BACKGROUND: Hypertension often persists after adrenalectomy for primary aldosteronism.
OBJECTIVE: To determine factors associated with resolution of hypertension after adrenalectomy for primary aldosteronism.
DESIGN: Retrospective cohort study.
SETTING: Tertiary care referral center in Rochester, Minnesota.
PATIENTS: All patients who underwent adrenalectomy for primary aldosteronism between 1 January 1993 and 31 December 1999.
MEASUREMENTS: Preoperative plasma renin activity, plasma and urinary aldosterone concentrations, and adrenal imaging. Follow-up blood pressure, measured at a clinic

Epidemiologic studies have shown an important increase in the high mortality of patients with congestive heart failure (CHF) despite optimal medical management. Ventricular arrhythmia was recognized as the most common cause of death in this population. Electrolyte imbalance, myocardial fibrosis, left ventricular dysfunction, and inappropriate neurohumoral activation are presumed responsible for sudden cardiac death. In this study, we focused on the deleterious effects of the overproduction of aldosterone that occurs in patients with CHF. Secondary hyperaldersteronism can be part of several fac

PURPOSE: We compare the effectiveness and efficiency of laparoscopic adrenalectomy to open surgical management of adrenal disorders.
MATERIALS AND METHODS: A retrospective comparison was undertaken of 21 patients who underwent transperitoneal laparoscopic adrenalectomy between April 1996 and May 1997 with 17 patients who underwent open adrenalectomy between October 1994 and January 1996. Any patient suspected of having primary adrenal carcinoma and/or an adrenal lesion larger than 6 cm. was excluded from the study.
RESULTS: Patient demographics were matched well. Mean laparoscopic surgical t

Insulin receptor (IR) gene expression at the mRNA level was investigated in hindlimb skeletal muscle, epididymal adipose tissue and in the liver of rats exposed to prolonged in vivo administration of deoxycorticosterone acetate (DOCA). Following treatment, plasma insulin levels were reduced while glucose levels increased compared to values in control rats. DOCA-treated animals showed an increase in blood pressure and a reduction in body weight. This treatment also induced hypokalemia and decreased plasma protein levels. Sodium levels were unaffected. Moreover, no differences in DNA and protein

Between January 17, 1992 and July 31, 1993, 11 patients with primary aldosteronism, 3 with Cushing's syndrome and 3 with a nonfunctioning adrenal tumor underwent laparoscopic adrenalectomy at our university hospital. In every patient adrenal tumor was removed successfully with adjacent normal adrenal tissue. The initial 10 operations were performed without use of an ultrasonic aspirator or argon beam coagulator, and the subsequent 7 procedures were done with those items. There was no difference between the groups in intraoperative blood loss or operative time. However, the tumors in the former

The problems inherent in visualizing the adrenal glands are illustrated by the variety of imaging modalities that have been employed. Although urography has been the basic screening examination, one can make a sound argument for CT as the procedure of choice in the evaluation of the patient with suspected adrenal pathology. Angiography, venous sampling, and radionuclide studies continue to have a role in selected problem cases. This article reviews the usefulness of available modalities in the context of the clinical evaluation of adrenal disease.

Primary aldosteronism (PA) is a common cause of secondary hypertension, and accurate screening is essential for early detection and intervention. The aldosterone-to-renin ratio (ARR) is the primary screening tool, but most diagnostic laboratories currently rely on immunoassays, which may have limitations in accuracy and reliability. To address this, we investigated the use of liquid chromatography-tandem mass spectrometry (LC-MS/MS) to measure plasma aldosterone concentration (PAC) and plasma renin activity (PRA) and to establish an optimal ARR cutoff value for PA screening. In this study, 123

BACKGROUND: The loss of physiological nocturnal blood pressure (BP) decline is a major contributor to cardiovascular risk, yet its endocrine underpinnings in primary aldosteronism (PA) remain underexplored. This study aimed to elucidate the dose-dependent relationship between aldosterone excess and circadian BP rhythm disruption and assess whether targeted aldosterone suppression could ameliorate nocturnal dipping.
METHODS: We prospectively analyzed 24-hour ambulatory BP data from 681 patients with confirmed PA, stratified by plasma aldosterone concentration. A subset of 99 patients with adre

CONTEXT: Biochemical screening for endocrine hypertension may be complex; the known presence of an adrenal mass markedly increases the pretest probability for adrenal hypertension and may inform diagnostic efforts.
OBJECTIVE: Model the potential incidence of adrenal masses that may be detected by routine adrenal imaging in patients with suspected endocrine hypertension.
DESIGN: Retrospective cohort study.
SETTING: Provincial administrative and clinical databases for radiology and biochemistry, Alberta, Canada.
PARTICIPANTS: Patients with hypertension and any measurement of aldosterone-reni

INTRODUCTION: Obstructive sleep apnea (OSA) and primary aldosteronism (PA) are common and treatable conditions that are often overlooked in the evaluation of hypertension. We assessed the prevalence and relative contribution of identifiable causes of hypertension using a comprehensive diagnostic approach.
METHODS AND MATERIALS: In this cross-sectional study, consecutive adults (aged 18-80 years) with hypertension from the endocrinology OPD were screened for secondary causes. All participants underwent standardized blood pressure assessment and systematic evaluation for major causes of seconda
