Oncocytic (Hürthle Cell) Carcinoma Invading the Innominate Vein: A Case Report.

Oncocytic (Hürthle cell) carcinoma is a rare subtype of differentiated thyroid carcinoma associated with increased vascular invasion, distan … sive oncocytic carcinoma with extensive angioinvasion (pT3a). Completion thyroidectomy with central neck dissection revealed tumor invasion of the left internal jugular and middle thyroid veins, requiring ligation. Despite adjuvant RAI therapy, serum thyroglobulin (Tg) levels progressively increased. Subsequent positron emission tomography and computed tomography …

Small Bowel Herniation Into a Post-bariatric Pannus After Cesarean Delivery Managed With Single-Stage Panniculectomy and Rives-Stoppa Repair: A Case Report.

Redundant pannus after massive weight loss can impair function and obscure abdominal wall pathology. Incisional hernia after cesarean delivery is uncommon, and extension of the small bowel into a post-bariatric pannus is particularly unusual. We report a 42-year-old woman with three prior cesarean deliveries and prior bariatric surgery who presented with a functionally limiting grade 3 pannus and a long-standing, nonreducible lower abdominal bulge. Computed tomography (CT) demonstrated a 12.3 cm × 5.6 cm low ventral incisional hernia with a 6.7 cm neck containing small bowel loops extending in

The foramen of Hyrtl: a neuroanatomical legacy.

Prominent 19th-century anatomist Josef Hyrtl (1810-1894) made contributions to anatomy that continue to influence contemporary medical practice. Highlighting his discovery of the foramen of Hyrtl and its relevance in neuroradiology and neurosurgery, the authors explore Hyrtl's life, career, and lasting impact. Dr. Hyrtl transformed anatomical education through innovative teaching strategies, emphasizing practical dissection and observation. His comprehensive research into comparative anatomy, the vascular system of the head and neck, and ear anatomy advanced our understanding across various me

Solitary benign thyroid nodule in an elderly female cadaver.

A thyroid nodule (TN) is a structurally discrete lesion within the thyroid gland that can be readily distinguished from the adjoining normal thyroid parenchyma. TNs are more frequently detected these days because of the widespread use of high-resolution imaging techniques for various neck pathologies. A well-circumscribed macroscopic lesion measuring 17 mm ×14 mm ×12 mm was observed in the left lobe of the thyroid gland during routine dissection performed on an embalmed elderly female cadaver for undergraduate teaching. It was firm to the touch and greyish brown in color. In the histological e

Phenotype-driven cascade screening in a Syrian family with clinically consistent multiple endocrine neoplasia type 2A.

Multiple endocrine neoplasia type 2A (MEN2A) is an autosomal dominant hereditary endocrine tumor syndrome classically associated with medull … ryonic antigen. He underwent bilateral adrenalectomy followed by total thyroidectomy with bilateral neck dissection. Histopathology confirmed bilateral pheochromocytoma and multifocal MTC with gross extrathyroidal extension and lateral cervical lymph-node metastasis. Phenotype-driven cascade screening identified a 39-year-old sister with elevated calcitonin, meta …

Treatment of Traumatic Frey Syndrome and Facial Contour Deformity Using a De-Epithelialized Free SCIP Flap: A Case Report.

Frey syndrome (FS), or gustatory sweating, is typically characterized by sweating and flushing in the parotid region during eating. While FS most commonly develops after parotidectomy, it has also been reported following traumatic causes such as submandibular gland surgery, radical neck dissection, mandibular condyle fracture, or forceps delivery. The authors present the case of a 58-year-old male who developed FS and a right-sided facial contour deformity 11 months following severe blunt maxillofacial trauma. On the basis of the diagnosis, a de-epithelialized superficial circumflex iliac arte…

Large Language Models Versus Multidisciplinary Tumor Board Decisions in Thyroid Cancer.

OBJECTIVES: Large language models (LLMs) are increasingly proposed as clinical decision-support tools; however, their agreement with real-wo … ement (κ = 0.527; 95% CI 0.359-0.684). Discordance increased in complex scenarios involving lateral neck dissection, radioactive iodine therapy, and active surveillance. CONCLUSIONS: While LLMs demonstrate promising concordance in standardized thyroid cancer management, they are best positioned as supportive decision aids-such as in MDT preparation and workflow …

SYNCHRONOUS ONCOCYTIC CARCINOMA, MEDULLARY CARCINOMA AND PAPILLARY MICROCARCINOMA OF THYROID.

Thyroid tumors are the most frequently reported endocrine malignancies. However thyroid collision tumors are rare pathological findings, representing less than 1% of thyroid cancers. In our case, a 50-year-old female patient presented with a complaint of neck swelling. During the exams, nodules were identified in both thyroid lobes. Pathologic analysis of a fine-needle aspiration biopsy(FNA) has been obtained from a 60×50 mm hypoechoic nodule raised suspicion of oncocytic carcinoma(OC). Another FNA was performed on a 17×11 mm hypoechoic solid nodule, revealing features of medullary thyroid car

Half Trans-Sylvian and Half Subtemporal Approach for Microsurgical Clipping of Basilar Apex Aneurysm.

Basilar apex aneurysms (BAAs) are considered among the most challenging pathologies in open cerebrovascular neurosurgery.1 While often managed endovascularly, complex BAAs or those in which endovascular therapy is not prudent may require microsurgical treatment. Microsurgical approaches include trans-sylvian, subtemporal, and pretemporal. Here, we present a case of a growing BAA in a 65-year-old man with a history of multiple intracranial aneurysms treated with stent-assisted coil embolization. His clinical picture was complicated by left vertebral artery dissection and occlusion during angiog

Metastatic Melanoma to the Parotid Gland: A Case Report and Review of Current Diagnostic and Treatment Approaches.

Metastatic melanoma to the parotid gland is rare and represents a significant diagnostic challenge due to its atypical presentation, often r … ed high suspicion of spindle cell melanoma, leading to an urgent total parotidectomy with selective neck dissection. Histopathological examination confirmed the diagnosis of metastatic melanoma. This case underscores the importance of considering melanoma metastasis in the differential diagnosis of parotid gland lesions, particularly in patients with a history of …

Case report: a rare case of left-sided sinus of Valsalva aneurysm with concomitant coronary artery fistula and carotid artery aneurysm.

BACKGROUND: Sinus of Valsalva aneurysms (SVAs) are rare cardiac anomalies often occurring with co-existing cardiac lesions, such as ventricular septal defects and aortic regurgitation. While often asymptomatic, SVAs can lead to severe complications, including rupture, aortic regurgitation, and dissection. Carotid aneurysms are also uncommon and pose a significant stroke risk if left untreated. CASE SUMMARY: We report the case of a 56-year-old woman who initially presented with a pulsatile right neck mass and was diagnosed with a right internal carotid artery aneurysm. Subsequent imaging revea

Noninvasive and Sensitive Biosensor for the Detection of Oral Cancer Prognostic Biomarkers.

Early detection of oral squamous cell carcinoma (OSCC) significantly enhances treatment outcomes and survival rates, with lymph node metastasis serving as a main prognostic factor. However, current clinical practices rely on TNM classification, including histological confirmation of metastatic disease in lymph nodes, often involving elective neck dissection, a procedure that can cause post-operative morbidity. Here it is shown that zinc imidazole framework-8 (ZIF-8) electrochemical biosensors can effectively distinguish non-metastatic (N0) from lymph node metastatic (N+) OSCC saliva samples. B…

Malignant melanoma located in the ureter and gallbladder: A case report and literature review.

RATIONALE: Melanoma is one of a common cutaneous malignancy. Currently, metastatic malignant melanoma is difficult to be diagnosed through imaging examinations. Furthermore, the incidence of metastatic melanoma affecting the gallbladder and ureter is exceptionally rare. PATIENT CONCERNS: A 54-year-old female was admitted to the hospital with a half-month history of left lower back pain. Correlative examination revealed an occupying lesion in the mid-left ureter and the neck of the gallbladder. DIAGNOSES: The patient was initially diagnosed with gallbladder cancer and left ureteral carcinoma

Temporalis Flap for Soft Palate Reconstruction - More than an Alternative for Patients Unsuitable for Microvascular Reconstructive Surgery.

BACKGROUND/AIM: The treatment of squamous cell carcinoma (SCC) in the oral cavity for operable patients usually consists of surgical tumor resection, unilateral or bilateral neck dissection and defect reconstruction. In addition to local flaps, multiple, particularly microsurgical, distant flaps have been developed, which are mainly considered state of the art reconstruction. However, depending on previous operations and individual patient factors, microsurgical reconstruction is sometimes not suitable.
