A case of 56-year-old man with tuberculous encephalopathy following pulmonary tuberculosis was reported. Computed tomography (CT) revealed low density virtually confined to the white matter of the cerebral hemisphere. Contrast enhanced CT demonstrated intracranial multiple spotted lesions, all of which were homogeneously enhanced, supporting multiple intracranial tuberculomas. Magnetic resonance miss spelling imaging (MRI) suggested brain edema and demyelination of the white matter. Clinically this case was characterised by evidence of diffuse cerebral involvement in the form of convulsions, a

The pharmacological profile of chlordesmethyldiazepam was studied in mice and compared with that of other benzodiazepines (diazepam, lorazepam, clonazepam, nitrazepam, oxazepam, flunitrazepam, medazepam and chlordiazepoxide). All the drugs were administered perorally and their anticonvulsive activity (antagonism towards pentetrazole-induced clonic convulsions), anxiolytic action (the four-plate test), myorelaxant activity (the rota-rod test), sedative effect (inhibition of the locomotor activity) and neurotoxic effect (abolition of the righting reflex) were estimated. Chlordesmethyldiazepam re

Since 1955, a standardized treatment regimen has been used to manage 245 cases of eclampsia at Parkland Memorial Hospital. Magnesium sulfate alone effectively controlled controlled convulsions in the great majority of cases. The only maternal death among the 245 cases reemphasizes the risk of respiratory arrest that is inherent in the administration of magnesium sulfate when given in large doses intravenously. Hydralazine to lower the diastolic blood pressure somewhat, when it was 110 mm Hg or higher, prevented intracranial hemorrhage. Avoidance of diuretics and hyperosmotic agents and limitat

We describe 31 children with runs of high amplitude and variable frequency spike-and-wave discharges on the occipito-postero-temporal regions at the EEG. This characteristic EEG pattern can be seen on the posterior areas of one or both hemispheres only when the eyes are closed. A functional spike-wave focus of temporo-rolandic localization also occurred in 13 cases. Clinically, 10 children were seizure-free with minimal brain dysfunction in 8 of them. Of these, only 4 patients suffered from febrile convulsions while the 17 other patients were undergoing treatment for visual, motor or vegetativ

The effects of hemorrhage and parenteral hypertonic saline on the behavioural responses to centrally-administered arginine vasopressin (AVP) were examined in rats. Both hemorrhage and hypertonic saline act as potent stimuli for neurohypophysial vasopressin release, and may serve as potential stimuli for cerebral AVP release. When administered into a lateral cerebral ventricle of the rat brain, AVP has a potent convulsant action; this effect increases in severity upon subsequent administration. Removal of 15% of the estimated blood volume from the conscious rat or infusion of 1.0 ml of 1.5 M so

During a national survey of Hunter's syndrome, 52 boys with the severe form were ascertained. The average ages of onset and death were 2.47 and 11.77 years respectively. Most patients had a large head and short stature. Persistent diarrhoea was noted in 65 per cent of the patients. 76 per cent had an umbilical hernia. Right and left inguinal herniae were noted in 51 and 32 per cent respectively. Evidence of cardiovascular disease was found in 65 per cent of cases; and serious lower respiratory-tract disease occurred in 84 per cent. Induction of anaesthesia proved difficult or impossible in fiv

A severely mentally retarded infant with congenital lactic acidosis due to pyruvate carboxylase deficiency is reported. The patient suffered from vomiting and convulsions soon after birth and developed severe mental and motor retardation at 3 months of age. The persistent elevation of pyruvate and lactate in both blood and cerebrospinal fluid and hyperalanaemia suggested an impairment of pyruvate oxidation. The enzyme activities of pyruvate carboxylase in both liver tissues and cultured skin fibroblasts of the patient revealed values of about 5% of controls. However, pyruvate dehydrogenase and

In the intensive care unit of Leiden University Neurosurgical Department 52 critically ill patients were given continuous infusions of etomidate to achieve 'total immobilization'. This can be defined as a state whereby there is complete elimination of all psychic, sensory and motor unrest. Etomidate administration varied from 2 hours to 25 days and dosages began at 20 micrograms/kg/minute and were increased to 110 micrograms/kg/minute according to need and indications for varying periods of time. Attempts were made to control unrest, convulsive seizure activity and raised intracranial pressure

378 classifiable epileptics attending the neurology clinics at Lagos University Teaching Hospital were investigated for acquired aetiological factors. 34.9% had identifiable aetiology. The commonest aetiological factor was childhood febrile convulsion, a factor most commonly associated with partial epilepsy with complex symptomatology. Other factors were: head injury, birth injury, CNS infections, cerebrovascular accidents, intracranial space occupying lesions and sickle cell disease; 39% of patients with partial epilepsy had acquired aetiological factors. The 61% of them without identifiable

Case report on a premature male infant, born in the 34th week of pregnancy with bronchopulmonary dysplasia, bronchitis, hepatitis, marked cortical and subcortical atrophy of the brain with enlargement of the inner and outer subarachnoid space, with severe cerebral motility disorders and convulsions. Blood serology showed rapidly increasing cytomegaly titers, which were also elevated in the serum of the mother. The ophthalmological examination revealed a hypoplastic, white papilla in both eyes. Neither retinal arteries nor veins were seen. The macula and retinal periphery were normal. There wer

Strychnine poisoning results in a predictable and treatable sequence of events involving blockade of the inhibitory neurotransmitter, extensor muscle spasms, seizures, and respiratory paralysis. These spasms may lead to hyperthermia, profound lactic acidosis, and rhabdomyolysis. Acidosis is primarily attributable to lactate, as indicated by the correlation between arterial pH and log of lactic acid concentration (r = -0.878). Interruption of the strychnine blockade is the primary therapy for strychnine poisoning. Phenobarbital in moderate doses should be the first intervention and anesthetic d

Complications from mydriatic and cycloplegic drugs are rare compared with their extensive use. Adverse effects are often related to dosage or other factors. The ocular complications include increased intraocular pressure, pigmentation of the conjunctiva and cornea, pigment in the anterior chamber, lacrimal duct blockage, macular edema, corneal endothelium damage, hyperemia, allergy, discomfort, and blurred vision. The systemic complications are those common to sympathomimetic and parasympatholytic drugs and include tachycardia, hypertension, headache, faintness. pallor, trembling, excessive sw

Autopsies and laboratory examinations of material from 24 Australian pelicans found sick or dead in southern coastal Queensland in 1977 to 1979 revealed dieldrin poisoning in 8 from the Brisbane region and botulism in 8 from Brisbane, Bundaberg and Gladstone. In those diagnosed as dieldrin poisoning, brain and liver samples contained 12.1 to 27.4 and 34.0 to 48.1 mg/kg dieldrin respectively. All of these birds were emaciated, 2 had convulsed and 1 had muscle tremors. Low and probably insignificant residues of DDE were detected in many birds. Type C botulism was confirmed in 4 of the 6 birds te

Two cases of congenital lactic acidosis associated with pyruvate carboxylase deficiency are described. One 2-mo-old infant had a fulminant clinical course with extremely severe intractable acidosis and died after 48 h in hospital. The second infant, aged 2 1/2 mo, had a milder clinical course, characterized by moderate acidosis and frequent convulsive episodes. He died at the age of 3 mo due to respiratory arrest following prolonged status epilepticus. Pyruvate carboxylase activity in liver biopsy specimens obtained from the two patients was 1 and 50% of normal, respectively. Both patients fai

A 24-yr-old male patient that suffered from chronic tetany since school age. At the age of 20 tetanic convulsions occurred due to hypocalcemia. His mother also had chronic tetany due to pseudohypoparathyroidism. At the age of 24, hypocalcemia caused by pseudohypoparathyroidism was noted. Hypopotassemia persisted even when the hypocalcemia improved with the administration of 1 alpha-hydroxycholecalciferol and calcium lactate. Other findings were normal blood pressure, high levels of plasma renin activity and serum aldosterone, a fall in blood pressure after angiotensin II antagonist infusion, b

The authors performed galactose loading tests in children suffering from chronic diseases: recurrent bronchitis vomiting, diarrhoea, milk-intolerance, somatic and mental retardation, cramps. In 32 of the 92 examined cases galactose levels rose until pathological, pseudo- diabetic levels. Stillbirth, cataract, hyperbilirubinaemia, convulsions occurred among family members of 10 patients. Galactose-1-phosphat-uridyl-transferase levels were decreased only in 4 of the 17 patients examined. In the other cases some different pathway of galactose metabolism is suspected. Complete remission of symptom

Use of subarachnoid/intraventricular administration of hyaluronidase in the treatment of 15 cases of cranial arachnoiditis which occurred as a complication of tuberculous meningitis is reported. Eleven of these cases had communicating hydrocephalus and four had optochiasmatic arachnoiditis. An average of 4.5 injections were administered at weekly to fortnightly intervals. These patients had been followed-up for a mean period of 6.1 months per patient. Serious side effects such as flare-up of the underlying meningitis process, miliary spread of tuberculosis, convulsions or allergic reactions we

The frequency of the various degrees of the intellectual defect and convulsive syndrome was studied in 473 patients with phenylketonuria divided into four groups as regards the phenylalanine blood level, that was 150 to 199 mg/l in group I, 200 to 299 mg/l in group II, 300 to 399 mg/l in group III, and 400 mg/l and higher in group IV. A substantial increase of the number of patients with grave mental derangement and epileptic seizures was noted in groups II and IV: this shows that the risk of the grave mental affection is the greater the higher the phenylalanine serum level. An increased numbe

22 patients with severe preeclampsia-eclampsia were treated in our Intensive Care Unit from 1972 to 1978. Control of convulsions was achieved by diazepam, diphenylhydantoin and phenobarbital. In 11 comatose patients brain monitoring was carried out by frequent neurological examination and use of computerized x-ray tomography; aspiration of gastric contents was prevented by nasotracheal intubation. Brain oedema therapy included controlled hyperventilation, steroids and mannitol (7 patients). 10 patients with respiratory failure (due to pulmonary oedema, "shock lung" or aspiration pneumonitis) w

A 28-year-old man suffered from recurrent facial exanthema, arthritis and stomatitis for ten years and died six months after a catatonic episode with terminal cerebral convulsions. Three years before his death high KBR-Antititers to Herpes simplex- and cytomegalic virus were observed, while Lupus-Erythematosus-Tests (LE-Tests) only became positive in the last months. At autopsy, changes compatible with Systemic Lupus Erythematosus (SLE) were found in the mitral valves, the spleen, and the kidneys. The brain displayed hemorrhagic infarction of the striate bodies and thrombophlebitis of the inte
