Availability and affordability of blood pressure-lowering medicines and the effect on blood pressure control in high-income, middle-income, and low-income countries: an analysis of the PURE study data.

BACKGROUND: Hypertension is considered the most important risk factor for cardiovascular diseases, but its control is poor worldwide. We aimed to assess the availability and affordability of blood pressure-lowering medicines, and the association with use of these medicines and blood pressure control in countries at varying levels of economic development. METHODS: We analysed the availability, costs, and affordability of blood pressure-lowering medicines with data recorded from 626 communities in 20 countries participating in the Prospective Urban Rural Epidemiological (PURE) study. Medicines

Assessing Pregnancy, Gestational Complications, and Co-morbidities in Women With Congenital Heart Defects (Data from ICD-9-CM Codes in 3 US Surveillance Sites).

Improved treatment of congenital heart defects (CHDs) has resulted in women with CHDs living to childbearing age. However, no US population-based systems exist to estimate pregnancy frequency or complications among women with CHDs. Cases were identified in multiple data sources from 3 surveillance sites: Emory University (EU) whose catchment area included 5 metropolitan Atlanta counties; Massachusetts Department of Public Health (MA) whose catchment area was statewide; and New York State Department of Health (NY) whose catchment area included 11 counties. Cases were categorized into one of 5 m

Life's Essential 8 in Pregnancy and Time to Incident Cardiometabolic Disease Over 7 Years Follow-Up.

IMPORTANCE: Assessment of cardiovascular health (CVH) during pregnancy may unmask latent metabolic vulnerability and indicate long-term disease risk. However, the prognostic value of the American Heart Association's Life's Essential 8 (LE8) framework during pregnancy remains uncertain. OBJECTIVE: To evaluate CVH during pregnancy using a modified LE8 (mLE8) score in association with time to incident cardiometabolic disease. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used electronic medical record (EMR) surveillance for 7 years post partum (August 2018 to March 2026) and included sin

[Treatment and prognosis of idiopathic intracranial hypertension in children. Retrospective study (1995-2009) and literature review].

AIM: Idiopathic intracranial hypertension (IIH) may cause severe visual loss due to the optic nerve damage. Routine management involves mainly medical treatment. The aim of this study was to improve diagnosis and management of IIH in children. METHODS: The medical records of all patients with definite IIH seen at the children's hospital of Toulouse between 1995 and 2009 were reviewed. Cases of secondary intracranial hypertension were included because they did not present any cerebral lesions and underwent a similar therapeutic approach. The clinical and ophthalmological data at the beginning

Comparative efficacy and safety profile of amlodipine 5 mg/losartan 50 mg fixed-dose combination and amlodipine 10 mg monotherapy in hypertensive patients who respond poorly to amlodipine 5 mg monotherapy: an 8-week, multicenter, randomized, double-blind phase III noninferiority study.

BACKGROUND: The number of hypertensive patients achieving treatment targets is not ideal with therapies that engage a single mechanism of action, and combination therapies using different mechanisms of action can increase drug efficacy in a synergistic way. OBJECTIVE: This noninferiority study compared the clinical efficacy and safety profile of fixed-dose combination of amlodipine/losartan 5/50 mg and amlodipine 10 mg monotherapy in essential hypertensive patients who respond poorly to amlodipine 5 mg monotherapy. METHODS: This was a double-blind, multicenter, randomized trial of hypertensi

Isometric hand grip training on populations with different baseline resting blood pressure: a systematic review and meta-analysis.

BACKGROUND: Hypertension is a major global public health challenge. Previous studies have confirmed that isometric hand grip training (IHG) reduces resting blood pressure (BP). However, most existing trials have focused on mixed populations, and the differential effects of IHG on participants with varying baseline BP classifications remain unclear. While guidelines vary globally, it is essential to determine whether IHG exerts similar efficacy across Normal, High-Normal, and Hypertensive categories to optimize exercise prescriptions. OBJECTIVE: Based on the blood pressure classification crite

A ten-year review of indications and outcomes of obstetric admissions to an intensive care unit in a low-resource country.

INTRODUCTION: Obstetric intensive care unit admission (ICU) suggests severe morbidity. However, there is no available data on the subject in Ghana. This retrospective review was conducted to determine the indications for obstetric ICU admission, their outcomes and factors influencing these outcomes to aid continuous quality improvement in obstetric care. METHODS: This was a retrospective review conducted in a tertiary hospital in Ghana. Data on participant characteristics including age and whether participant was intubated were collected from patient records for all obstetric ICU admissions f

[Glomerulonephritis and vasculitis as causes of arterial hypertension].

The various types of glomerulonephritis, including many forms of vasculitis, are responsible for about 15% of cases of end-stage renal disease (ESRD). Arterial hypertension represents a frequent finding in patients suffering from glomerulonephritis or vasculitis and hypertension also serves as an indicator for these severe types of diseases. In addition, there are symptoms and signs like hematuria, proteinuria and renal failure. Especially, rapidly progressive glomerulonephritis (RPGN) constitutes a medical emergency and must not be missed by treating physicians. This disease can either occur

White Matter Hyperintensities of Presumed Vascular Origin: Current Understanding of Development and Progression.

BACKGROUND AND PURPOSE: White matter hyperintensities (WMH) of presumed vascular origin are among the most common MRI manifestations of cerebral small vessel disease and are a major contributor to vascular contributions to cognitive impairment and dementia. Although WMH have traditionally been attributed to chronic ischemia resulting from small vessel pathology, the biological mechanisms underlying their development and, more importantly, their progression remain incompletely understood. Recent advances in longitudinal neuroimaging, quantitative MRI, pathology, genetics, and physiological imag

Efficacy of mental and physical training on blood pressure and resting heart rate among physically inactive young adults with elevated blood pressure: a randomized controlled trial.

Physically inactive adults with elevated blood pressure (BP) are at an increased risk of developing hypertension and cardiovascular comorbidity. Lifestyle-based non-pharmacological interventions are therefore essential. While mindfulness meditation and aerobic exercise independently improve BP, limited evidence exists on their combined effect-mental and physical training (MAPT). This study examined the effectiveness of MAPT on systolic BP (SBP), diastolic BP (DBP), and resting heart rate (RHR) among young adults with elevated BP. In a single-blinded randomized controlled trial, 58 young adults

Women with suspected diagnosis of ovarian cancer in Ghana: how much do we know about them?

BACKGROUND: Addressing disparities in ovarian cancer care between sub-Saharan Africa and other regions begins with the fundamental question, "Who gets ovarian cancer?" This study aimed to identify the demographic and clinical predictors of cancer among women with ovarian tumours. METHODS: We conducted a cross-sectional analysis of women with histology-confirmed ovarian tumour discussed at a multidisciplinary tumour board in a tertiary hospital in Ghana between 2013 and 2024. Descriptive statistics were used to summarise the characteristics of the women, and logistic regression was performed t

Postpartum Care for Parents of Neonatal Intensive Care Unit Infants before and after Adoption of Telemedicine.

OBJECTIVE: Despite higher morbidity and mortality risks, parents of preterm infants miss postpartum visits more often than parents of full-term infants. Whether the introduction of telemedicine improved access to postpartum care among parents of infants in the neonatal intensive care unit (NICU) is unknown. We aimed to compare postpartum visit attendance and care comprehensiveness for NICU parents before and after the option of telemedicine. STUDY DESIGN: We conducted a retrospective cohort study of postpartum parents without a history of hypertension who gave birth <32 weeks at two Philadelp

The Hidden Burden of COMISA: Clinical Implications and Treatment Challenges.

BACKGROUND: Insomnia disorder and obstructive sleep apnea (OSA) are two of the most common sleep disorders in the general population. Their coexistence, referred to as comorbid insomnia and sleep apnea (COMISA), exacerbates nighttime disturbances, increases daytime dysfunction, and further diminishes overall quality of life. COMISA is also associated with elevated cardiovascular and mental health risks, including resistant hypertension, heart failure, stroke, depression, and anxiety. Despite its clinical importance, COMISA often remains underdiagnosed, and its management poses notable challeng

Global incidence and risk factors of pregnancy-related stroke: a systematic review and meta-analysis of over 270 million women.

OBJECTIVE: Stroke is a leading cause of death and disability worldwide, with its prevalence increasing due to risk factors such as hypertension, diabetes, and hyperlipidemia. Pregnancy-related stroke (PRS), a severe pregnancy complication, particularly during the postpartum period, has shown rising incidence rates, occurring at 30 per 100,000 pregnancies, three times higher than in the general young adult population. This study systematically reviews and analyzes global and regional data regarding the incidence and risk factors of PRS. METHODS: Comprehensive searches of PubMed/MEDLINE, Scopus,

Gaps in Community-Based Screening for Non-Communicable Diseases in Saudi Arabia.

BACKGROUND: Non-communicable diseases (NCDs) such as cardiovascular diseases, diabetes, obesity, and cancer are the leading cause of mortality globally and in Saudi Arabia, accounting for more than 70% of all deaths. Despite national initiatives offering free preventive services, screening uptake remains low. This study aimed to describe the demographic and clinical characteristics of individuals participating in community-based NCD screening campaigns in the Eastern Province of Saudi Arabia and to evaluate screening uptake, compliance, and diagnostic outcomes. METHODS: A retrospective cross-

Primary aldosteronism complicated by early-onset heart failure in a young male with a coexisting DMD variant: A case report and literature review.

RATIONALE: Heart failure (HF) is slightly more common in primary aldosteronism (PA) than in essential hypertension, but early-onset HF remains rare. In such cases, underlying genetic cardiomyopathies should be considered. Autonomously secreted aldosterone and activation of the renin-angiotensin-aldosterone system can lead to extremely high aldosterone levels, worsening cardiac function and creating major therapeutic challenges. PATIENT CONCERNS: A 39-year-old male presented with progressive chest tightness and shortness of breath for 4 months. He had a 7-year history of hypertension and persi…

The relation between Down syndrome and co-occurring conditions in children and young adults: A population-based cohort in Denmark, 1977-2016.

BACKGROUND AND OBJECTIVE: Recognizing the common co-occurring conditions among individuals with Down syndrome (DS) is essential for maintaining appropriate screening and optimizing healthcare resources. However, many previous studies relied on hospital- or community-based convenience samples with small sample sizes, limiting their generalizability. This study aimed to investigate the association between DS and co-occurring conditions in a large cohort of children and young adults. METHODS: We conducted a population-based retrospective cohort study using Danish national registers. The cohort h
