Basic diagnostic procedures in pulmonary disease include chest-ray (pa/lat.), screening fluoroscopy, tomography and bronchoscopy. Due to its high specificity and sensitivity computerized tomography is the most effective non-invasive examination for identifying metastatic lymph nodes in the mediastinum. Further diagnostic steps include a perfusion/ventilation scan, pulmonary angiography (with centrally located tumors), upper caval venography (in upper venous congestion), thoracoscopy (with pleural effusion to exclude pleural carcinomatosis), and cardiopulmonary function tests.

Ifosfamide was tested in a phase II study in 12 evaluable patients with advanced malignant melanoma. The drug was administered at the dose of 3 g/m2 on days one and two every 3-weeks. Seven patients were not previously treated and 5 received only minimal prior chemotherapy. Visceral involvement was present in 9 patients, and the remaining 3 had only soft tissue lesions. Forty early gastric cancers (37 cases) were treated endoscopically by diathermic polypectomy and Nd; YAG laser irradiation. In 23 cases of protruded early gastric cancer, 16 cases were treated by diathermic polypectomy only, 2 cases by Nd; YAG laser only and 5 cases underwent combination therapy with both procedures. The paper discusses the results of a histologic study of intrathoracic lymph nodes removed during surgery for lung cancer in 621 cases at different stages of metastatic spread. The gross appearance of some lymph nodes did not match their real histologic pattern: about 36% of grossly-identifiable metastatic nodes were microscopically shown to be intact whereas about 12% of grossly normal lymph nodes proved to be involved, particularly in cases of adenocarcinoma. From 1977 to 1983, 44 cases with nevus cells in the capsule and trabeculae of axillary lymph node specimens from about 7000 mastectomies performed for primary breast cancer were found. This finding was therefore less than 1%. In 3 cases, the nevus cells were present in 2 nodes, whereas in 41 cases solitary nodes were involved. In 13 cases, nevus cells were present in nodes that also contained metastatic carcinoma. Silver impregnation of reticulin fibers is very useful for the differential diagnosis of nodal metastases of breast carcinoma.

The authors have studied 43 patients operated on for medullary thyroïd carcinoma. Plasma calcitonin was measured regularly in all patients, while carcinoembryonic antigen assay was performed in only 30 patients. Calcitonin assay was found to be useful for preoperative diagnosis of medullary carcinoma, and the level of plasma calcitonin appeared to be roughly correlated with tumor extension. Curettage of lymph nodes in 36 patients with cancer of the thoracic esophagus was performed in the bilateral supraclavicular, right intrathoracic and abdominal regions. The overall metastatic ratio in lymph nodes was 52.8 percent (19 of 36).
