Spinal muscular atrophy in the neonate.

Spinal muscular atrophy (SMA) type I is an autosomal recessive disorder characterized by loss of lower motor neurons in the spinal cord. This severe hereditary neurodegenerative disorder is an important cause of morbidity in the neonate and the leading hereditary cause of infant mortality. The characteristic degeneration of anterior horn cells in the spinal cord leads to progressive muscular weakness and atrophy of the skeletal muscles. In SMA type I, the most severe form of SMA, death usually ensues by 2 years of age from respiratory failure or infection. Accurate diagnosis is now available t…

The pharmacologic closure of the patent ductus arteriosus.

The ductus arteriosus is a fetal vessel that allows most of the blood leaving the right ventricle of the heart to bypass the lungs. Fetal patency of the ductus, and its spontaneous closure after birth, is the result of a balanced interaction of locally produced and circulating mediators (of which prostaglandins seem to be the most important), and the unique structure of the vessel wall. Persistent patency of the ductus occurs in almost 60% of very low birthweight infants. A significant left-to-right shunt through the ductus increases morbidity and mortality in premature infants. As prostagland

Recurrent hope for the treatment of preterm delivery.

Preterm delivery is the major determinant of infant mortality and there is a lack of treatments for this condition. Women presenting for prenatal care with a history of a spontaneous preterm delivery were assigned 17 alpha-progesterone caproate (17P) 250 mg/week i.m. between 16 and 20 weeks of gestation or placebo until 36 weeks of gestation or delivery, whichever came first. The primary outcome was preterm delivery before 37 weeks of gestation and this occurred less often in the 17P group than in the placebo group (36.3 versus 54.9%, respectively; p < 0.001). Among the infants, there was a re…

Clinical applications of pulmonary function and graphics.

Pulmonary function and mechanics testing are emerging as a valuable tool to aid clinical decision making in the management of ventilated infants. Although there are as yet no published randomized controlled trials to suggest that pulmonary mechanics testing reduces mortality or morbidity, it has--in conjunction with clinical, radiological, and blood gas monitoring--changed neonatal ventilation from "good judgement" to "informed judgement". It is not surprising that pulmonary graphics are increasingly being used as a tool for assessment of patient status, therapeutic evaluation, and management

Cyanosis caused by a huge obstructive right ventricular fibroma.

Cardiac fibromas are rare lesions which occur more often in infants and children than in adults. These tumors are benign proliferations of connective tissue most often found in the left ventricular myocardium or septum. In an 8-month-old infant with cyanosis and progressive exertional dyspnea, a huge cardiac tumor obstructing the right ventricular outflow tract (RVOT) was diagnosed by means of 2-dimensional echocardiography and cardiac catheterization. At surgery, a whitish gray solitary tumor measuring 5.0 x 4.5 cm could be well visualized. It was nearly totally resected, and the RVOT was rec

Intrauterine growth retardation.

Intrauterine growth retardation (IUGR), which is defined as less than 10 percent of predicted fetal weight for gestational age, may result in significant fetal morbidity and mortality if not properly diagnosed. The condition is most commonly caused by inadequate maternal-fetal circulation, with a resultant decrease in fetal growth. Less common causes include intrauterine infections such as cytomegalovirus and rubella, and congenital anomalies such as trisomy 21 and trisomy 18. When IUGR is recognized, it is important to attempt to correct reversible causes, although many of the conditions resp

Neonatal infections.

Neonatal infection still carries a high mortality and morbidity. The spectrum of infection is broad and includes congenital syphilis and viral infections. Sadly, nosocomial infection is common, particularly due to coagulase negative staphylococci. Very low birth weight infants are at high risk, particularly following prolonged central venous catheterization; continuous low-dose vancomycin may offer a solution to this important problem. Early identification of infected infants can be facilitated by measurement of interleukin-6 levels. The premature newborn, deficient in white cells and humoral

[Clinical protocols concerned with disorders of fatty acid beta oxidation].

Inborn defects in fatty acid oxidation are a newly described group of diseases affecting infants and child. This disorder frequently masquerades as Reye's syndrome or sudden infant death syndrome. The known defects, now totaling 11, share many clinical similarities. Clinical manifestations include episodes of coma and hypoglycemia that are induced by fasting, or chronic, progressive muscle weakness and cardiomyopathy. Affected patients are often free of symptoms between episodes. A characteristic organic aciduria or enzyme activity in cultured skin fibroblasts permit diagnosis. Because of high

[Methods of surgical treatment of Hirschsprung's disease in newborns].

Fifty-nine newborn infants with Hirschsprung's disease were treated at the Saint-Petersburg Center of Surgery of Developmental Anomalies from 1978 to 1991. The typical rectosigmoid form was found in 47.5% and a long zone of agangliosis in 35.6% of children. Mortality rate--16.7%. According to the development of enterocolitis, three variants of the course of the disease in the newborns were distinguished. It is shown that the informativeness of the diagnostic methods increases with the growth of the infants, and the final diagnosis of Hirschsprung's disease is best established at the age of ove

Group B streptococcus amnionitis with intact membranes associated with quintuplet delivery.

Chorioamnionitis is a frequent cause of premature labour and delivery, as well as of maternal and neonatal mortality. Group B streptococcus (GBS) has emerged over the past decade as a common pathogen in the etiology of neonatal sepsis. The case of chorioamnionitis reported here is unusual for three reasons: the premature labour was associated with intact membrane and amniotic fluid infected with GBS; all 5 infants were contaminated with GBS and all infants survived. Chorioamniotitis with intact membranes raises some questions regarding the antepartum use of steroids (potent anti-inflammatory a

Surgical treatment of coarctation of the aorta in the first year of life. Immediate and late results in 35 patients.

Between February 1974 and September 1981, 35 consecutive infants younger than 12 months underwent repair of coarctation of the aorta. The mean age was 57 days and 24 were younger than 60 days. The indications for operation were congestive heart failure, metabolic acidosis and severe systemic hypertension. Only 3 cases had isolated coarctation, whereas the remaining 32 had associated anomalies. The first 4 patients were submitted to primary repair; after this, patch aortoplasty was utilized in 14 infants and in the remaining 17 patients the aortoplasty using the left subclavian artery was perfo

Unilateral pulmonary interstitial emphysema: a surgical approach to treatment.

Pulmonary interstitial emphysema is a condition that has become a significant problem chiefly as a complication of mechanical ventilation of the newborn. In its severe forms, it may compromise ventilation and restrict pulmonary blood flow to the degree that it becomes a significant cause of mortality. When its occurrence is unilateral, or predominantly so, it becomes more amenable to therapy. A number of different approaches have been described, especially for milder forms of the disease, but none has proved entirely satisfactory. A surgical approach has been used in four patients with severe

Post-hemorrhagic ventricular dilatation and hydrocephalus in preterm infants: a five-year retrospective cohort study from a tertiary referral center.

BACKGROUND: Post-hemorrhagic hydrocephalus (PHH) is a serious complication of intraventricular hemorrhage (IVH) in preterm infants and is associated with increased morbidity and mortality. Although neonatal risk factors are well described, the contribution of maternal and perinatal factors remains inconsistent across studies. OBJECTIVE: To determine the incidence, clinical characteristics, and independent predictors of PHH among preterm neonates with IVH, and to evaluate early outcomes in a tertiary referral center. METHODS: This retrospective cohort study included preterm neonates (< 34 wee

Experience with physiological based cord clamping is associated with increased effect size: a post-hoc analysis of the ABC3 trial.

BACKGROUND: A preplanned exploratory analysis of the ABC3 trial, performed in all 10 NICU centres in the Netherlands, evaluating the effect of physiological-based cord clamping (PBCC) on intact survival in very preterm infants, found a positive association between intact survival and experience with the PBCC approach. OBJECTIVE: To evaluate the effect of PBCC on intact survival in preterm infants in centres with more experience with PBCC and in centres with less experience. METHODS: A post-hoc secondary analysis was performed. Experienced and less experienced centres participating in the ABC

The landscape of congenital heart disease treated with the Ross procedure.

BACKGROUND: The Ross procedure has excellent outcomes in the pediatric population. Some series have reported age- and anatomy-dependent outcomes, but a comprehensive analysis stratified by these variables has not been reported to date. We sought to describe the landscape of congenital heart disease (CHD) treated with the Ross procedure and identify the patients best served by this operation. METHODS: Over a total of 30 years, 317 pediatric patients underwent the Ross procedure. These patients were stratified into 4 age groups-neonates (age <31 days; n = 21), infants (age 31 days to 1 year; n

Clinical characteristics of meconium aspiration syndrome in neonates with different gestational ages and the risk factors for neurological injury and death: A 9-year cohort study.

BACKGROUND: The presence of meconium is associated with gestational age, and the incidence of meconium aspiration syndrome (MAS) increases with gestational age. Our study compared the differences in the clinical characteristics of patients with MAS at different gestational ages and discussed the risk factors for neurological injury and death from MAS. METHODS: A total of 294 neonates diagnosed with MAS between 2013 and 2021 were included. Patients were divided into preterm, early-term, full-term, and late-term groups according to gestational age. We compared the patients' basic demographic, t

Neurodevelopmental Impairment at 1 Year of Age in Infants With Previous Invasive Group B Streptococcal Sepsis and Meningitis.

BACKGROUND: Invasive group B streptococcal (GBS) disease causes considerable morbidity and mortality in young infants, and 18% of GBS-meningitis survivors have moderate-to-severe neurodevelopmental impairment. However, there is a paucity of data regarding neurologic impairment following GBS sepsis. METHODS: A case-control study was undertaken in infants at 3 secondary-tertiary hospitals in Johannesburg, South Africa. Neurodevelopmental assessments were done at 1 year of age using the Denver II Developmental screening tool. A case was defined as isolation of GBS from blood or cerebrospinal flu

Congenital heart anomaly in newborns with congenital diaphragmatic hernia: a single-center experience.

OBJECTIVE: To evaluate the impact of the presence of a congenital heart anomaly (CHA) and its potential contribution to morbidity and mortality in infants with congenital diaphragmatic hernia (CDH). METHODS: In this retrospective cohort study, prenatal and postnatal data of all newborns diagnosed with CDH between January 2004 and December 2012 in a single center were reviewed. Cases were classified into two groups: those with 'isolated' CDH and those with both CDH and CHA. Patients with CHA were further subclassified into those with a major or minor CHA based on the Risk Adjustment for Congen

Bovine Surfactant Replacement Therapy in Neonates of Less than 32 Weeks' Gestation: A Multicenter Controlled Trial of Prophylaxis versus Early Treatment in China--a Pilot Study.

BACKGROUND: A domestic surfactant preparation has been used in China for a number of years. However, as for other surfactant preparations, there is debate among neonatologists regarding the optimal dose, mode of administration, and the best time of intervention. OBJECTIVE: To evaluate whether prophylactic administration of surfactant is superior to early treatment in preterm infants < 32 weeks with a high risk of respiratory distress syndrome (RDS). METHODS: We prospectively compared small premature infants (< 32 weeks) receiving 70 mg/kg bovine surfactant within 30 minutes after birth (prop

Umbilical cord blood plasma vitamin A levels in low birth weight (LBW) babies.

BACKGROUND: Role of vitamin A in reducing the mortality in infants more than six months of age is well known. Supplementing newborn infants with vitamin A within 48 hours of birth reduces infant mortality by almost a quarter, with the greatest benefit to those of low birth weight (LBW). Studies that could highlight deficiency states in neonates, particularly LBW babies by objective measurement of vitamin A levels would help in formulating the recommendations to supplement these babies with vitamin A. METHODS: Cord blood plasma vitamin A levels of 154 LBW babies with birth weight in the range …
