Controversies in the Management of Bicolumnar Fractures of the Distal Humerus.

Bicolumnar fractures of the distal humerus pose numerous treatment challenges for upper-extremity surgeons. Although open reduction and internal fixation demonstrates advantages compared with nonsurgical treatment, restoration of osseous anatomy can be difficult, particularly for comminuted, intra-articular fractures. Despite well-recognized complications, total elbow arthroplasty remains an option for elderly patients with fractures not amenable to fixation. Although indications remain controversial, distal humerus hemiarthroplasty has emerged as a potential alternative to total elbow arthrop…

Imaging the Postsurgical Upper Limb: The Radiologist Perspective.

Imaging has a paramount role in postsurgical assessment. Radiologists need to be familiar with the different surgical procedures to be able to identify expected postsurgical appearances and also detect potential complications. This article reviews the indications, normal expected postsurgical appearances, and complications of the most frequently used surgical procedures in the shoulder, elbow, and wrist. The emphasis is on points that should not be overlooked in the surgical planning.

Capitellum Fracture Open Reduction and Internal Fixation With Headless Screws.

Partial articular fractures involving the capitellum represent an injury to the lateral column of the distal humerus. Capitellum open reduction and internal fixation restores this portion of the distal humerus articular surface and biomechanical integrity to the lateral column. This video demonstrates open reduction and internal fixation of a type I capitellar fracture with headless compression screws through an extensile lateral approach.

Major League Baseball pitch velocity and pitch type associated with risk of ulnar collateral ligament injury.

BACKGROUND: The number of Major League Baseball (MLB) pitchers requiring ulnar collateral ligament (UCL) reconstructions is increasing. Recent literature has attempted to correlate specific stresses placed on the throwing arm to risk for UCL injury, with limited results. METHODS: Eighty-three MLB pitchers who underwent primary UCL reconstruction were evaluated. Pitching velocity and percent of pitch type thrown (fastball, curve ball, slider, and change-up) were evaluated 2 years before and after surgery. Data were compared with control pitchers matched for age, position, size, innings pitched

Posterior interosseous nerve incarceration with endobutton repair of distal biceps.

Distal biceps ruptures are uncommon injuries that comprise approximately 3% of all biceps pathology. This injury is most commonly seen in 40- to 60-year-old men, and the mechanism of injury involves a forceful extension movement to a flexed elbow. Without surgical intervention, patients are left with measurable weakness in elbow flexion and supination as well as a cosmetic deformity that often leaves them dissatisfied. Consequently, early surgical repair is advocated for physically active individuals. A variety of surgical approaches and fixation devices are currently used for distal biceps re…

[Pediatric supracondylar fractures of the humerus].

Supracondylar fractures are on the of the most common injuries in paediatric patients. Therapeutic consequences depend on the one hand on the degree of the osseous displacement according to the Gartland classification. On the other hand, the diagnosis of concomitant neuro-vascular injuries is crucial to prevent long term impairment. Never the less, clinical diagnosis can be challenging. In-depth knowledge of pediatric supracondylar humerus fractures is a prerequisite to achieve a functionally and cosmetically satisfactory long-term outcome.

[Type I Salter-Harris lesion of the distal humeral epiphysis in children under one year of age. Case report and literature review].

The pure physeal lesion of the distal humeral epiphysis is an uncommon and difficult-to-diagnose condition due to the absence of calcified ossification centers during the first three months of life, which usually leads to delaying the diagnosis or to misdiagnosis. However, the prognosis is good and complications are few. We report the case of a 3-month-old female patient who was admitted for open reduction and fixation with Kirschner nails.

Unusual pattern of Essex-Lopresti injury with negative plain radiographs of the wrist: a case report and literature review.

The Essex-Lopresti injury consists of a fracture of the radial head, rupture of the interosseous membrane and disruption of the distal radioulnar joint. The greatest challenge with this injury pattern is the diagnosis, because it is frequently missed and the attention usually focused on the elbow joint. In this paper we report an unusual pattern of Essex-Lopresti injury with a radial neck fracture, a tear of the interosseous membrane and a disruption of the distal radioulnar joint in which initial wrist radiographs did not show significative abnormalities. Open reduction and internal fixation …

Ulnar collateral ligament injury in the overhead athlete.

The ulnar collateral ligament (UCL), particularly the anterior portion of the anterior oblique ligament, is the primary static contributor to elbow valgus stability. UCL injuries are most common in athletes participating in overhead sports. Acute and chronic injuries to the UCL result in valgus instability, which may predispose the athlete to the development of disabling secondary elbow conditions. Provocative physical examination maneuvers include the valgus abduction test, the modified milking maneuver, and the moving valgus stress test. Plain radiographs and magnetic resonance imaging are t…

Anatomic variation--a bony canal for the median nerve at the distal humerus: a case report.

We report a rare anatomic variation in which the median nerve travels in a bony canal of the distal humerus, separating from the brachial artery. We encountered this during neurolysis for posttraumatic median nerve palsy in a 10-year-old boy. We suggest that this anatomic variation was related strongly to this patient's median nerve palsy.

[Late reconstruction of distal biceps tendon rupture with fascia lata graft and Mitek anchors].

Ruptures of the distal biceps tendon are rare but well documented. A technique for reinserting the distal biceps tendon into the radial tuberosity with two Mitek anchors and a fascia lata graft is presented and illustrated by a case report. Ectopic ossification as a complication after the procedure is discussed as well as the final outcome. It could be concluded that late reconstruction of the distal biceps tendon rupture with fascia lata and Mitek anchors is a safe procedure that can be advocated in a restricted number of cases where acute injury has been overlooked.

The role of the extensor digitorum communis muscle in lateral epicondylitis.

A common finding in tennis elbow is pain in the region of the lateral epicondyle during resisted extension of the middle finger (Maudsley's test). We hypothesized that the pain is due to disease in the extensor digitorum communis muscle, rather than to compression of the radial nerve or disease within extensor carpi radialis brevis. Thirteen human forearm specimens were examined. It was found that the extensor digitorum communis was separable into four parts. The part to the middle finger originated from the lateral epicondyle, but the muscle slips to the other fingers originated more distally…

[The DASH questionnaire. French translation of a trans-cultural adaptation].

The DASH (Disability of Arm-Shoulder-Hand) is a self-administered questionnaire developed in 1994 by representatives of the Institute for Work & Health (IWH) and the American Academy of Orthopaedic Surgeons (AAOS). It measures the physical disability and symptoms for all upper limb disorders in a heterogeneous population and for acute as well as chronic disorders. The original american version has been already tested for reliability and validity. Interest in the DASH was raised by several European publications. It appeared that the DASH could provide a common measure for upper extremity physic

Delayed presentation of 50 years after a World War II vascular injury with intraoperative localization by duplex ultrasound of a traumatic false aneurysm.

A case of delayed presentation of a traumatic false aneurysm in the left arm 50 years after penetrating injury sustained during World War II is described. The original injury resulted in brachial artery occlusion and complete median nerve palsy. The false aneurysm presented with a spontaneous, contained rupture. Surgical repair was performed after duplex ultrasound localization of the lesion to a small collateral artery lateral to the elbow, thereby avoiding dissection in the densely scarred tissue plains in the antecubital fossa. Duplex ultrasound was also used intraoperatively to facilitate …

Traumatic dislocation of the radial head at birth.

A female delivered in a breech presentation after manipulation of the arms was found 2 days later to have sustained anterior dislocation of the left radial head. Surgical correction proved successful, and when re-examined 5 years later the elbow had remained normal. It is probable that in many cases of unilateral displacement of the radial head, detected later in life but reported as congenital, the malposition has been a sequela after unrecognized birth injury.

A wearable functional electrical stimulation device with a movable electrode for motor point tracking.

Functional electrical stimulation is widely applied in the rehabilitation of individuals with cerebrovascular disease or spinal cord injury but is limited by rapid muscle fatigue. We present a novel stimulation strategy based on real-time motor point tracking using a wearable functional electrical stimulation device with a movable electrode. A crank-slider mechanism drives the electrode to follow the biceps brachii motor point trajectory according to elbow joint angle, aiming to optimize stimulation site and reduce fatigue. Seven healthy male participants compared this approach with time-shift…
