Prevention of joint damage in hemophilic children with early prophylaxis.

Radiological and orthopaedic outcome in severe and moderate haemophilia A and B patients undergoing long-term prophylactic treatment were pr … for those with haemophilia A and twice per week for those with haemophilia B. Elbow, knee and ankle joints were investigated at 3-4 yearly intervals according to the radiological and orthopaedic scores recommended by the World Federation of Haemophilia (WFH). The total number of joint bleedings before and after start of prophylaxis were recorded in all p …

Ultrastructural study on adhesions in internal derangement of the temporomandibular joint.

PURPOSE: This study examined the ultrastructural characteristics of adhesions in the upper joint compartment of temporomandibular joint (TMJ). MATERIALS AND METHODS: Tissue biopsy specimens of adhesions were obtained during arthroscopic operation on 36 joints in 22 patients with internal derangement (ID). The biopsy specimens were examined by light and transmission electron microscopy. RESULTS: Adhesions were grossly divided into two types based on arthroscopic observation: 1) a band-like type, which connected the articular fossa and TMJ disc, and 2) a pseudowall-like type, which faced the s…

Serotonin in an antigen-induced arthritis of the rabbit temporomandibular joint.

The aim was to investigate the joint perfusate concentration of serotonin (5-HT) in antigen-induced monoarthritis of the rabbit temporomandibular joint (TMJ) and knee joint. Thirty adult male New Zealand White rabbits, of whom eight were first used as healthy controls, were divided into TMJ and knee arthritis groups. Unilateral arthritis was induced with ovalbumin intra-articularly and the contralateral joint was sham-induced. The joints were perfused with saline (flow rate, 0.05 ml/min; 10-min intervals during 50 min) 3 weeks later and the 5-HT concentration analysed. After the perfusion, the…

Diagnostic value of quantitative sacroiliac joint scintigraphy in brucellosis.

Increased bone activity in the sacroiliac joints has been shown to be a sensitive method for detecting sacroiliitis in brucellosis. Because symmetrically increased uptake usually is difficult to detect, this study was performed to improve the sensitivity by quantifying sacroiliac joint uptake. Quantification was accomplished by normalizing sacroiliac joint activity to activity in the lumbar spine and sacrum. From rectangular regions of interest over a standardized posterior pelvic view, the ratios of the sacroiliac:lumbar spine and sacroiliac:sacrum were calculated. Abnormal sacroiliac joint u…

Osteomyelitis of the temporomandibular joint in patients with malignant otitis externa.

Malignant (invasive) otitis externa is an infection involving the external ear canal, often in elderly diabetic patients, which carries a high morbidity and mortality. It may involve widespread areas of soft tissue around the skull base, and in more advanced cases, may give rise to osteomyelitis and cranial neuropathy. We describe two patients who were treated for malignant otitis externa complicated by destructive osteomyelitis of the temporomandibular joint (TMJ). For both patients, diagnosis was made using magnetic resonance imaging (MRI), and repeat scans were employed during follow-up. Im

The potential role of doxycycline in the treatment of osteoarthritis of the temporomandibular joint.

Collagenase and gelatinase are matrix metalloproteinases (MMPs) which play an important role in tissue destruction in arthritic joints. Studies have demonstrated that tetracyclines can inhibit MMPs and prevent tissue destruction independent of their antimicrobial activity. The purpose of this pilot study is to assess the potential therapeutic role of Doxycycline in patients with advanced osteoarthritis of the temporomandibular joint (TMJ). This ongoing investigation includes patients with a diagnosis of osteoarthritis of the TMJ based on clinical and diagnostic imaging findings, symptoms (loca…

Ulnar styloid malunion with dislocation of the distal radioulnar joint.

Four patients with dorsal dislocation of the distal radioulnar joint and ulnar styloid malunion had corrective osteotomy of the ulnar styloid. Dislocation of the distal radioulnar joint was reduced in three of four patients. Subluxation persisted in the remaining patient. Wrist function improved in all patients. These results support the contention that a displaced ulnar styloid fracture with distal radioulnar joint dislocation should be reduced and internally fixed. Corrective osteotomy is recommended for malunion of the ulnar styloid associated with dislocation of the distal radioulnar joint

The importance of the distal radioulnar joint in distal radial fractures.

In a prospective study we evaluated the results of 272 distal radial fractures by their involvement of the distal radioulnar joint. Impaired function following altered anatomy at the distal radius can be explained by dysfunction of the distal radioulnar joint. Ulnar styloid avulsions contribute to a poorer result because of their effect on distal radioulnar joint function.

Dorsal chevron flap for exposure of the proximal interphalangeal joint.

We have used a dorsal V-flap, or chevron flap, to expose the proximal interphalangeal joint. A retrospective clinical study of its use in 16 patients over 5 years is presented, including indications, functional return of movement, and complications. Long-term follow-up showed an average range of active movement of 70 degrees (range, 13-84 degrees). Surgical exposure of the joint was better than other dorsal approaches and overall recovery of movement was not significantly compromised. Alternative surgical techniques such as the dorsal longitudinal split, dorsolateral split and palmar approach

Charcot's joint following Keller arthroplasty. A case report.

Neuropathic osteoarthropathy, i.e., Charcot's joint, has not previously been reported as a sequela of elective foot surgery. The authors present a challenging case of a patient with long-standing diabetes mellitus and peripheral neuropathy who developed neuropathic osteoarthropathy after a Keller arthroplasty for a recalcitrant hallux ulcer. The radiographic findings, diagnostic tests, and histopathology are discussed. Finally, the authors offer suggestions for surgeons contemplating a Keller arthroplasty for patients with peripheral neuropathy.

The role of arthroscopy in ankle and subtalar degenerative joint disease.

Treatment options for degenerative joint disease of the ankle and subtalar joints are limited. When conservative management fails, the only effective procedure is arthrodesis. With the advent of the small arthroscope and the development of better instrumentation and distraction techniques, small joint arthroscopy has gained popularity as an important diagnostic and therapeutic tool in the treatment of ankle and subtalar disorders. Although the benefits of arthroscopic ankle arthrodesis are well established, and arthroscopic subtalar arthrodesis has been described recently, the role of arthrosc…

Association between joint hypermobility syndrome and panic disorder.

OBJECTIVE: The purpose of this study was to assess whether joint hypermobility syndrome is more frequent in patients with panic disorder, agoraphobia, or both than in control subjects and, if so, to determine whether mitral valve prolapse modifies or accounts in part for the association. METHOD: A case-control study was conducted in a general teaching hospital outpatient clinic. Subjects were 99 patients, newly diagnosed and untreated, with panic disorder, agoraphobia, or both and two groups of age- and sex-matched control subjects: 99 psychiatric patients and 64 medical patients who had neve

Influence of age and gender on quantitative sacroiliac joint scintigraphy.

UNLABELLED: The value of quantitative sacroiliac joint scintigraphy for detecting sacroiliitis is controversial. Age and gender may contribute to this discordance. In previous reports, the number of control groups has been small and might not exactly reflect the change of sacroiliac/sacral (SI/S) ratios related to different age. In addition, the selection of control subjects was not strict. In most studies, care was not taken to ensure that control subjects did not have a history of back pain or any other relevant conditions. In addition, there was no requirement for a normal radiograph as a c

Experimental model of synovitis/capsulitis in the equine metacarpophalangeal joint.

OBJECTIVE: To develop and define a model of acute synovitis/capsulitis in the equine metacarpophalangeal joint (fetlock) to study clinical effects of diagnostic or therapeutic procedures. ANIMALS: 5 adult Standardbreds. PROCEDURE: Polyvinyl alcohol foam particles were injected into the left front fetlock of horses; the right front fetlock was used as a control. Horses were examined clinically and for lameness on a regular basis. Blood samples were taken to measure routine variables. Synovial fluid samples were collected from both fetlocks, and macroscopic, microscopic, and biochemical variab

[Clinical evaluation and psychological aspects of temporomandibular joint disorders].

Establishing the patient's clinical diagnosis depends on gathering as much information of the patient and his or her signs and symptoms as possible. This information can be gathered from history, physical and psychological examination, diagnostic analysis. It is also important to look upon pain as a disorder and to consider the relationship between pain and psychological factors. The differential diagnosis is constructed through a biopsychological model of illness rather than through a more traditional biomedical model of disease. To arrive at a consistently accurate clinical diagnosis in pati

Contribution of the anterolateral joint capsule to the mechanical stability of the ankle.

Published reports describe the clinical efficacy of modified Brostrom anatomic repairs, surgical procedures that involve repair and fortification of the lateral capsuloligamentous complex of the ankle for patients suffering from chronic lateral instability of this joint. A cadaveric serial sectioning study of the anterior talofibular ligament, the calcaneofibular ligament, and the remainder of the anterolateral joint capsule was completed to quantitate the contribution of the superior portion of the anterolateral joint capsule, independent of the other two structures, to the overall mechanical
