Chronic Subdural Hematoma Treatment with Embolization Versus Surgery Study (CHESS): Trial Rationale and Protocol.

BACKGROUND: Current treatment paradigms for Chronic Subdural Hematoma (CSDH) include either medical management for mildly symptomatic patients or surgical evacuation for severely symptomatic patients. Middle Meningeal Artery Embolization (MMAE) has demonstrated potential to reduce rate of recurrence and rescue surgery as an adjunctive or standalone treatment for CSDH. However, there currently lacks randomized trial data to support the safety and efficacy of standalone MMAE in CSDH patients. OBJECTIVE: The objective of Chronic Subdural Hematoma Treatment with Embolization versus Surgery Study

A mixed-methods study on spousal caregivers' burden in young-onset dementia in China: a joint display of qualitative and quantitative data.

BACKGROUND AND OBJECTIVES: Becoming spousal caregivers for persons with young-onset dementia (YOD) is an unexpected event that deviates from the normal life course trajectory, creating unique and profound challenges. Understanding their burden, its primary sources, and their coping resources is essential for developing tailored support. RESEARCH DESIGN AND METHODS: We employed a mixed-methods approach to comprehensively understand the caregiving burden experienced by 11 community-dwelling spousal caregivers of YOD. We used the Zarit Burden Interview to quantify caregiving burden and conducted

Whole-Eye Transplantation: How Far Are We From a Breakthrough?

PURPOSE: Traumatic facial injuries and resultant eye enucleation remain a devastating life-changing event for many. However, whole-eye transplantation (WET) has remained a distant goal until recently. This narrative review explores the existing literature on WET, assesses current hurdles to its success, and considers the ethical challenges to the expansion of WET programs globally. METHOD: The authors identified pertinent keywords by conducting an initial literature exploration which were subsequently used to search scientific databases. In line with the narrative methodology employed in this

Coronary Microvascular Disease Registry (CMDR): Study design and rationale.

BACKGROUND: Coronary microvascular dysfunction (CMD) is a prevalent condition among patients with cardiovascular risk factors, leading to a reduced quality of life and an increased risk of major adverse cardiovascular events. Novel invasive techniques have emerged to more accurately diagnose CMD. However, CMD's natural history remains poorly understood due to limited data. To address this knowledge gap, the Coronary Microvascular Disease Registry (CMDR) was established with the primary aim of standardizing comprehensive coronary functional testing and understanding of CMD. DESIGN: CMDR is a p

Patterns of trace elements deposition in Indo-Pacific humpback dolphin (Sousa chinensis) teeth reflect early life history: A pilot study.

Understanding early life history events within a population is imperative for developing effective conservation and management practices, particularly for vulnerable species in degraded environments with high environmental variability. Here, we first investigated the lifetime record of trace element (TE) accumulation in the teeth of Indo-Pacific humpback dolphins from the Pearl River Estuary, China, using in-situ laser ablation-inductively coupled plasma-mass spectrometry microanalysis, and further explored the suitability of teeth TEs as bioindicators of critical life stages. A total of 26 TE

Site-specific pathophysiology in a neonatal mouse model of gastroparesis.

BACKGROUND: Early-life events impact maturation of the gut microbiome, enteric nervous system, and gastrointestinal motility. We examined three regions of gastric tissue to determine how maternal separation and gut microbes influence the structure and motor function of specific regions of the neonatal mouse stomach. METHODS: Germ-free and conventionally housed C57BL/6J mouse pups underwent timed maternal separation (TmSep) or nursed uninterrupted (controls) until 14 days of life. We assessed gastric emptying by quantifying the progression of gavaged fluorescein isothiocyanate (FITC)-dextran.

The impact of comorbidities, neutrophil-to-lymphocyte ratio, and drug toxicities on quality of life in lung cancer patients receiving EGFR-TKI therapy.

BACKGROUND: Epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs) are used as the standard first-line treatment for patients with advanced EGFR-mutated non-small cell lung cancer (NSCLC). However, the impact of comorbidities and treatment toxicities on quality of life (QoL) was seldom investigated. OBJECTIVE: We aimed to investigate the association of comorbidities, adverse events (AEs), and QoL in treatment-naïve advanced NSCLC patients receiving EGFR-TKI treatments. METHODS: This multi-center prospective observational study was conducted to evaluate QoL and AEs at baselin

Patient periprocedural stress in cardiovascular medicine: friend or foe?

Stress, a disruption of homeostasis, is an unavoidable part of everyday life. In medical procedures, stress profoundly affects both operators and patients. Although the stress reaction has evolved to aid survival of physical trauma, it may also be harmful, by aggravating the baseline medical condition and/or creating new stress-related medical problems. Stress responses comprise several protective mechanisms that are particularly relevant in the clinical setting (e.g., a procoagulatory state and blood loss counteraction, preservation of blood perfusion pressure, prevention of hypoglycemia, enh

Weighing the benefits and risks of oral immunotherapy in clinical practice.

Food allergies are common and affect 6-8% of children in the United States; they pose a significant burden on the quality of life of children with allergy and their caregivers due to multiple daily restrictions. Despite the recommended dietary avoidance, reactions tend to occur due to unintentional exposure to the allergenic food trigger. Fear of accidental ingestions with potentially severe reactions, including anaphylaxis and death, creates anxiety in individuals with food allergy. Oral immunotherapy has emerged as a form of active and potentially disease-modifying treatment for common food

Cost-Effectiveness of Drug Treatment for Chinese Patients With Stage I Hypertension According to the 2017 Hypertension Clinical Practice Guidelines.

Systolic/diastolic blood pressure of 130 to 139/80 to 89 mm Hg has been defined as stage I hypertension by the 2017 Hypertension Clinical Practice Guidelines. Drug treatment is recommended for stage I hypertensive patients aged ≥65 years without cardiovascular disease in the 2017 Hypertension Clinical Practice Guidelines but not in the 2018 Chinese guidelines. However, the cost-effectiveness of drug treatment among this subgroup of Chinese patients is unclear. This study developed a microsimulation model to compare costs and effectiveness of drug treatment and nondrug treatment for the subgrou

Repeated Sport-Related Concussion Shows Only Minimal White Matter Differences Many Years After Playing High School Football.

OBJECTIVE: Multiple concussions sustained in youth sport may be associated with later-life brain changes and worse cognitive outcomes. We examined the association between two or more concussions during high school football and later-life white matter (WM) microstructure (i.e., 22-47 years following football retirement) using diffusion tensor imaging (DTI). METHOD: Forty former high school football players aged 40-65 who received 2+ concussions during high school football (N = 20), or denied concussive events (N = 20) were recruited. Participants underwent neurocognitive testing and DTI scanni

Metabolic programming in early life in humans.

An association of low birth weight with an increased risk of adult cardiovascular disease and diabetes led to the developmental origins of health and disease (DOHaD) hypothesis, which proposes that undernutrition during early development permanently 'programmes' organ structure and metabolism, leading to vulnerability to later cardio-metabolic disease. High birth weight caused by maternal gestational diabetes is also associated with later diabetes, suggesting that fetal over-nutrition also has programming effects. Post-natal factors (excess weight gain/obesity, smoking, poor diets and physical

Maternal seizures can affect the brain developing of offspring.

To elucidate the impact of maternal seizures in the developing rat brain, pregnant Wistar rats were subjected to the pilocarpine-induced seizures and pups from different litters were studied at different ages. In the first 24 h of life, blood glucose and blood gases were analyzed. (14)C-leucine [(14)C-Leu] incorporation was used to analyze protein synthesis at PN1, and Western Blot method was used to analyze protein levels of Bax, Bcl-2 and Poly(ADP-ribose) polymerase-1 (PARP-1) in the hippocampus (PN3-PN21). During the first 22 days of postnatal life, body weight gain, length, skull measures,
