Association between obesity and outcomes of first-line CDK4/6 inhibitor therapy in metastatic breast cancer: a multicenter real-world study.

Obesity has been associated with inferior outcomes in early-stage breast cancer, but its clinical relevance in metastatic disease treated with CDK4/6 inhibitors remains uncertain. We retrospectively analyzed 332 patients with hormone receptor-positive/HER2-negative metastatic breast cancer who received first-line ribociclib or palbociclib plus endocrine therapy between 2018 and 2023. Patients were stratified by baseline BMI: <30 kg/m2 (n = 221) and ≥ 30 kg/m2 (n = 111). Survival outcomes were estimated using Kaplan-Meier and Cox models, and treatment-related adverse events were compared betwee

Targeting homologous recombination deficiency with intensified chemotherapy versus standard chemotherapy followed by olaparib in stage III breast cancer (SUBITO): an open-label, randomised, controlled, phase 3 trial.

BACKGROUND: Patients with stage III, human epidermal-growth-factor-receptor 2 (HER2; also known as ERBB2)-negative breast cancer with homologous recombination deficiency (HRD) had a 4-year overall survival of 35% after anthracycline-based chemotherapy versus 78% after intensified alkylating chemotherapy with autologous stem cell rescue (IACT) in a post-hoc analysis of an earlier randomised controlled trial. In this study, we aimed to prospectively assess 4-year overall survival with IACT and establish whether this approach remains superior to a contemporary HRD-targeting regimen in patients wi

Emergence of artemisinin-based combination therapy resistance markers in Plasmodium falciparum from the Brazilian tri-border region of the Guiana Shield.

Plasmodium falciparum malaria remains a public health concern in the Brazilian Amazon, particularly among mobile and remote populations. Antimalarial drug resistance threatens elimination efforts, particularly in the Guiana Shield, which has historically been linked to the spread of resistant parasites. We analyzed 99 P. falciparum isolates collected in Roraima State, northern Brazil (2016-2020), using targeted deep sequencing. Variants were assessed in pfcrt, pfmdr1, and pfk13, as well as copy number variation in pfpm2 and pfpm3, including detection of the pfpm2/3 hybrid. High coverage enable

Dolutegravir restores gut microbiota in late-stage HIV-1 unlike darunavir: an open-label, randomized clinical trial.

Late presentation of HIV-1 infection is linked to gut dysbiosis, impaired immune reconstitution, excess inflammation, immune activation, and increased morbidity and mortality. It is unclear if antiretroviral therapy initiation can reverse HIV-associated gut dysbiosis at all, or if specific antiretroviral regimens are more effective in restoring the gut microbiota than others. This has important implications for the long-term health of individuals with HIV. In this multicenter, open-label, randomized clinical trial (NCT02337322), 88 antiretroviral-naïve individuals with advanced HIV-1 infection

Simulation-Based Predictions to Support the Development of a Pediatric Fixed-Dose Combination of Dolutegravir/Emtricitabine/Tenofovir Alafenamide Fumarate.

BACKGROUND AND OBJECTIVE: Approximately 1.4 million children (aged 0-14 years) are living with human immunodeficiency virus, most infected through mother-to-child transmission. Despite the availability of combination antiretroviral treatment, only 57% of children had access to combination antiretroviral treatment in 2023, with suboptimal formulations contributing to high mortality. Limited access to age-appropriate formulations is a key barrier, particularly in low- and middle-income countries. This study presents stochastic simulations with previously published population pharmacokinetic mode

ARMED: an Australian cohort retrospective observational study to understand motivations for switching and clinical outcomes of individuals switching to dolutegravir/lamivudine.

BACKGROUND: Although clinical trials and real-world studies demonstrate the efficacy and tolerability of dolutegravir and lamivudine (DTG + 3TC), Australian real-world evidence remains limited despite differences in access to health care and prescribing context. Here, we present the motivations for switching to DTG/3TC (a fixed-dose, single-tablet regimen) and treatment outcomes. METHODS: We performed a retrospective, observational analysis of individuals with HIV with an undetectable viral load (VL; <50 copies/mL) who switched to DTG/3TC during a 24-month inclusion window from 1 December 202

Real-world feasibility of first-line palbociclib for hormone receptor-positive HER2-negative locally advanced/metastatic breast cancer in women aged 70 years and older - final results of the endocrine-sensitive cohort in the French prospective non-interventional PalomAGE study.

BACKGROUND: Breast cancer (BC) is the most commonly diagnosed cancer among women, with 30% of cases occurring at age ≥70 years (70+). Because patients 70+ are underrepresented in BC clinical trials, real-world data may help fill treatment knowledge gaps for this population. PATIENTS AND METHODS: PalomAGE is a French observational, multicentre, prospective study assessing the real-world feasibility of ET + palbociclib in women 70+ with hormonal receptor-positive/HER2-negative (/HER+/HER2-) locally advanced/metastatic BC (ABC). The primary endpoint was palbociclib permanent discontinuation rate

GNL3 SUMOylation is essential for DNA double-strand break repair by homologous recombination.

DNA double-strand break repair via homologous recombination is critical for maintaining genomic integrity and requires proper DNA end resection. Here, we identify GNL3, a nucleolar GTP-binding protein, as a key regulator of homologous recombination in human cells via SUMOylation-dependent control of DNA end resection. Expression of wild-type GNL3, but not the SUMO-defective K196R mutant, abolished DNA damage induced by knockdown of endogenous GNL3. GNL3 interacts with the BLM-DNA2 helicase-nuclease complex, promoting DNA end resection and subsequent RPA and RAD51 loading. This interaction requ

Susceptibility of Brca1(L63X/+) rat to ovarian reserve dissipation by chemotherapeutic agents to breast cancer.

BRCA1 is one of the causative genes for hereditary breast and ovarian cancer syndrome with a high risk of early-onset breast cancer. Whereas olaparib (OLA), an inhibitor of poly-ADP-ribose polymerase, has been applied as adjuvant therapy to those cancer patients, its effect on ovarian reproductive function remains unelucidated. Recently, a rat model (MUT; Brca1(L63X/+) mutation) mimicking a human BRCA1 pathogenic variant has been established. Using this model, we evaluated the effects of OLA on ovarian reproductive function in comparison with the wild-type (WT) rats. MUT showed a significantly

Palbociclib and dsRNA sensor co-operate to enhance anti-cancer effects through ER stress and modulation of immune evasion.

Cytoplasmic pattern recognition receptors (PRR) for double-stranded RNA, such as RIG-I/MDA5, are key mediators of anti-viral responses. Here we screen for synergistic drug-virotherapy combinations and find that the reovirus type III Dearing strain (Rt3D)-palbociclib combination augments oncolytic virus-induced stress responses and increases interferon production and signaling. Data from RIG-I agonist and ER stress-inducing agents further confirms the crosstalk between RNA-sensing and ER stress in inducing cancer cell death and interferon production. Combined Rt3D-palbociclib also increases inn

Hepatobiliary Adverse Events Associated With the KRAS p.G12C Inhibitor Sotorasib.

PURPOSE: The p.G12C mutation in KRAS is commonly found in many cancers and was previously untreatable until drugs like sotorasib were developed. However, up to 15% of patients treated with sotorasib have experienced hepatobiliary adverse events. To investigate whether these side effects are more common among sotorasib users, a pharmacovigilance study is necessary. METHODS: This study used the FDA adverse event reporting system (FAERS) database, a publicly available repository of reported drug adverse events, and AERSMine, an open-access pharmacovigilance tool, to investigate these adverse eve

Olaparib in treatment-refractory isocitrate dehydrogenase 1 (IDH1)- and IDH2-mutant cholangiocarcinoma: Safety and antitumor activity from the phase 2 National Cancer Institute 10129 trial.

BACKGROUND: Neomorphic isocitrate dehydrogenase (IDH) mutations lead to the accumulation of 2-hydroxyglutarate (2-HG), an oncometabolite implicated in tumor progression via inhibitory effects on alpha-ketoglutarate. Moreover, mutant IDH-dependent accumulation of 2-HG results in homologous recombination deficiency (HRD), which preclinically renders tumors sensitive to poly(adenosine diphosphate ribose) polymerase inhibitors. Here, the results of the cholangiocarcinoma (CCA) arm of the National Cancer Institute (NCI) 10129 olaparib in IDH-mutant solid tumors basket trial are reported. METHODS:

Risk factors for Overweight/Obesity among people living with HIV on antiretroviral therapy: An ambidirectional cohort study at a tertiary health facility in Zambia.

BACKGROUND: Overweight and obesity are major concerns among people living with HIV (PLWH), particularly those on integrase inhibitors, as they elevate the risk of cardiovascular diseases. However, longitudinal data on the burden and risk factors for overweight/obesity in sub-Saharan Africa (SSA) remain limited. This study aimed to estimate the incidence and identify factors associated with overweight and obesity among PLWH who switched to a dolutegravir (DTG)-based ART regimen at Livingstone University Teaching Hospital. USP28 promotes PARP inhibitor resistance by enhancing SOX9-mediated DNA damage repair in ovarian cancer.

PARP inhibitor (PARPi) resistance presents a significant challenge in ovarian cancer treatment, necessitating the development of effective therapeutic strategies to overcome this resistance and improve patient outcomes. Our study demonstrated that elevated expression of SRY-box 9 (SOX9) contributes to olaparib resistance in ovarian cancer. Mechanistically, the deubiquitinating enzyme USP28 was identified as a novel interacting partner of SOX9. USP28 inhibited the ubiquitination and subsequent degradation of SOX9, which is mediated by the E3 ubiquitin ligase FBXW7 during olaparib treatment. ChI
