance, well-being, and academic or work performance. Students, especially those with asthma or other chronic health conditions, as well as staff with preexisting health concerns, are more susceptible to these exposures and face greater risk. School nurses are uniquely positioned at the intersection of healthcare and education to

Periodontal Disease as a Marker of Malnutrition Risk in Community-Dwelling Older Adults.

OBJECTIVE: Periodontal disease (PD), a prevalent condition in older adults, may impair nutritional status through reduced masticatory function, systemic inflammation, and altered dietary intake. However, few studies have examined the relationship between clinically assessed PD and validated nutritional risk metrics, that is, the Geriatric Nutritional Risk Index (GNRI). In the present study, we examined the association of PD severity with GNRI-defined malnutrition risk in a nationally representative sample of the US National Health and Nutrition Examination Survey (NHANES). DESIGN: A cross-sec

Genetic variability of SARS-CoV-2 in wastewater and associations with community transmission.

Sequencing genetic material from community wastewater facilitates the study of virus diversity. Diversity can be estimated using the variation in nucleotides and number of variants identified, increasing the information obtained from one wastewater sample. Using 12,290 wastewater sequences of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), we found that virus diversity is a strong indicator of disease transmission, with a significant correlation between average weekly diversity and average weekly new COVID-19 infections. Although the concentration of viruses is the common measure

Comparison of community-level deprivation indices and their association with adverse pregnancy outcomes.

BACKGROUND: The Social Vulnerability Index, Maternal Vulnerability Index, and Area Deprivation Index measure community-level resilience to stressors, vulnerability to adverse maternal health outcomes, and socioeconomic disadvantages, respectively. These indices are commonly used to gauge community-level influence on health outcomes. Thus, for them to serve as effective clinical tools, their ability to predict adverse obstetrical outcomes must be evaluated. OBJECTIVE: This study aimed to compare the 3 indices and their associations with adverse pregnancy outcomes. STUDY DESIGN: This was a sec

Persistent Postural-Perceptual Dizziness: A Practical Approach to Diagnosis and Patient Communication.

BACKGROUND: Despite clear diagnostic criteria established in 2017, delivering and explaining the diagnosis of persistent postural-perceptual dizziness (PPPD) can be challenging. METHODS: We outline a step-by-step approach to clearly explain the diagnosis, underlying mechanisms and treatment options to patients with PPPD, followed by a brief overview of current treatment approaches. This approach is adapted from the Cambridge-Calgary Consultation Model and previous recommendations for functional neurological disorders but is mainly based on our expertise accrued over decades and input from pat

We Dissent: Lessons From the 2025 Community-Acquired Pneumonia Guidelines.

Clinical practice guidelines profoundly influence patient care, making transparency, rigor, and fairness in their development essential. The 2025 community-acquired pneumonia (CAP) guideline update, developed by the American Thoracic Society (ATS) and initially co-sponsored by the Infectious Diseases Society of America (IDSA), included a recommendation to prescribe antibacterials for CAP upon detection of a respiratory viral pathogen. Here, we acknowledge the guidelines' strengths with respect to supporting shorter treatment duration but highlight the lack of supporting data for antibacterial

Nutrition inequities in the Deaf community: a call for inclusive public health action.

Research shows that understanding the nutritional status and eating habits of people with disabilities is essential for creating effective preventive healthcare strategies. Communication barriers in healthcare settings, low health literacy and socioeconomic inequities are among the challenges this community faces. These difficulties can lead to poor nutritional knowledge, food insecurity and chronic diseases. Deaf individuals also experience higher rates of undernutrition, obesity and micronutrient deficiencies, primarily due to limited access to linguistically appropriate nutrition education.

Community Assessment of Hurricane Helene Recovery in Rural Western North Carolina.

Rural communities face unique challenges after a disaster as a result of overlapping vulnerabilities related to limited housing and transportation infrastructure, employment or income loss, and fewer emergency response and recovery resources. Hurricane Helene (Southeast Coast, USA; 2024) made landfall in Florida as a Category-4 hurricane, later impacting Western North Carolina with severe flooding, landslides, and hurricane-force winds. Communications and transportation were interrupted for months, leading to disinformation, recovery disruptions, and a loss of trust. To assess household impact
