The opioid epidemic and pregnancy: implications for anesthetic care.

PURPOSE OF REVIEW: This review summarizes evolving knowledge regarding adverse maternal, fetal, and neonatal effects of opioid exposure during pregnancy, and current treatment options for opioid use disorder (OUD). Maternal and fetal implications of maternal opioid maintenance with methadone and buprenorphine are described. Finally, acute and chronic pain management strategies in opioid-tolerant parturients are reviewed. RECENT FINDINGS: Opioid use among parturients has risen dramatically, with opioid use during pregnancy as high as 20%. Of women with chronic pain, most continue to take opioi

Maternal variables associated with physiologic stress and perinatal complications in preterm infants.

OBJECTIVE: Complications of prematurity may be related to dysregulation of the hypothalamic-pituitary-adrenal axis in preterm infants. Increased intrauterine exposure to cortisol may be responsible for adverse prenatal programming and subsequent dysfunction of the infant's hypothalamic-pituitary-adrenal axis. The aim of the study was to describe maternal social variables and their association with infant cortisol levels and complications of prematurity. METHODS: Preterm infants <32 weeks' gestation were recruited. Primary outcomes were development of complications of prematurity and physiolog

Maternal and infant characteristics influencing the anogenital distance and penile length in newborns.

Recent studies have suggested that maternal characteristics can affect reproductive health of offspring, possibly through pre-natal hormonal influence. Anogenital distance (AGD) is an anthropometric measure which is a sensitive reproductive endpoint of masculinisation. It provides a read-out of pre-natal androgen exposure and has been associated with several reproductive health outcomes in humans. We studied AGD and stretched penile length (SPL) in a large, racially homogenous sample of consecutive newborns to understand their association with maternal and infant characteristics. A prospective

Presymptomatic differences in Toll-like receptor function in infants who have allergy.

BACKGROUND: Microbial exposure might play a key role in allergy development, but little is known about the role of Toll-like receptors (TLRs). OBJECTIVE: This study explored the association between neonatal TLR microbial recognition/function, allergy risk (maternal allergy), and prospective allergy development. METHODS: Cord blood mononuclear cells (n = 111) were cultured either alone or with optimal concentrations of TLR ligands: lipoteichoic acid (TLR2), polyinosinicpolycytidylic acid (TLR3), LPS with IFN-gamma (TLR4), flagellin (TLR5), imiquimod R837 (TLR7), or CpG (TLR9). Immune parameters and morbidity in hard drug and human immunodeficiency virus-exposed but uninfected infants.

OBJECTIVE: To examine the association of maternal hard drug use (injection drugs, cocaine, and opiates) on lymphocyte subsets and clinical morbidity in uninfected infants who are born to human immunodeficiency virus-infected mothers who were enrolled in the Women and Infants Transmission Study (1990-2000). METHODS: Maternal hard drug use was identified by self-report and/or urine toxicology. Infant evaluations occurred at birth and at 1, 2, 4, 6, 9, 12, 18, and 24 months of age. RESULTS: A total of 401 (28%) of the 1436 uninfected infants were born to drug-using mothers. Maternal CD4 lymphoc

Maltreatment exposure is associated with metabolomic disease risk scores in children.

Exposure to maltreatment during childhood is highly prevalent and a major risk factor for cardio-metabolic disorders that emerge in adult life, as well as for earlier mortality. Metabolite profiling technologies have emerged as a promising tool to detect biochemical aberrations occurring before the onset of symptoms or diseases to elucidate pathophysiology and aid risk prediction. Here, we tested the association in children between maltreatment exposure and metabolomics-based scores that predict cardio-metabolic diseases and mortality in later life. Maltreatment exposure was assessed using the

Prenatal exposure to antiepileptic drugs and early processing of emotionally relevant sounds.

INTRODUCTION: Prenatal exposure to antiepileptic drugs (AEDs) is associated with developmental compromises in verbal intelligence and social skills in childhood. Our aim was to evaluate whether a multifeature Mismatch Negativity (MMN) paradigm assessing semantic and emotional components of linguistic and emotional processing would be useful to detect possible alterations in early auditory processing of newborns with prenatal AED exposure. MATERIAL AND METHODS: Data on AED exposure, pregnancy outcome, neuropsychological evaluation of the mothers, information on maternal epilepsy type, and a st

Obstetric emergencies and adverse maternal-perinatal outcomes in Ethiopia; A systematic review and meta-analysis.

BACKGROUND: Obstetric emergencies are life-threatening medical problems that develop during pregnancy, labor, or delivery. There are a number of pregnancy-related illnesses and disorders that can endanger both the mother's and the child's health. During active labor and after delivery, obstetrical crises can arise (postpartum). While the vast majority of pregnancies and births proceed without a hitch, all pregnancies are not without risk. Pregnancy can bring joy and excitement, but it can also bring anxiety and concern. Preterm birth, stillbirth, and low birth weight are all adverse pregnancy

A molecular basis for estrogen-induced cryptorchidism.

Male sexual differentiation relies upon testicular secretion of the hormones testosterone, Mullerian inhibiting substance, and insulin-3 (Insl3). Insl3 is responsible for testicular descent through virilization and outgrowth of the embryonic gubernaculum. In mouse, prenatal exposure to 17beta-estradiol and the nonsteroidal synthetic estrogen diethylstilbestrol (DES) disturbs the endocrine balance, causing demasculinizing and feminizing effects in the male embryo, including impaired testicular descent (cryptorchidism). In the current study, we show that maternal exposure to estrogens, including…

Pediatric Toxicology: Children Are Not Small Adults.

In pediatrics, the phrase "children are not small adults" underscores essential physiologic and pathologic differences between children and adults, particularly in clinical toxicology. Key variations include body mass, organ maturity, enzyme activities, drug metabolism, behavior, and communication abilities. Notably, over half of toxic exposures occur in children, with 76% affecting those under 5 years of age. Toxic exposures can occur at any point in childhood, including in utero from maternal drug use, and can cause lifelong effects. Effective management of pediatric overdoses involves speci

Ellagic acid alleviates nicotine-induced placental injury in pregnant rats: Histomorphometric and molecular evidence.

Maternal nicotine exposure during pregnancy is associated with placental dysfunction, oxidative stress, and impaired fetal development. Ellagic acid (EA), a naturally occurring polyphenolic compound with potent antioxidant properties, may protect placental tissues against nicotine-induced damage. This study investigated the protective effects of EA on placental histomorphometric alterations, oxidative stress, and apoptosis-related gene expression in nicotine-exposed pregnant rats. Twenty-four pregnant Wistar rats were randomly assigned to four groups (n = 6): control, EA (60mg/kg), nicotine (1

Warfarin-Induced Developmental Toxicity: Insights into Embryogenesis, Teratogenicity, and Molecular Pathways.

Warfarin is a coumarin-derived oral anticoagulant widely used for the prevention and treatment of thromboembolic disorders, particularly in patients with mechanical heart valves. The drug exerts its anticoagulant effect by inhibiting vitamin K epoxide reductase, thereby impairing γ-carboxylation of vitamin K-dependent coagulation factors. Despite its clinical efficacy, warfarin therapy is associated with a narrow therapeutic index, substantial interindividual variability in dose response, numerous drug interactions, and significant hemorrhagic risk. Maternal warfarin therapy during pregnancy i

Particulate matter exposure during pregnancy and birth outcomes: exposure windows of susceptibility and socioeconomic inequalities.

We aimed (i) to assess the relationship of pregnancy-average particulate matter (PM) exposure with birthweight, birthweight at term, low birthweight at term, small for gestational age, and preterm birth, (ii) to identify critical windows of susceptibility to PM exposure across pregnancy on birthweight and small for gestational age, and (iii) to assess the presence of socioeconomic inequalities on these associations. We established a population-based, nationwide cohort using the Spanish birth registry between 2004 and 2016 (N = 3,678,445). We estimated daily PM10 and PM2.5 concentrations for th

Pregnancy outcomes of non-tumour necrosis factor inhibitor biologic disease-modifying antirheumatic drugs among individuals with autoimmune diseases: a scoping review and proposed framework for reporting outcomes.

OBJECTIVES: Biologic disease-modifying antirheumatic drugs (bDMARDs) have transformed autoimmune disease treatment. Although the perinatal impacts of tumour necrosis factor inhibitors (TNFis) have been systematically synthesised, comparable evidence syntheses for non-TNFi bDMARDs are lacking. We conducted a scoping review to synthesise evidence on the impact of non-TNFi bDMARDs on pregnancy outcomes. METHODS: We searched Embase, MEDLINE, and CENTRAL databases in November 2023. We included studies among individuals with chronic autoimmune disease that examined non-TNFi bDMARD exposure in mothe

Transgenerational neurotoxic effects of triphenyltin on marine medaka: Impaired dopaminergic system function.

Triphenyltin (TPT), a widely used environmental contaminant in antifouling paints, is known for its neurotoxic effects. To investigate the multigenerational impacts of long-term exposure (6 weeks) to environmental concentrations of TPT (100 ng/L) on either parent, we performed mixed mating between control and exposed groups (males or females). Although there was no direct contact with TPT in the subsequent generations, both the first and second generations displayed behavioral abnormalities, including reduced activity and impaired cognitive function, with pronounced gender differences and anxi
