Advanced meditation, sleep, and consciousness science: An emerging frontier.

Consciousness can persist, transform, and dissolve across wakefulness, sleep, and advanced meditation. Despite empirical and theoretical advances within each domain, these states are rarely examined within an integrated scientific framework. An interdisciplinary perspective reveals converging phenomena that challenge binary accounts of consciousness and highlight its graded, dynamic, and trainable nature. Here, we argue that the interface of advanced meditation, sleep, and consciousness science constitutes a promising yet underexplored frontier for understanding the structure, dynamics, and li…

Sleep disturbances in midlife women: prevalence, correlates, and treatments.

Sleep disturbances, including fragmented and poor-quality sleep and clinical insomnia, are common in midlife women and may be attributed in part to the menopause transition. Sleep disturbances often co-occur with depressive and anxiety symptoms, vasomotor symptoms, substance use, trauma history, and relationship stressors during midlife. Chronic sleep disturbance is related to poorer health and productivity. Numerous pharmacologic and nonpharmacologic treatments exist for sleep disturbance, including evidence-based cognitive-behavioral sleep interventions. Clinicians should query midlife women…

Brain oscillations and sleep microarchitecture: From EEG to fMRI.

This chapter covers the role of brain oscillations in shaping the microarchitecture of sleep and their role in physiological and pathological states. Neural oscillations (delta to gamma frequencies) are fundamental to the structure of sleep stages and play a central role in cognitive and restorative processes of the human brain. This chapter discusses how different oscillatory signatures characterize non-rapid eye movement (NREM) and rapid eye movement (REM) sleep, with slow waves dominating deep sleep and desynchronization in REM sleep. Electroencephalography (EEG) and functional magnetic res…

Light exposure and sleep architecture in real-world settings.

Light exposure can influence sleep timing, quality, and architecture, yet evidence from real-world settings remains limited. We investigated personal light exposure, quantified as melanopic equivalent daylight illuminance, and sleep parameters using consumer-grade wearables in a convenience sample of UK adults (n = 89). Participants simultaneously wore light sensors and sleep trackers for 7 days, yielding over 500 days of data, complemented by daily sleep diaries. Earlier sleep/wake timing was associated with longer-duration daytime light exposure, as well as more regular, less fragmented ligh…

HSF1-mediated Proteostasis Decline Links Aging and Sleep Disruption.

Sleep disruption increases with age and is associated with adverse age-related outcomes, yet the molecular mechanisms linking these phenomena remain unclear. Here, through integrative analysis of human and mouse transcriptomic and proteomic datasets, we identify proteostasis-related pathways whose aging trajectories align with transcriptional responses to chronic sleep disruption across tissues and cell types. In the human prefrontal cortex, gene expression exhibits coherent age-associated directional shifts. Across human peripheral blood following sleep restriction and multiple aging mouse ti…

Redefining Obstructive Sleep Apnea: Treatment in the Modern Era.

BACKGROUND: Obstructive sleep apnea (OSA) is a highly prevalent and heterogeneous disorder associated with substantial cardiometabolic morbidity. Although continuous positive airway pressure (CPAP) remains first-line therapy, long-term effectiveness is frequently limited by suboptimal adherence. Advances in airway devices, surgical techniques, neuromodulation, and pharmacologic therapies have expanded the therapeutic landscape and created opportunities for individualized, mechanism-based treatment. METHODS: We conducted a selective, narrative review with structured quantitative synthesis of r…

Cardiac Rhythm Disorders in Patients with Obstructive Sleep Apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent but often underdiagnosed sleep disorder associated with intermittent hypoxia and fragmented sleep. It has been increasingly implicated in the pathogenesis of cardiac rhythm disorders (CRDs), including atrial fibrillation (AF), bradyarrhythmia's, and ventricular arrhythmias. Despite substantial global evidence, limited Indian data exist regarding the burden and characteristics of CRDs among OSA patients. OBJECTIVE: The objective is to determine the prevalence and types of CRD in patients diagnosed with OSA and explore their correlation w…

REM Sleep Abnormalities in Children With Autism Spectrum Disorder.

Sleep disturbances are common in children with autism spectrum disorder (ASD). However, the sleep pattern changes including rapid eye movement (REM) sleep for ASD in pediatric populations remain unknown. Using polysomnography (PSG) data from the National Children's Hospital (NCH) Sleep Databank, we identified 193 children with ASD and matched them with 193 Non-ASD controls. We found children with ASD showed reduced REM sleep proportion (17.8% ± 6.4% vs. 19.1% ± 7.0%; p = 0.049) and duration (71.5 [49.5-91.0] min vs. 81.5 [52.5-98.0] min; p = 0.036), prolonged REM latency (134.5 [94.0-181.5] mi…

Hormonal contraceptives and women's sleep health: A systematic review.

Sex hormones and hormonal contraceptives influence the regulation of sleep-wake behavior. However, there are very few large-scale studies to date that have comprehensively evaluated how hormonal contraceptives influence women's sleep health. The purpose of this systematic review was to synthesize the existing research on hormonal contraceptive use and sleep in women ages 18-50. The systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement checklist. Nineteen studies were included in this review. Data were extracted and evalu…

Genetic risk scores, perceived neighborhood disorder, and sleep duration.

STUDY OBJECTIVES: Most studies of neighborhood context and sleep health emphasize direct effects and fail to account for the role of genetics. In this paper, we draw on the socioecological model to examine the interplay of genetics, neighborhood context, and sleep health. We specifically examine the independent and joint effects of genetic risk scores (GRS) and perceived neighborhood disorder on sleep duration. METHODS: We combine genomic and cross-sectional survey data from the All of Us Research Program, a non-probability sample of 22 575 adults of European ancestry living in the United Sta…

Craniofacial endotype of obstructive sleep apnea: Spatial matrix hypothesis.

This overview examines the craniofacial and myopathic endotypes of obstructive sleep apnea in relation to the spatial matrix hypothesis. Recently, attention has been drawn to the phenotype of obstructive sleep apnea since this condition includes neurologic, metabolic, craniofacial and myopathic endotypes. Most often, practitioners of dental sleep medicine focus their attention on the manipulation of the mandible for therapeutic purposes, but the etiology of the craniofacial endotype may reside elsewhere. Specifically, in line with the cranio-caudal gradient of development, consideration needs …

Shared genetic architecture between sleep traits and cardiometabolic diseases.

BACKGROUND: Sleep traits, including sleep apnoea (SA), insomnia, daytime sleepiness, and snoring, frequently co-occur with cardiometabolic diseases (CMDs), with shared genetic factors suspected to underlie these associations. However, the contribution of shared genetic determinants to these associations is not fully understood. METHODS: We conducted a genome-wide pleiotropic association study applying sequential genetic methods to identify shared genetic variants, genes, pathways and causal associations between the four sleep traits and seven CMDs, including LDSC, high-definition likelihood a…

Hypothalamic MCH neurons links tau pathology to sleep disruption.

Sleep disruption is an early and pervasive feature of Alzheimer's disease (AD), yet the circuit mechanisms linking tau pathology to sleep-wake dysfunction remain unresolved. Here, we identify melanin-concentrating hormone (MCH) neurons in the lateral hypothalamus (LH) as a critical node disrupted in tauopathy. Longitudinal EEG/EMG recordings in PS19 mice reveal progressive impairments in sleep architecture and homeostasis. Histological analyses revealed significant degeneration of both MCH neurons and the neighboring hypocretin (Hcrt) neuronal population in the LH at late stages of tauopathy. …

Tryptamine from wake-active monoaminergic neurons regulates sleep homeostasis.

Wakefulness produces sleep-promoting substances and the cerebrospinal fluid contains substances that reflect homeostatic sleep pressure. However, identities of such molecules, and the neural mechanisms for producing and sensing them, remain mysterious. Here we show that cerebrospinal fluid levels of tryptamine (TrpA) track homeostatic sleep pressure in nocturnal mice and diurnal pigs, reflecting physical activity history independently of light-dark cycles. We developed a ratiometric fluorescent sensor for TrpA and showed that TrpA is produced by wake-active monoaminergic nuclei in the dienceph…

Respiratory insufficiency and sleep impairment in facioscapulohumeral muscular dystrophy.

Respiratory involvement in facioscapulohumeral muscular dystrophy is a clinically relevant yet frequently underdiagnosed. Manifestations range from subclinical restrictive ventilatory impairment to sleep-disordered breathing and nocturnal hypoventilation, and contribute to reduced quality of life as well as increased morbidity and mortality. Restrictive patterns occur in approximately 10-45 % of patients and are primarily associated with early disease onset, greater clinical severity, spinal deformities, and loss of ambulation. Pathophysiology is multifactorial, involving respiratory muscle we…
