Vitamin A supplementation to prevent mortality and short and long-term morbidity in very low birthweight infants.

BACKGROUND: Vitamin A is necessary for normal lung growth and the ongoing integrity of respiratory tract epithelial cells. Preterm infants have low vitamin A status at birth and this has been associated with increased risk of developing chronic lung disease. Several studies have been undertaken to assess whether vitamin A supplementation beyond that routinely given in multivitamin preparations can reduce the incidence of this outcome. OBJECTIVES: To assess the benefit and risk of supplementation with vitamin A in very low birthweight infants. SEARCH STRATEGY: Searches were made of the Oxford

The efficacy and safety of cannabinoids for the treatment of mental disorders and substance use disorders: a systematic review and meta-analysis.

BACKGROUND: Mental disorders and substance use disorders (SUDs) are among the leading reasons for which the medical use of cannabinoids has been approved, but their efficacy and safety in treating these conditions is yet to be established. We conducted a systematic review and meta-analysis of randomised controlled trials (RCTs) testing the efficacy and safety of cannabinoids as the primary treatment for mental disorders or SUDs. METHODS: We searched Ovid MEDLINE, PsychINFO, Cochrane Central Register of Controlled Clinical Trials, Cochrane Database of Systematic Reviews, and Embase for peer-re

Antiepileptic drugs for neuropathic pain and fibromyalgia - an overview of Cochrane reviews.

BACKGROUND: Antiepileptic drugs have been used for treating different types of neuropathic pain, and sometimes fibromyalgia. Our understanding of quality standards in chronic pain trials has improved to include new sources of potential bias. Individual Cochrane reviews using these new standards have assessed individual antiepileptic drugs. An early review from this group, originally published in 1998, was titled 'Anticonvulsants for acute and chronic pain'. This overview now covers the neuropathic pain aspect of that original review, which was withdrawn in 2009. OBJECTIVES: To provide an over

Drugs for preventing migraine headaches in children.

BACKGROUND: It has been estimated that about ten per cent of children between six and 20 years of age suffer from migraine. It is estimated that children with migraine lose one and a half weeks more schooling per year than their peers. Prophylactic drugs can be prescribed when children suffer from frequent or disabling headaches. OBJECTIVES: We aimed to describe and assess the evidence from controlled trials on the efficacy and tolerability of pharmacological agents taken on a regular basis to prevent the occurrence of migraine attacks and/or reduce the intensity of such attacks in children w

Timing of Vasopressin Initiation in Sepsis Patients on Norepinephrine: A Target Trial Emulation.

BackgroundThe optimal timing for initiating vasopressin in septic shock remains uncertain, with conflicting evidence potentially arising from variations in patient severity and methodological limitations such as immortal time bias. This study aimed to evaluate the time-varying effect of early vasopressin initiation across a spectrum of baseline norepinephrine dose requirements.MethodsWe included 5461 patients with septic shock from the MIMIC-IV 2.2 database. A target trial emulation framework was applied, using a clone-censor-weight approach with inverse probability of censoring weighting to a

Retention in Telehealth-Delivered Buprenorphine Treatment for Opioid Use Disorder: Multistate Retrospective Cohort Study.

BACKGROUND: Buprenorphine is an effective treatment for opioid use disorder and is associated with reduced mortality when retention is high; however, national studies show low retention. Evidence from outpatient settings, including telehealth, remains limited, despite population-level analyses suggesting improved retention with virtual care. Further research is needed to better understand retention and optimize care for people with opioid use disorder. OBJECTIVE: This study aimed to describe patient retention in a telehealth-delivered buprenorphine program over 24 months, both overall and by

Theorizing causality: A qualitative study of xylazine-related wound diversity and perceived etiology among people who inject drugs.

BACKGROUND AND AIMS: Incidence and severity of opioid-associated skin and soft-tissue infections, often requiring intensive and costly medical care, have increased substantially since the emergence of xylazine in the US drug supply. Although progress has been made in clinical and harm reduction recommendations for treating xylazine-related wounds and slowing their progression, the mechanisms remain insufficiently established. This study aimed to describe wound experiences among people navigating xylazine exposure and explore their perspectives on wound etiology to inform collaborative clinical…

A feasibility study of a co-designed intervention to manage benzodiazepine dependence and high-risk use in those receiving opioid agonist treatment.

BACKGROUND: Problematic benzodiazepine use alongside opioids contributes to drug-related deaths among people who use drugs. Clinical management varies considerably. An intervention to address the root causes of benzodiazepine use with opioids has been developed, which included maintenance prescribing of diazepam with anxiety, sleep, and pain management, harm reduction, and safety conversations. This study tested the feasibility of recruiting and retaining people in the intervention to address 'street' benzodiazepine use. Outcome measures and economic evaluation data collection were piloted to …

Clinical Impact of Nasal Obstructive Syndrome and Its Current Management Strategies.

Nasal obstruction syndrome (NOS) is inherently complex due to the combination of nasal anatomy and physiology and pathophysiologic processes that together affect airflow, filtering, smell, and the general health of the respiratory system. The nasopharynx consists of different structures together: the septum, turbinates, and nasal valves that together perform the jobs associated with the nasal polyp, which is to regulate air conditioning and mucociliary clearance. Changes that can be recognized in the septum, turbinates, and nasal valve will individually and/or collectively affect the potential
