Supporting patients using a digital self-management intervention for symptoms of fatigue, pain, and urgency/incontinence in Inflammatory Bowel Disease: a mixed methods process evaluation of trial facilitators.

BACKGROUND: A randomised controlled trial (RCT) of a facilitator-supported digital programme (IBD-BOOST) for symptom management in Inflammatory Bowel Disease found no difference in disease-specific quality of life and symptom relief compared with controls. Process evaluation is necessary to inform the interpretation of trial outcomes. This study evaluated facilitator adherence to intervention delivery and facilitator perspectives on training, intervention delivery and supervision during the trial, and implementation. METHODS: A mixed-methods process-evaluation study nested within an RCT. A ra

Comparison of patient-reported symptoms with multi-item patient-reported outcome measures of fatigue, anxiety, and depression in the clinical care of women undergoing chemotherapy for early breast cancer.

BACKGROUND: As patient-reported symptoms are increasingly incorporated into routine clinical practice and captured in electronic medical records these data can be used to conduct health-related quality of life research studies. This study compares symptom reports from the Patient-Reported Outcomes version of the Common Terminology Criteria for Adverse Events (PRO-CTCAE) and its precursor patient reported symptom monitoring (PRSM) (hereafter PRSM/PRO-CTCAE) with multi-item patient-reported outcome (PRO) scales for fatigue (Functional Assessment of Chronic Illness Therapy/FACIT-Fatigue) and depr…

Which cognitive-behavioural factors play a role in the reduction of post-COVID-19 fatigue following cognitive behavioural therapy and care as usual? A secondary analysis of the ReCOVer study.

OBJECTIVE: A previous randomized controlled trial (RCT) showed the efficacy of cognitive behavioural therapy for fatigue (CBT-F) in relieving severe fatigue in a subgroup of post-COVID-19 patients as compared to care as usual (CAU). The aim of this study, a secondary analysis of the RCT, was to investigate which cognitive behavioural variables mediate the fatigue-reducing effect of CBT and which variables predict a reduction in fatigue independent of the intervention condition. METHODS: A total of 114 patients (CBT-F = 57, CAU = 57) were included. The primary outcome was fatigue severity asse…

Anodal and cathodal transcranial direct current stimulation can decrease force output of knee extensors during an intermittent MVC fatiguing task in young healthy male participants.

Transcranial direct current stimulation (tDCS) has the capacity to enhance force output during a short-lasting maximal voluntary contraction (MVC) as well as during a long-lasting submaximal voluntary contraction until task failure. However, its effect on an intermittent maximal effort is not known. We hypothesized that anodal tDCS applied during or before a maximal fatigue task increases the amplitude of maximal voluntary contraction (aMVC) and voluntary activation (VA) in young healthy male participants. We measured VA, potentiated twitch at rest (Ptw), root mean square electromyogram (EMG),…

Effect of wearing a dorsiflexion assist orthosis on mobility, perceived fatigue and exertion during the six-minute walk test in people with multiple sclerosis: a randomised cross-over protocol.

BACKGROUND: Fatigue in combination with gait and balance impairments can severely limit daily activities in people with multiple sclerosis (PWMS). Generalised fatigue has a major impact on walking ability, with moderately disabled PWMS experiencing difficulty in walking extended distances. Localised motor fatigue in the ankle dorsiflexors can lead to foot drop, further reducing functional ambulation. The aim of this study is to evaluate the effect of a simple dynamic dorsiflexion assist orthosis on walking-induced fatigue, gait, balance and functional mobility in PWMS. METHODS: A randomised c…

An under-active or over-active internal world? An exploration of parallel dynamics within psyche and soma, and the difficulty of internal regulation, in patients with Chronic Fatigue Syndrome and Myalgic Encephalomyelitis.

This paper explores the dynamics brought into analytic work when there is a symmetric fusion between psyche and soma within the patient. It … pact of the latter in relation to internal regulation systems, or lack of, in patients with Chronic Fatigue Syndrome (CFS) and Myalgic Encephalomyelitis (ME). I will draw on similar work carried out by Holland (1997), Simpson (1997) and Simpson et al. (1997). The paper will also employ the concept of the reflective function (Fonagy 2001; Knox 2003), and c …
